I am taking clonotril 12.5 mg of torrent brand for four months for my anxiety problem. Since it is not available in my place. So can I change and take a different brand. Does it effect or cause any side effect. Pls advice.

There is a wide range of remedies that are available to treat depression in young adults. While there is no proven way of treating depression, broadly there are three approaches that is psychological, medical and natural. The key is to know the cause of depression. Once the cause is known, a qualified professional decides the course of treatment.

Psychological Treatment: Psychological treatments revolve around talking with the patient and change the way he thinks. It equips an individual to cope up with stress, depression and conflicts.

The three most popular treatment approaches are CBT, IPT and MBCT:

  1. Cognitive behavior therapy (CBT) is an approach where the behavioral pattern of the patient is analyzed and a certain change in behavior is suggested to deal with depression. CBT is a problem-solving approach where a psychologist first analyzes the problem and helps a patient to see the problem from a more positive aspect. 
  2. Interpersonal therapy (IPT) deals with problems related to personal relationships. It analyses the relationship pattern of a patient and specifically addresses them to drive away depression.

To feel sad depends on many factors such as environmental factors, changes occurring at chemical levels in brain. Try to identify the factors or events which could b a root to your sadness of mood. Dear Sir It is often difficult for people to cope up with increasing work pressure especially if the work environment is not conducive. There are certain healthy lifestyle techniques that will help you cope with stress. Do engage in stone firm of often activity at least for 30 minutrs everyday. Practice yoga and meditation. Try to spend some time everyday doing things you live like reading, listening to music etc. Remember, work is just a part of your life and not your whole life. Try not to take everything personally that happens at wish place. You will also benefit from psychological counseling sessions. Please feel free to contact me for further help.

What is Acute Stress Disorder

Acute stress reaction usually happens when some stressful events occur. Acute usually indicates that the symptoms occur quickly and do not last for a long time. Symptoms such as anxiety and stress occur when exposed to stressful events. It is believed that acute stress disorder occurs after unexpected crisis like traumatic events, serious accidents, domestic violence, sexual assaults or sudden bereavement. Acute stress reactions are commonly seen in people who have experienced major disasters or terrorist incidents.

Symptoms of Acute Stress Reaction

Symptoms of the condition develop very quickly - within minutes to hours in response to a stressful condition. Some of the symptoms are:

-   Anxiety, mood changes, emotional trauma (ups and downs), poor concentration and sleep, irritability and the desire to be alone

-   Unpleasant recurrent dreams

-    Avoiding situations and people who may cause anxiety and stress

-   Emotionally numb

-   Nausea, palpitations, headaches, abdominal pain, difficulty in breathing and chest pain are usually caused by the stress hormones like epinephrine.

Management of Acute Stress Disorder

Generally treatment is not required as the symptoms go when the stressful episode is over. Talking to family and friends or understanding the symptoms is helpful. Some people might experience extended symptoms. The following can be done:

Cognitive Behavioral Therapy (CBT): This is the therapy where a person can talk to an experienced therapist to understand the patterns and the triggers that charge the problem. The aim of the therapist is to change the way a person thinks and help him avoid the thoughts and be more realistic and calm. Usually when CBT is used for acute stress disorder patients then it is termed as trauma focused CBT.

Counselling: This is usually done when the symptoms are more severe or persistent. It is helpful as it helps to explore the way to deal with the stress and its symptoms. Psychiatrists help by counselling the people and also there are many online resources, which might be helpful.

There is a commonly held view that eating disorders are a lifestyle choice. Eating disorders are actually serious and often fatal illnesses that cause severe disturbances to a person's eating behaviors. Obsessions with food, body weight, and shape may also signal an eating disorder. 

Signs and Symptoms of Common Eating Disorders are discussed below:

Anorexia Nervosa:

People with anorexia nervosa may see themselves as overweight, even when they are dangerously underweight. People with anorexia nervosa typically weigh themselves repeatedly, severely restrict the amount of food they eat, and eat very small quantities of only certain foods. Anorexia nervosa has the highest mortality rate of any mental disorder.

Symptoms:

  • Extremely restricted eating
  • Extreme thinness (emaciation)
  • A relentless pursuit of thinness and unwillingness to maintain a normal or healthy weight
  • Intense fear of gaining weight
  • Distorted body image, a self-esteem that is heavily influenced by perceptions of body weight and shape, or a denial of the seriousness of low body weight

Other symptoms may develop over time, including:

  • Thinning of the bones (osteopenia or osteoporosis)
  • Mild anemia and muscle wasting and weakness
  • Brittle hair and nails
  • Dry and yellowish skin
  • Growth of fine hair all over the body (lanugo)
  • Severe constipation
  • Low blood pressure, slowed breathing and pulse
  • Damage to the structure and function of the heart
  • Brain damage
  • Multiorgan failure
  • Drop in internal body temperature, causing a person to feel cold all the time
  • Lethargy, sluggishness, or feeling tired all the time
  • Infertility

Bulimia Nervosa:

People with bulimia nervosa have recurrent and frequent episodes of eating unusually large amounts of food and feeling a lack of control over these episodes. This binge-eating is followed by behavior that compensates for the overeatingsuch as forced vomiting, excessive use of laxatives or diuretics, fasting, excessive exercise, or a combination of these behaviors. People with bulimia nervosa usually maintain what is considered a healthy or relatively normal weight.

Symptoms:

  • Chronically inflamed and sore throat
  • Swollen salivary glands in the neck and jaw area
  • Worn tooth enamel and increasingly sensitive and decaying teeth as a result of exposure to stomach acid
  • Acid reflux disorder and other gastrointestinal problems
  • Intestinal distress and irritation from laxative abuse
  • Severe dehydration from purging of fluids
  • Electrolyte imbalance (too low or too high levels of sodiumcalciumpotassium and other minerals) which can lead to stroke or heart attack

Binge-eating Disorder:

People with binge-eating disorder lose control over his or her eating. Unlike bulimia nervosa, periods of binge-eating are not followed by purging, excessive exercise, or fasting. As a result, people with binge-eating disorder often are overweight or obese. Binge-eating disorder is the most common eating disorder.

Symptoms:

  • Eating unusually large amounts of food in a specific amount of time
  • Eating even when you're full or not hungry
  • Eating fast during binge episodes
  • Eating until you're uncomfortably full
  • Eating alone or in secret to avoid embarrassment
  • Feeling distressed, ashamed, or guilty about your eating
  • Frequently dieting, possibly without weight loss

Unspecified Eating Disorder:

  • This happens when the behavior do not meet the criteria for any food or eating problems but still pose significant threat and problems.
  • This also happens the clinician is not able to assess whether an individual is affected by a certain disorder.

Treatment:

Treatment is available. Recovery is possible.

Eating disorders are serious health conditions that can be both physically and emotionally destructive. People with eating disorders need to seek professional help. Early diagnosis and intervention may enhance recovery. Eating disorders can become chronic, debilitating, and even life-threatening conditions.

When you begin to notice that disordered eating habits are affecting your life, your happiness, and your ability to concentrate, it is important that you talk to somebody about what you are going through.

The most effective and long-lasting treatment for an eating disorder is some form of psychotherapy or psychological counseling, coupled with careful attention to medical and nutritional needs. Ideally, this treatment should be tailored to the individual and will vary according to both the severities of the disorder and the patient's particular problems, needs, and strengths.

1. Exercises 

Everyday a bit of exercise not only keeps the body fit and healthy but also goes a long way in easing and calming the mind. You need not indulge in strenuous exercises but must make it a conscientious habit to work out a bit daily.

2. Talk to people

Though depression makes one inexorably quiet and silent, the first thing that is prescribed as a remedial measure is talk to people. Engaging in conversations with people, telling them what exactly is troubling you are proven ways to combat depression.

3. Meditate

4. Develop a support system

5. Change the daily routine

Although there is no single reason why someone may have suicidal tendencies, certain things can increase the risk. An individual is more likely to have suicidal thoughts if he has a mental health condition like depression, bipolar disorder or schizophrenia. The vulnerability also increases in cases of alcoholism, drug abuse or poor job security. It may not be possible to prevent suicidal thoughts, but keeping one's mind healthy with regular exercise, healthy eating and maintaining friendships can help one deal better with stress

Signs Pointing Towards Impending Suicide:

People who commit suicide do not want to die but find pain-bearing worst than dying. You should never take suicide talks of a person just as threats as it is the most basic sign of a person having suicidal tendencies. Here are some other signs pointing towards an impending suicide:

  • Focus on death: If you find a person only talking about the ways of dying, you should immediately start to talk to the person in private to understand the rood of his problem.
  • Buying or stocking pills: The person might be buying or stocking pills for committing suicide.
  • After-life planning: The person might start to take necessary steps to prepare for death, like updating the will, giving away stuff, calling close ones and saying goodbye or writing a suicide note.
  • Withdrawal: The person might boycott his close ones, family members, social events and family gathering and become isolated.
  • Speaking About His/Her Pain: The person might start to talk openly about his/her pain or feel like they are a burden on others.
  • Mood swings: A person might get angry too quickly or get depressed, anxious or sad and at times they may also become calm when they have decided to go with suicide.
  • Substance Abuse: The person might start drinking too much or consuming drugs once in an attempt to reduce their pain.

It can be very upsetting when someone says things which sound as if the person is considering suicide. One may not be sure what to do to help as to whether one should take talk of suicide seriously, or if intervention might make the situation worse.

Conditions in which suicide is common:

  • Mental disorders
  • Addictions to alcohol or other drugs
  • A serious physical illness
  • A major loss (such as the death of a loved one or the loss of a relationship or job)
  • Serious legal or financial problems
  • A history of trauma or abuse 

Taking action is always the best choice. One of the best things one can do if someone is feeling suicidal is to encourage them to talk about their feelings and to listen to what they say. One can provide the best help by listening and not being judgemental. If the person has been diagnosed with a mental health condition like depression, one can take advice from his care team. 

Ways to help a person having suicidal thoughts:

  • One should avoid offering solutions.
  • While listening, do not give in to the temptation of providing a solution.
  • One should ask open-ended questions so that the person remains in control and expresses his feelings.
  • On the contrary, it will offer an opportunity to talk about feelings which may reduce the risk of acting on suicidal feelings.
  • Ensure that the person is not left on his own if there is an immediate danger.
  • One cannot always tell when a loved one or friend is considering suicide, so one must be on the lookout for warning signs.

After listening to their feelings, one may feel that certain aspects of their thinking and behaviour are making their problems worse. But pointing this out will not be particularly helpful to them. Reassurance, respect and support will only help someone during these difficult periods. As a final note, one must realise that talking to someone about their feelings is not long lasting and can help them feel safe and secure temporarily.

A professional help can provide the long-term support to help someone overcome their suicidal tendencies. That will the person to deal with the primary issues behind someone's suicidal thoughts, besides offering advice and support.
