You need to be examined by a general surgeon and proctoscopy to be done to find out what exactly it is, if it is a fissure then keep your stool soft by taking plenty of fluids orally and high fibre diet even you can take syp cremaffin 3 to 6 tsf hs, use ointment lox anally before defecation and ointment nitrogesic anally qid for 6 months.

The only way you can try is to keep away constipation and passing hard stools I suggest you to take three full meals with lot of vegetables fruits and plenty of water regularly avoid fast food. Also add stool softener in your daily routine. Sitting in warm water 4-5mint 2 times a day can help you.

I have evaluated your query thoroughly.* That depends upon which type of surgery is being performed for fissure issue.* If there is Lord`s dilatation, then under general anesthesia, gradual dilatation of anal opening with gloved fingers from 2-3-4 fingers done, to make relax the external tight sphincter from fissure pain & help to recover the fissure faster.
