Dengue Vs. Flu Fever & all about it...

MD - GENERAL PHYSICIAN
Integrated Medicine Specialist, Bangalore
Dengue Vs. Flu Fever & all about it...
A lot of patients come to me asking for advice on how to treat fever, how they contact it, and if Dengue, then is it fatal? A lot of time & money is lost due to the employees taking leaves on account of Viral Illness. It is the number one most common illness worldwide, and yet it cannot be eradicated, why?
⦁ Spread:Fever or flu or infuenza, in most cases is a communicable illness spread by water or airborne viruses called Hemophilus Influenzae a & b. From the name itself they love(-philus) blood(hemo-). Hence they need a body to attach to where they grow & thrive. They are found in air & droplets too, from where they are inhaled by us and then spread too, while sneezing or breathing close to any infected person.
⦁ The Flu Virus: By structure, viruses are a organism that can change their morphology as & when they desire. And the flu virus does this every 4-5 months, or every season change. Hence the immunity that the body develops while recovering from the previous season's flu attack is lost by the time the season changes again because the body does not recognize the new changed structure of the virus.
⦁ Flu vaccine: Hence the Flu vaccination is different every 6 month and must be taken every 6 months(with every new season) since it needs to be updated to immunize you to the latest viral strains. But even with the vaccine it offers you only 60-70% protection, since it is biologically impossible to predict the variations the virus undergoes and produce a response to it accordingly.
⦁ Dengue Virus: As with the case of the flu virus, the Dengue virus also loves blood, particularly Platelets. So the virus needs blood to live in, and flourish, and whenever the virus attacks, it affiliates to the platelets(cells which cause clotting and stop any bleeding), because they share the same morphology. Hence the Dengue virus penetrates the platelets, causing them to burst and die eventually.
⦁ Fever: (means high temperature> 37.5'c or 98.5'F)This is the response produced by the body to the flu virus. It means the White Blood Cells(cells that fight infections) in your body are activated and running around to fight a war with the Flu or Dengue Virus. It produces multiple other changes like shivering, feeling cold, generation of heat to make the virus unable to thrive, to ward off the virus.
⦁ Symptoms: Body pains, colds, sore throat, cough is usually suggestive of a Flu virus, as it damages/infects the passage where it enters from (nose, throat). However a fever with severe pain, described as deep/bone pain with red skin spots (rashes) all over the body, headache and vomiting can sometimes measure upto be a Dengue virus, or might even be just a flu virus.
⦁ Labs: When should you test yourself in fever? Though blood tests sometimes come negative for Dengue symptomatic patients, any patient with fever more than 2-3 days must be tested for a blood count & Dengue. But these can be falsely negative if done in the first 1-2 days of the illness, and sometimes even when the patient seems symptomatic of Dengue symptoms.
⦁ Treatment: The most common treatment any Physician would advise would be to take a Paracetamol tablets (500mg, 650mg, four times upto 1g thrice daily for adults), depending your fever and age. This is a symptomatic treatment for fever and also helps in all the body pains. In Dengue usually a patient tends to have thrombocytopenia(fall in platelets), hence needs monitoring closely for this, and if required a blood transfusion at later stages as platelets drop. A drop in platelets would mean inadequecy of the body to deal with bleeding, and hence patients need to be closely monitored. As the heat in the body (fever) causes body's water to lose, it may cause extreme lethargy, weakness and dizziness. This important issue must be solved by giving lots of (extra) water to the patient, either orally or in the form of drips (IV). Preparing water with a sachet of ORS (Electral powder) can enhance the body's natural composition of water better and help in speedy recovery.

HIV spread through Contact between broken skin, wounds, or mucous membranes and HIV-infected blood or blood-contaminated body fluids. Deep, open-mouth kissing if both partners have sores or bleeding gums and blood from the HIV-positive partner gets into the bloodstream of the HIV-negative partner. HIV is not spread through saliva.

What Causes Bladder Control Problems in Women?

What Causes Bladder Control Problems in Women?

Causes of bladder control problems in women

Urinary incontinence is the term used to describe bladder control problems that affect several people. Many think it only occurs among older, menopausal women but it actually isn't uncommon amid young and active women.

Inability to control the bladder accompanied by pain can be symptomatic of various disorders ranging from a minor infection to cancer. Fortunately, bladder cancer is rare, and bladder pain is usually not serious.

The varied causes of urinary incontinence in women are as follows:

  • Urinary tract infection: this is the most common cause of urinary incontinence and it affects women more than men due to anatomical factors. It can also happen at any age.
  • Medicinal side effect: the inability to control one's bladder may be a result of the administration of certain medicinal substances such as alpha-blockers, antidepressants, sleeping pills and various narcotics.
  • Changes in the reproductive system: pregnancy and fluctuations in hormone levels can cause bladder irritation, which leads to urinary incontinence.
  • Impacted stool: when stool gets tightly packed in the lower end of the digestive system and rectum, bladder control is affected. Chronic constipation and constraint strain on the lower intestines leads to the weakening of bladder muscles and hampers the bladder control.
  • Surgical side effects: temporary loss of control of the bladder is often a result of prior surgery and radiation therapy in the pelvic region.
  • Nerve damage and neurological disorders: strokes and spinal cord injuries have a heavy impact on bladder control and amount to urinary incontinence. Diseases such as Alzheimer's disease, Parkinson's disease and multiple sclerosis also lead to the same.
  • Health complications: disorders such as diabetes and obesity directly affect the bladder and the ability to exercise control over it.
  • Disability and impaired mobility: for women who are physically impaired and suffer from problems of arthritis, urinary incontinence is a major problem as they are unable to reach the toilet easily.
  • Bladder cancer: even though it is extremely rare, bladder cancer affects a number of women every year and is treated through surgery and chemotherapy.

Infertility in Men

MD - General Medicine
Sexologist, Delhi

About 10 percent of couples of reproductive age in the United States will have difficulty conceiving. Approximately 30 percent of cases are due to problems in man, another 30 percent due to problems in women and the remainder is unexplained causes or multiple factors involving both partners. If you had sex regularly and without using any birth control for over a year (or six months if you're over 35) without conceiving, see your doctor. Almost 90 percent of couples with infertility problems can successfully undergo fertility treatment. Read about common causes of infertility, possible treatments and how many couples become pregnant after treatment. Note that the success rates vary, since a couple can have multiple problems. 1. Obstructions A small percentage of men has a blockage in their ejaculatory duct that prevents sperm from entering the ejaculation fluid. If the vas deferens or epididymis vessels become clogged or damaged, possibly your sperm can not reach the egg of your partner. The cause of the obstruction could be an infection, injury, birth defects or a vasectomy. Possible symptoms: None. Possible solutions: surgery to repair an obstruction or reverse the vasectomy. Success rate: Depending on the problem and the type of surgery is performed about 50 to 90 percent of men have more sperm in their semen after surgery. About 20 to 65 percent of couples can conceive either through sexual intercourse or by in vitro fertilization or intracytoplasmic sperm injection. 2. Varicocele The varicocele (dilated veins, similar to varicose veins in the scrotum) increases the temperature of the testicles, which can affect sperm production. Possible symptoms: None (but can detect the problem through a physical examination). Possible solutions: surgery to repair the varicocele. Success rate: Up to 43 percent of men impregnate their partner within one year after surgery and to 69 percent do so within two years. 3. Irregular sperm If you have few sperm or none, if motility (ability to move) is low or abnormally shaped, possibly they are unable to fertilize the eggs of your partner. Possible symptoms: None. Possible solutions: fertility drugs; artificial insemination with donor sperm or with your own sperm if the quantity, shape and motility not have excessive anomaly; or intracytoplasmic sperm injection. Success rate: When fertility drugs that aim to increase egg production are combined with artificial insemination, the success rate is between 8 and 17 percent per cycle. With ICSI, the success rate is about 30 percent per cycle. 4. Sperm allergy Your body may develop antibodies that kill your own sperm, most of the time after a vasectomy, testicular torsion (where the testicle twists inside the scrotum), infection or trauma. Possible symptoms: None. Possible solutions: artificial insemination, assisted reproduction or intracytoplasmic sperm injection (drugs with corticosteroids such as prednisone are sometimes used to inhibit sperm antibodies but many doctors do not recommend them because of their side effects). Success rate: When fertility drugs that aim to increase egg production are combined with artificial insemination, the success rate is between 8 and 17 percent per cycle. With ICSI, the success rate is around 30 percent per cycle. 5. Unexplained fertility problems Your doctor may diagnose you a 'problem unexplained fertility' when you cannot clearly state the cause of your problem conceiving. Some experts believe that environmental toxins may be factors that contribute to the disorder but no confirmed no direct association between fertility and these possible problems. Possible symptoms: None. Possible solutions: fertility drugs combined with artificial insemination or assisted reproductive techniques such as in-vitro fertilization. Success rate: When fertility drugs that aim to increase egg production are combined with artificial insemination, the success rate is between 8 and 17 percent per cycle. With in vitro fertilization, about 20 to 40 percent of pregnancies end in cycles. After three years of treatment, the probability that a couple will conceive decreases by about 20 to 25 percent each year. 6. Combination fertility problems It is said that a couple has fertility problems 'combined' when both the man and the woman have a fertility problem. Possible Symptoms: Symptoms vary, depending on causes Possible Solutions: Treatments vary, depending on causes Success Rate: The percentages vary, depending on causes The objectives of the diagnostic evaluation, either exhaustive or limited, are: Determine the medical and psychological causes. Assess the severity and degree of responsibility for it. Establishing a therapeutic strategy that supports the specific diagnosis and meets the expectations of the patient and his partner.

Why You Should Exercise For Better Sex

Why You Should Exercise For Better Sex

Exercising regularly has many benefits for the body and many times these benefits can be life changing. It has been proved that doing exercise sets the foundations for a happy sex life as well. While exercising the brain releases endorphins the sex hormone. These hormones not only spice up the sex but also help in digestion, lower the heart rate, blood pressure and cortisol and ease the body.

Here is how exercising routinely can improve the relationship in bed and make a huge difference:

  1. Blood Circulation: Blood flow and the exciting sensation is very important prior to and during sex. When a person exercises regularly then there is an increase in the blood circulation and thus the blood is pumped to all parts of the body. In women who exercise, the circulation increases the fluids of the vagina which in turn helps in a quicker lubrication when in the act.
  2. Release of Endorphins: Having a clear mindset while having sex is very important. This is more important for women as they tend to over-ride things, thus working best when they have a clear mind and are able to concentrate only on the sexual act. When orgasm is achieved then endorphins are released which is basically a mood elevating chemical. It makes a person feel good and gives relaxed feeling of the mind and body along with euphoria. Endorphins are usually released during exercise, sports and sex. Along with the endorphins putting stress at bay and lifting the mood it also give a clear mind for the sexual activity.
  3. Longer and Stronger Orgasms: On average orgasm in men and women lasts for 10 and 20 seconds, respectively. With exercise one can easily extent this time, pleasure and strength of the orgasm. On contraction of the pelvic floor muscles an orgasm is achieved. When these muscles are kept strong then a longer orgasm is achieved. Sacrum is the area which supports the pelvis and spine; when it is exercised with glutes then it helps in strengthening the muscles. Also, kegels, squats, and pelvic thrusts are some of the exercises which can be done.
  4. Increases Self-confidence: A sexiest trait is that someone can show is that they feel good and confident about the way they look. Studies prove that people who exercise regularly and have good fitness levels display good confidence in their sexual acts and desirability.
  5. Exercises the Brain: It is said that a great sex usually starts in the mind and the brain is a mighty sex organ. Thus it is of utmost importance that the brain must be fed with nurturing information, so that it becomes stronger. Research backs that by simply changing the way we think can change the brain to react to situations.

It looks low. Please repeat after 48 hours to know whether there is an increase. Do an ultrasound and correlate. Low hcg indicates either late conception, or abnormal intra uterine pregnancy or pregnancy in the tubes. Please check with your gynaecologist.

Other early signs of pregnancy include fatigue, feeling bloated, frequent urination, mood swings, nausea, and tender or swollen breasts. Infertility Treatment - 2 Complications Involved

MBBS, MS - Obstetrics and Gynaecology, DNB (Obstetrics and Gynecology)
Gynaecologist, Bangalore
Infertility Treatment - 2 Complications Involved

The inability to conceive after indulging in unprotected sex is known as infertility. It can also be referred to the biological incapability of a male to cause the conception or a woman being unable to carry the pregnancy for full term. Research has shown that female problems contribute to over half of all the infertility cases while a majority of the other causes include sperm disorders.

Treatment of infertility depends on:

  1. Cause of Infertility
  2. Duration of Infertility
  3. Age of both partners
  4. Personal preferences

Causes:
Causes of infertility in women include:
1. Ovulation disorders: This is regarded as the most common cause of infertility in women. The disorders can be caused due to the following

2. Problems in fallopian tubes or uterus: Abnormalities in the uterus or fallopian tubes render the woman incapable of conceiving naturally. This might be due to:

3. Medications: There is a possibility that treatment will cure infertility. Examples include NSAIDs (non-steroidal anti-inflammatory drugs), Chemotherapy and Radiotherapy.
Treatment of infertility might involve a significant amount of psychological, physical, temporal and financial commitments.
In men, treatment is done to treat lack of healthy sperm or general sexual problems. The treatments include:

  1. Medications
  2. Surgery
  3. Change in lifestyle
  4. Sperm retrieval

Even though it is possible to restore fertility in women using only one or two therapies, a number of treatments might be required before conception is possible. Some of these treatments include:

  1. Intrauterine insemination (IUI)
  2. Stimulation of ovulation with fertility drugs
  3. Surgery to restore fertility

In situations where pregnancy does not happen spontaneously, Assisted Reproductive Technology (ART) can be used by couples to achieve pregnancy. It is a form of fertility treatment which involves the handling of sperm and egg. The entire ART team consists of psychologists, physicians, embryologists, nurses and lab technicians.

One common ART technique is In vitro fertilization (IVF). It is a process where an egg and sperms are manually combined in a laboratory dish, followed by transfer of embryo to the uterus. Some aspects involved in an IVF cycle are:

  1. Intracytoplasmic sperm injection (ICSI)
  2. Assisted hatching
  3. Donor eggs or sperm
  4. Gestational carrier

Some complications that may occur during the treatment of infertility are:

  1. Multiple pregnancy
  2. Ovarian hyperstimulation syndrome (OHSS)

If you wish to discuss about any specific problem, you can consult a Gynaecologist.
