Hello Lybrate user pls always mention the age of patient. If pain is in the area where upper and lower jaw meet then assesment for any last erupting tooth is needed. If it radiates up to the temple and ears then dental xray is required to see any new or re infection with the teeth present in that area.

Hey there, 6 molar (first molar) mein bhi rct krana padega. Tabi ye gaanth theek hogi.
Rx:
* tab. Amoxiclav 625 mg twice daily for 5 days.
* tab. Lyser d thrice daily for 5 days.
* warm saline rinses rwice daily.
* tan x gum paint for gum massage twice daily.

No. The old cap can be removed in a flick by your dentist without giving anaesthesia/ injection. And a new cap will be placed in its place without injection and very easily. Its not a painful procedure. So, don't worry and go ahead with the procedure.
