This will depend on which vision you need for your occupation, i.e.;
- if you require excellent distant vision then multi-focal iol are not a good choice
- cost: whereas mono focal are covered by insurance, multi-focal require additional premium.
* best is to have: mono vision treatment, which involves the use of two mono focal lenses with different focusing distances; one eye is selected for near vision and the other for distance.

Cataracts can be easily removed surgically in nearly all patients. I would usually advise people to go for surgery if it troubles them and hampers their life, but given that you have DM I would suggest in favor of early surgery to avoid any chances of uncontrolled diabetes in the future and not being able to perform the surgery at all.
