Control diabetes first and with deranged kidney function it is very difficult to titrate insulin dose, as there is a chance of low sugar so it is better to admit him and adjust sugrar level, or by calling sugar report and adjust insulin dose what is his daily urine output? U need a glucometer, start NOVOTAPID 5 UNITS BEFORE MEAL THICE DAILY IF PREMEAL SUGAR >150.

IN HYPOTHYROIDISM:--- Deficiency of thyroxine hormone leading to sluggishness, tired feeling, weight increase, slow heart beats, intolerance to cold, swelling of legs, feet and face, difficulty in concentrating and at times in speech. Treatment for hypothyroidism is giving ' THYROXINE ' tablets. And dose are according to level of thyroid for life time. Avoid iron medicines. Avoid bad habits . Avoid stress. Some Relax during work.go walk daily.avoid oily foods,junk foods. Do Tests T3,T4,TSH EVERY 3 TO 6 MONTHS. Take plenty of water, high-carbohydrate foods such as potatoes, rice, pasta and bread. Nutrient-rich foods that improve your health may also benefit your thyroid gland, antioxidant-rich fruits and vegetables more plenty of water: Blueberries, tomatoes, bell peppers, and other foods rich in antioxidants can improve overall health and benefit the thyroid gland. Eating foods high in B vitamins, like whole grains, may also help. flax oil, Selenium: Tiny amounts of selenium are needed for enzymes that make thyroid hormones to work properly. Dear lybrate user, one of the major nutritional substance required for human body is carbohydrate. We take carbohydrate in the form of rice, wheat, sugar etc. The complex molecules of carbohydrate are broken down into simple glucose molecules during digestion & released in the blood stream to be absorbed by the muscle cells. Muscle cells utilise the glucose molecules to release energy by breaking down those glucose molecules. Insulin hormone plays a vital role in the absorption of the glucose molecules by the muscle cells. In abnormal conditions, either due to less production of insulin or due to resistance to insulin, the process of absorption of glucose by the muscles comes to a stand still. The glucose molecules remains in the bloodstream which results in the increase in the blood sugar level. The failure of utilisation of the glucose molecules by the muscle cells results in production of less energy than normal condition. This is externally manifested as the fatigue & tiredness of the patient. This excess amount of blood sugar compels the kidney to function more than normal level in order to excrete the blood sugar molecules through urine. As a result the patient suffers from frequent urination. This abnormal condition is clinically known as diabetes mellitus. Once a patient is diagnosed with diabetes he has to take regular medicine to keep his blood sugar level within normal limits. Take homoeopathic mother tincture gymnema sylvestra q, 30 drops, thrice daily, after meals, in a cup of water. Also you need to walk at least 1 hour daily without any interval. Also you need to take a low carbohydrate rich diet. Avoid rice, sweet dishes, fruits, cold drinks, fast foods, alcoholic beverages, potato etc.

Appendicitis and uti can be cured by medicine or surgery. Pcod also needs needs and thyroid also can be controlled with medicines and you need not worry about studies. Ask me privately if more advice needed.

I am having type 2 diabetes since last 14 years. My latest FBS is 124 and PP2 reading is 165. HBA1C is 7.4. The medication taken by me is Gemer 2 before breakfast and dinner, Tenglyn 20 before breakfast and Pioglit 15 before lunch. I also do 45 minutes of brisk walking and exercise daily. I also have a diet as recommended by a dietician. Please advise as to whether any change in the medication is required to get the target HBA1C of less than 7.0.

Mr. Lybrate-user, looking at the details provided, it is possible to reduce your hba1c% <7, by just increasing your daily exercise by a few minutes and also by adujusting daily intake marginally. There is no need to add any additional drug. You are really maintaining blood sugar levels quite well. Keep it up. With regards.

You are not getting enough thyronorm and that is why your tsh is elevated. Please increase your thyronorm dose to 75 microgram. Also take it first thing in am and do not eat for 1/2 hour after taking the medicine. Have your tsh checked in 6 weeks. This should take care of most of your symptoms. Let us see how you do on this.

Hello how to detect if having diabetes. .wht kind of test needed to detect it. I'm having upper n mid back pain in right side since last year .cause still nt clear. Movement I can do any activity I can do. When sitting in chair or in something hard surface dat area painful. Pain sometime less sometime intense. Lybrate-userji, hypothyroidism can be effectively controlled only by taking correct dose of thyroxine hormone, dose depends upon tsh level, body weight and t4 levels. Unless correct dose is taken tsh level will not come down to desired level. A person with hypothyroidism should avoid cabbage, cauliflower, broccoli, radish, mustard, peanuts, soy beans, soy containing products and coffee. Plus should consume good deal of green leafy vegetables, salads & fruits. Use whole grain flour for chapati and roti. Should totally avoid refined flours lie maida. Also avoid high fat milk products. But basically adequate dose of thyroxine is the primary need. Thanks for the question.

Your blood sugar levels clearly show that you have developed t2dm, at a much earlier age. It would be better if you start treatment with insulin for first few months and control it better, later you can be shifted on to oral anti diabetics. The reason I am saying this is the glucotoxicity that harms the beta cells of pancreas can be better controlled and there is good possibility that beta cell activity revives rapidly with this treatment. Later insulin can be withdrawn and oral drugs can be substituted. Because of your younger age you have to be very careful in getting the sugar levels in good control. For any help you are welcome to ask me.

I was diagnostics with hypothyroid on November month where (TSH- 22.14, T4-7.2, T3-1.12). I was prescribed Thyroxine sodium tablets Eltroxin 50 mcg. After 6 weeks I had done test again where (TSH-3.59, T4-8.6, T3-1.09). And after taking tablet for 3 month my result shows (TSH-0.01, T4-10.0, T3-1.21). Result seems TSH level comes under normal level and T4 n T3 level increased gradually. Please advice. Thanks in advance.

lybrate-user, Thanks for the query. WHen on treatment with adequate dose of levothyroxine tablets (Eltroxin), TSH should be maintained between 3 to 4 mU/L, for being free from all the symptoms. In your case TSH level is much lower than expected. So you should reduce the dose by 12.5 mcg to 37.5 mcg per day. Then check TSH level after 6 weeks. Thanks.

The disease of teeth and gums in which there is bone loss is called periodontitis. It is the sixth complication of diabetes. First consult your physician and check whether you blood sugar is under normal level or not. If sugar is under normal level then start dental treatment. However you can consult a dentist, its better to bring your recent blood sugar investigation, (fasting, pp, glycated hb etc). If your sugar is not under normal values then you will be prescribed some mouthwash and medicines. If there is no emergency then dental treatment can be delayed until your sugar is at normal level.

lybrate-user, TSH value of 10.55 shows you have overt hypothyroidism and it needs to be treated right away with appropriate dose of thyroxine. There is no other option. Plus you wil have to exercise regularly so that normal weight is maintained. Thanks.

Firstly exercising is the best way to remain fit. Avoidance of fatty, oily and excessive sweets should be avoided. Maintain weight in normal range. because obesity is one more factor responsible. maintain stool habits and drink plenty of water.

Hi, I am 47 year old with pre-diabetic condition ( Fasting sugar level upto 130). Very frequently I noticed thin and white layer on penis shaft. No itching and can be easily removable. Doctor prescribed 5 days antibiotic course +Diprovat ointment mixed with Vaseline Jelly.But there is no improvement.Can you suggest cause of this and remedy aswell.
