Fungal infection or Ring worm. When occurs in groin, called as jock itch. Usually fungal infection is common in moist areas of body like skin folds. It's mostly circular or round in appearance. May be red or black in colour. Treatment depending on the type and severity. Avoid sharing clothes, towels and soap; sweating also increases it. As it has to be differentiated from eczema and infection, its a must to see and confirm the diagnosis for proper treatment. So, please send photos of the affected area by direct online consultation for accurate diagnosis and treatment.

Hello, please use a broad spectrum sunscreen for whole face, at least thrice daily in the morning and afternoon. At night, you can use a mild depigmenting cream for a month. Make sure you don't use a steroid based combination cream.

I am 27 years old. I had cesarean for my baby birth. Since then I suffer extreme pain and thick white discharge after 5-6 days of my periods. Pain remains for whole month. Pain is too much that I am unable to sit for 2 days.

Hi, Dysmenorrhea is when cramps are a result of a medical problem such as endometriosis, white discharge is called as Leucorrhea. The home remedies for leucorrhoea include eating a healthy and nutritious diet, practicing good hygiene, exercising regularly, and using a decoction made of fenugreek seeds and basil leaves. Other home remedies include coriander seeds, mango, neem, aloe vera, okra, Indian gooseberry, and bananas. Maintaining good hygiene will also help you prevent many vaginal infections and also provide relief from the existing symptoms. Leucorrhoea is a condition where there is a white, sticky, and smelly discharge from the vagina. This discharge is generally helpful in flushing out bacteria and other harmful toxins from the body. A pale white, clear or odorless discharge is commonly seen in all women, especially during the menstrual cycle. Sometimes this discharge changes color, and has a bluish tint or is yellow, green or brown. Some women might discharge just a few drops, while others may have a continuous discharge. Estrogen imbalances can cause luecorrheoa. So once consult a nearby gynaecologist she can examine and advise.

I am having a lot of warts on my neck how can I get rid of that? Also how can I get fairer skin?

I am having a lot of warts on my neck how can I get rid of that?
Also how can I get fairer skin?
What you have is probably skin tags. Warts are due to human papilloma virus infection. Consult a dermatologist as both these conditions can be treated easily in OPD. Desire to have a lighter skin is common. However getting lighter skin tone than one’s natural tone is difficult. Sunscreens can be of a little help. Various therapies like glutathione, peels, creams, are available for this purpose. Drawback is that they are costly and need continuous maintainance as long as you desire the improved tone. Consult a dermatologist and discuss your possible options. We have seen people landing up with irreversible skin damage after the use of over the counter fairness creams. So kindly donot self medicate.

Spots on skin may be secondary to acne, sun damage, chicken pox and many more reasons. You need different depigmenting creams for clearance and maintainence. Also you should use a sunscreen regularly. The choice of creams depends on your skin type and pigmentation type. Donot self medicate and consult a dermatologist.

Hormone Replacement Therapy - Who Should Go For It?

MBBS, DNB (General Medicine)
General Physician, Delhi
Hormone Replacement Therapy - Who Should Go For It?

Hormone replacement a therapy used to treat symptoms of menopause by restoring the balance of estrogen and progesterone by administering it.

Forms of Hormonal Therapy

Local therapy which includes creams, pessaries, and rings; systemic therapy in the form of oral drugs, transdermal patches, gels, and implants. Women who do not have womb take combined estrogen and progesterone hormones in the form of pills.

Effect of Menopause and How Hormone Replacement Therapy Helps
Menopause affects urogenital system causing atrophy of mucosal lining of urethra, vagina, and vulva leading to dryness and reduced vaginal secretions. It affects bones in which metabolism takes place causing progressive bone loss. It increases the risk of heart disease. By starting with hormone replacement therapy, most of the menopausal symptoms gets relieved like hot flushes, irregular menses, night sweats, mood swings, loss of libido, osteoporosis. It is believed that women who start hormone replacement therapy may develop cancer in future, but still, research is going on.

Who should go for it?

  • Menopausal women who suffer moderate to severe hot flushes, loss of libido, urogenital problems, psychological issues such as anxiety and mood swings
  • Menopausal women who have severe joint pain or pain all over due to decreased bone mass
  • Women who got menopause early (before the age of 40)
  • Women who had their ovaries removed

Hormone replacement therapy is carefully selected for those women who are at low risk to develop heart disease and breast cancer.

Guidelines for Menopausal Women Receiving Hormone Replacement Therapy

  1. Menopausal women who have undergone hysterectomy can receive estrogen replacement alone.
  2. Menopausal women, who have a uterus, receive a combination of estrogen and progestogen to prevent endometrial cancer.
  3. In menopausal women, who experience moderate to severe vasomotor symptoms, the duration of hormone replacement therapy use will optimally be less than 5 years.
  4. For menopausal women, who suffer vaginal dryness due to less vaginal secretion, intravaginal estrogen preparation is given.
  5. Hormone replacement therapy doesn’t improve loss of libido; however, if it is disturbing most, transdermal estrogen is given.
  6. To reduce the longer use of hormone replacement therapy, lower doses of both estrogen and progestogens is suggested.

Can Hormone Replacement Therapy be given to Older Women?

Yes. Women who had menopause 10-15 years back can go for hormone replacement therapy, but the benefit is quite less compared to younger age group. Possible risk of heart disease and cancer should be kept in mind before prescribing it to older women.

Risk/Side-effects of Hormone Replacement Therapy

  1. Breast cancer
  2. Endometrial cancer
  3. Thromboembolism Heart disease
  4. Breast tenderness
  5. Nausea Bloating

Treatment Regimen

Combination hormone replacement therapy regime is given by using lower doses of estrogen and progestin.

Autistic Children - 5 Effective Ways You Can Handle Them!

Autistic Children - 5 Effective Ways You Can Handle Them!

As parents, it is important that you should arrange for an early intervention if you notice that your child is suffering from any of the below-mentioned signs as they can be an indicator of autism in your child:

  • Not being able to utter a sound even after 6 months
  • Unresponsive to stimulants, whereby the child fails to make any gestures
  • Incoherent blabbering which fails to make any sense even after the child is 24 months old

An autistic child is way more sensitive than other children. Dealing with them can be challenging for the parents but it is not impossible. To deal with an autistic child effectively, follow these tips:

  1. Early intervention is a must: As soon as you detect that your child is suffering from autism, seek help. There are various methods to treat autism like behavior therapy, speech-language therapy, play-based therapy, occupational therapy amongst others. As parents, you have to figure out which therapy is better for your child or to which therapy is your child responding to. Based on your observation and the evaluation of the therapist, you need to engage your child and encourage changes.
  2. Repeat at home what your child learns at school: Often an autistic child learns certain skills at the school but fails to apply them in other environments, both social and personal. You need to get acquainted with the kinds of therapeutic methods that are being taught to him/her and repeat them in your home or within a social milieu to encourage proper implementation.
  3. Reward them: An autistic child needs a lot of support from the parents in order to function in a healthy manner. Therefore, reward his/her positive behaviors with items that the child cherishes, like his/her favorite toys or stickers. Show the child that you care, in whichever way you find it fit.
  4. Observe the body language of your child: Autistic children will make certain gestures to convey their feelings. You need to observe your child and understand what kind of stimulus irks him/her and what kind of stimulus makes him/her happy. Based on your observation you need to keep your child away from negative spurs and bring in more positive impetuses in his/her life.
  5. Engage your child in recreational activities: An autistic child is no different from a normal child as far as his/her need for fun time is concerned. Maintain your child’s schedule regularly and allocate time for recreational activities, most preferably games. Let your child mix with other autistic children. A happy disposition will help your child turn his/her obstacles into opportunities.

How To Stay Safe While Exercising Outdoors

Exercising outdoors is always fun and refreshing for the body. It de stresses the mind and increases energy in the body. Most people prefer outdoors workout due to the pleasant weather. There are many advantages of working out outdoors but you need to follow some tips to make sure your workouts are injury free.

Here are 4 tips to make your workouts safe:

  1. Warm up: Warming up before you begin your workout is very essential to a safe workout. A good warm up increases your heart rate and improves your blood circulation. It also improves the mobility of your joints, thus reducing the risk of injury. A sample warm up may consist of shoulder rotations, jogging in place, bodyweight squats and burpees.
  2. Stretch: Stretching after your workout is essential for injury free joints. Stretching helps to ease the tension in your muscles and allows greater range of motion. Blood circulation also improves along with reducing soreness in your muscles. It lengthens the muscles and allows it to return to its natural position, thus improving posture of the body. Stretches are either classified as static and dynamic. Static stretches are stretches which are held for a fixed period whereas dynamic stretches involve moving through various postures.
  3. Limit sun exposure: Make sure you are not over exposed to the rays of the sun. This may lead to various skin complications such as sunburns, tanning and even skin cancer. Too much sun exposure may increase free radicals in the skin, leading to various skin disorders. Apply a sunscreen before you head out as this will protect your skin. Wear proper gear such as caps and full sleeved clothing to protect your body from the harmful UV rays of the sun.
  4. Wear the right footwear: If you love running on roads, then you need to take special care in selecting your footwear. Running on roads without proper cushioning may increase your risk of injuries such as shin splints and other calf injuries.
