Lybrate-user, if you are on treatmnent with thyroxine tablets, then tsh should be between 3 to 4 milli units/l for complete symptoms control. In hypothyroidism there is no cure. Treatment has to go on for life time. If tsh levels are not maintained as mentioned above then efforts to reduce weight will not be benefiial. So first get that right. Thanks for the question.

The main ingredient of fruit is glucose and fructose. Fructose turns into fat and increase insulin resistance. Fruits juice contains much sugar and it absorb in blood easily and increase sugar level. Instead of this take whole fruits.

I have thyroid problem (1.4. Doctor advise me to take medicine thyrox25. Do I need to take this medicine or lesser power. Should I take for whole life.

Mr. Lybrate-user, hypothyroidism is diagnosed when tsh level is above 10 milli units/ml. Levels between 6 to 9.9 milli units of tsh are considered as subclinical and no treatment is advocated, unless t4 levels are low and some symptoms are also present. Dose of thyroxine in hypothyroidism is dependent on tsh level and body weight. Unless the details like tsh and t4 levels are given no comments can be made.

Mr. lybrate-user, Thanks for the query. Insulin dose decided on the basis of blood glucose level both fasting and after food. Plus there are different types of insulins, it is not which one you are using. There are rapid acting and long acting insulins plus combination of both. It is given in units per dose, not as ml. Taking excess insulin is not advised. It causes hypoglycemia. Thanks.

Please keep thyroid under control as you are over weight by 10-12 kg please consult your physician and do regular exercises All weight reducing drugs are not good as none has stayed in market for few years due to severe side effects and do not take.

Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance... if you keep in touch personally.

Mr. lybrate-user, From what you are writing it is clear that you have iron deficiency (anemia), hypothyroidism and liver function is affected. It is better if you post the actual report, then I can understand the whole thing more clearly. Even recommendation of specific medications is possible only seeing things in detail. Plus I notice you have grade 1 obesity with a BMI of 32 .14 kgs/M2 (normal <23 kgs/M2). This can be a contributing factor for liver related problem. Thanks.

Liver enzymes are normal, c reactive protein is high -it can be due to infection/inflammation anywhere in the body, need not be due to heart or only due to heart in absence of specific symptoms of other organs, apo-1 is normal it is a fat+protein combo in blood that is water soluble and keeps circulating in the blood, in a way it reflects the good cholesterol which should be at a higher level to prevent blockages in the blood vessels, apo-b is very high -means it is a precursor of bad cholesterol in the blood that is responsible for blockages and plaque formation. Your dm-2 is under control with medication. You have to bring down apo-b by cutting down bad fats like transfats, oily food, etc which I am sure you are doing to control your dm. By increasing good cholesterol precursors like apo-1 & other apos except b (remember b for bad) your chances of getting bad fats on higher levels will get reduced. Take water with a spoon of honey (allowed for dm) with juice of a lemon every morning on empty stomach -it does burn bad fats, eat a spoon of good fats like desighee/ butter + a katori of assorted nuts every day (allowed for dm) cut down extra carbs in place of good calories gained.

Lybrate-user, looking at the test results given following are high - triglycerides, ggt and tsh. This means you have hypothyroidism (tsh 20.5mu/l), which needs immediate treatment. Secondly high triglycerides (tgl) also needs to be checked. Ideally tgl should be less than 150 mg. High ggt value points to a problem with liver, particularly with alcohol drinking. Thanks.

Pumpkin Extract Beneficial for Diabetic Patients

C.S.C, D.C.H, M.B.B.S
General Physician,
Pumpkin extract has insulin–like effects. It can help people with diabetes keep their blood sugar under control.

Quoting Chinese researchers that animals with drug–induced diabetes treated with pumpkin extract had lower blood glucose levels, greater insulin secretion, and more insulin–producing beta cells than diabetic rats that weren’t given the extract. This action may be due to the presence of both antioxidants and D–chiro–inositol, a molecule that mediates insulin activity.

Pumpkin extract is potentially a very good product for pre–diabetic persons, as well as those who have already developed diabetes.

Pumpkin is frequently used to treat diabetes and high blood glucose in Asia.

Hi For diabetic which medication is good normally its increase after taking food please advise.

MD - Consultant Physician
General Physician, Jaipur
There are many medications available for diabetic patients. The first and foremost thing for starting a medication is your fasting blood sugar level (fbs) and 2hr post prandial after food sugar level(ppbs). Also get your creatinine done from a laboratory and then based upon levels medication to be started.
