Always increased sperm count is not needed. And always I don't advise varicocelectomy. Consult doctor to find out other possible causes so that we can start treatment ASAP. Even men with no sperm count in semen can father a baby from their own sperms. All the best.

Pl get a ultrasound of scrotum. If there is varicocele, you should get a microscopic varicocelectomy. Surgery by other methods sometimes result in poor sperm count.

Prolonged testicular pain occur due to over exhaustion of the organ mostly due to to excessive masturbation. It later transforms into varicocelectomy or testicular torsion. Usually varicoceles are not treated. Treatment is offered for males who have:
fertility problems (problems fathering a child), pain. There are no drugs to treat or prevent varicoceles. But pain killers (such as acetaminophen or ibuprofen) may help with pain. Additional treatment, such as varicocelectomy and varicocele embolization, might be necessary if your symptoms get worse.

The first priority should be reduction of weight. Would have to look for seminal parameters. If varicocele has resulted in reduction of testicular volume along with decreased sperm count/ motility than it is an indication for varicocelectomy. Hypertension and diabetes should be strictly controlled.
