The 9 mm hole as you have described is likely to be a small vsd if I can guess correctly and it has nothing to do with appetite and eating. She needs to be given the foods she likes and do not force to eat.

Use snowdent tooth paste for whitening until then. Apply snowdent paste on the teeth. Wait for 10 to 15 min. Then brush your teeth with the same paste. Applying it overnight for better results.

This is used after normal brushing with any paste. Then spread it to all your teeth with soft brush. Leave it for 5 mins or so. Rinse your mouth.

These white spots are caused by dehydration of the enamel surface of your teeth. Once saliva hits the teeth they will rehydrate and the white spots will disappear.

There are minimal chances of infection with injections but once there is infection its very difficult to treat keloid have a 80-90% chance of recurrence, but if the infection persist then better to get it removed with intralesional excision with skin grafting.

Dear lybrate-user, yellowish appearance of teeth is due to (1) deposits on teeth (2) thin enamel (3) calcified pulp chambers (4) pigmentation due to food substances/ medicines
every reason of causing yellowish teeth is dealt with separately. So my advice will be to go for teleconsultation on platform preferably video so that you may get appropriate remedial advice.
