Depending on the size and location of the arachnoid cyst, symptoms can include:
Headache.
Nausea and vomiting.
Lethargy, including excessive fatigue or low energy.
Seizures.
Visible lumps or protrusions from the head or spine.
Developmental delays.
Hydrocephalus due to obstruction of normal cerebrospinal fluid circulation.
The symptoms you have are associated with arachnoid cyst.

Functional cysts and hemorrhagic ovarian cysts usually resolve spontaneously. .
Treatment is required if the cyst grows or cause increasing pain.

Ovarian cyst size seems to be not very big if these dimensions are in mm. However the nature of the cyst needs to found out by a CT/ MRI. Based on this and the symptoms the management would be decided. Some cysts just need observation and close follow up others may need medicines to help in resolution and some would need removal.

The pms symptoms also looks like pregnancy so if you have skipped the cycle do a urine pregnancy test if it is negative then you can wait for at least 10 days from the expected date of period moreover from next month onwards tried to take clomiphene citrate tablet only if you are willing to do follicle study which is a better option otherwise there can be chance of irregular ovarian cyst forming without our knowledge.

NOT AT ALL. If it is asymptomatic then what is the need of surgery. If there is too much anxiety, get some blood tests done to rule out any malignancy.
