Ovarian cysts in the fetus are caused through maternal hormonal exposure usually hcg. Duphaston or proluton usually are not given in doses that can cause them. Treatment depends on their size. As many resolve spontaneously, they only require constant monitoring. It's better to wait after delivery and then decide further course of action.

Ovarian surgery is not a major surgery but it involves few complications which are rare. Damage to other organs during the surgery is one of the biggest risks involved. Problems related to the use of anesthesia may occur.

Yes, this may have cancer. Please get a ca 125 and cect abdomen and pelvis and consult a surgical oncologist or gynaec surgeon at the earliest. You need to get that ovary removed to assess the exact nature of that cyst. Please get operated at a place, with frozen section facility, so that a decision may be taken during the surgery. If it is cancer and you have completed your family, then you may need to get both ovaries and uterus removed, alongwith surgical staging for ovarian cancer.
