Direct bilirubin level rises when the liver is inflammed due to hepatitis infection or toxicity. Unless this inflammation regresses, level of bilirubin is less likely to come down. Rise in total bilirubin may be due to direct bilirubin alone or there may also be rise in indirect bilirubin too.
