You have obstruction on one side by a large stone which can potentially harm that kidney. Also such a large stone is very unlikely to pass out on its own. An appropriate treatment option would be surgical stone removal. Please consult a urologist at the earliest to get proper advice and treatment.

I will prefer stenting with Eswl for your size Kidney STONE in upper calyx.

You can try syp k-cit 15 ml thrice daily in a glass of water for 2-3 months and drink plenty of fluid daily around 4-5litres. If you have pain then you can get it removed endoscopically by a urologist by a procedure called RIRS.

You could have got rirs done in this kidney once you were undergoing PCNL. You can take plenty of fluid daily around 4-5litres and take syp k-cit 15 ml thrice daily in a glass of water for 3-4 months. Repeat USG abdomen after 4 months and see what is the status of the stones. They are not very big can come out on its own. If you have pain you can get RIRS done at any point of time.

Take plenty of fluid daily and continue same syrup for 2-3 months you can repeat USG abdomen after 3 months. If stone size is same or increased and if it is troubling you then get RIRS done by a urologist.
