Antenatal Care and Assessment - What does that mean?

Pregnancy is a special period that every woman waits with bated breath. While the happiness and excitement are there, the health of the mother and baby are extremely important as well. Incidences of complications during pregnancy and childbirth are not new. Many pregnant women suffer from preeclampsia (high blood pressure often associated with the presence of proteins in the urine), and gestational diabetes, which, left attended can be risky for both the mother and the baby. Lack of awareness and proper guidance are often found to be the main contributors, affecting the maternal and fetal health during pregnancy. The NICE (National Institute for Health and Care Excellence) suggests that complications or not, all women should be under medical guidance and assessment during pregnancy.

Antenatal Care and Assessment has been a revelation ensuring that a woman enjoys a safe and healthy pregnancy sans any complications. There are regular health check-ups, diagnostic and screening tests and prenatal counseling for the pregnant women and their families. The initiatives have been truly noteworthy, bringing about a significant reduction in the incidences of miscarriages and other pregnancy-related complications.

What does Antenatal Care and Assessment include?
Each trimester brings about changes that need care and attention. The Antenatal Care involves assessing and guiding a pregnant woman and their loved ones as the pregnancy progresses through each trimester.

  • The program aims at identifying the conditions that may pose a threat to the maternal and fetal health during pregnancy and work towards eliminating it.
  • Medical conditions such as anemia, HIV infection, Sexually Transmitted infections, stress, and fatigue can spell doom, affecting the pregnancy beyond imagination. Thus, time and again, a pregnant woman is screened for the mentioned complications.
  • The diet can play a crucial role during pregnancy. A pregnant woman should eat a nutritionally enriched diet as the fetus receives the nutrients directly from the mother. Antenatal care lays great emphasis on the diet of the pregnant woman.
  • De-worming and immunization programs are carried out to immunize the mother and the baby against harmful infections, including, tetanus and malaria.
  • Antenatal care is a complete package that includes prenatal as well as postnatal care and assessment so that the mother and the baby enjoy good health.
  • Pregnancy brings about a new phase in the life of couples. While most couples accept the changes graciously, many find it difficult to cope with the situation. Thus, the program aims at counseling such couples both mentally as well as emotionally so that things get a less stressful and challenging. The program also educates the other family members and loved ones about prenatal and postpartum care and management.

Many women have benefitted immensely from the antenatal care and assessment programs, safeguarding the mother and the baby from various health problems. In fact, efforts are being made to aware and educate the rural India about the immense health benefits associated with the antenatal care.

Antenatal Exercises: What, Why And How?

Antenatal exercises are specifically designed to prepare the body for parturition/birth. Fitness for the expectant mother includes a combination of breathing techniques, prenatal yoga or stretches, and exercises that strengthen core muscles (especially the back and pelvic floor), boost stamina and enhance energy levels.
The specially patterned breathing helps in aiding relaxation, relieving stress, and coping through contractions and labour.

Benefits of exercises in pregnancy:
1) improves blood flow - the increased oxygen is passed on to your baby
2) releases'happy hormones' which make you feel good
3) prepares your body for late pregnancy when you will carry that extra weight

4) prepares your body for childbirth - it's not called labour for nothing!
5) shorten the labor process
6) Increase the chances of a natural birth
7) decrease the need for pain relief
8) keeps your muscles toned so that they will recover more quickly after birth.

Tips and cautions:-

  • Women who were exercising regularly before pregnancy, and who are healthy during pregnancy, should be able to continue exercising as before, with slight changes depending on the trimester.
  • Women who have not been exercising before pregnancy will benefit from taking up a low-intensity program and gradually move to a higher activity level.
  • Exercise is recommended for 20 to 30 minutes each day, on most days. Most exercises are safe to perform during pregnancy if done with caution.

Prenatal, Antenatal And Postnatal Care - An Intro!

Prenatal, Antenatal And Postnatal Care - An Intro!

There are three stages of caring for an expecting mother: prenatal, intranatal, postnatal care. This is essential to ensure smooth pregnancy and labour and to keep the mother healthy after giving birth. This care is a combined effort between the doctor and the expectant couple. Here are a few things to keep in mind if you are expecting an addition to your family.

Prenatal care from the moment you realize that you are pregnant, a woman must start taking extra care of her body. This reduces the risks in your pregnancy and at the time of labour. But even before planning a pregnancy, it is important that couples should consult with the doctor, in order to avoid any complications in future. Thus you should ensure that regular check up not just during the pregnancy, but also before the pregnancy is equally important, so that your doctor can rule out complications arising due to factors, such as Thalassemia, Thyroid, Blood Sugar and PCOS, etc.

The main objectives of prenatal care are:

  1. To maintain the health of the mother
  2. Detect high risk cases
  3. Foresee complications and find ways to prevent them
  4. Reduce anxiety associated with delivery

Thus, an important part of prenatal care is to visit your gynaecologist regularly. These visits will be scheduled according to your age and stage of pregnancy. A few other points to keep in mind are:

  1. Stop smoking and stay away from passive smoking as well
  2. Stop drinking alcohol
  3. Take folic acid supplements and multivitamins
  4. Avoid contact with toxic chemicals like insecticides etc.
  5. Ensure your vaccination is up to date
  6. Avoid X-rays
  7. Do not start or stop any medication without informing your doctor first
  8. Eat healthy food and drink plenty of water
  9. Exercise regularly for half an hour daily
  10. Get plenty of rest
  11. Avoid stress
  12. Educate yourself about childbirth

Intranatal care refers to care given to the mother and baby at the time of delivery. The main objectives here are:

  1. Cleanliness
  2. Smooth delivery without injuring mother or baby
  3. Preventing complications
  4. Delivery resuscitation for the baby

There are a number of ways to deliver a baby and whatever you choose, you must ensure the presence of a doctor at your side, while delivering a baby. Your doctor will determine the position of the fetus and help you through your delivery.

Postnatal care is essential for 6-8 weeks after the baby is born. During this period, the mother goes through a number of physical and emotional changes and thus requires rest, nutrition and vaginal care. The main objectives here are:

  1. Prevent postpartum complications
  2. Restore mother to optimal health
  3. Ensure problem free breastfeeding

Sharing responsibility is essential for the health of the mother, after delivery. Get as much sleep as possible and pay attention to what you eat. Do not try and lose your pregnancy weight instantly. Schedule a checkup with your doctor six weeks after delivery to ensure your vagina has healed properly. Additionally abstain from intercourse at this time. With proper care, every stage of your pregnancy can be a beautiful experience. In case you have a concern or query you can always consult an expert & get answers to your questions!

Antenatal Maternal HIV Infection - Things You Need To Know About It!

Dr. Deore Sapana 87% (26 ratings)
MBBS
HIV Specialist, Nashik
Antenatal Maternal HIV Infection - Things You Need To Know About It!

HIV testing in pregnancy should be done early so as standard care can be taken for the pregnant women. The HIV testing must be repeated in the third trimester, usually before 36 weeks of gestation. This should be done in women with HIV seronegative and also in pregnant women who are at a high risk of developing HIV infection.

Expedited HIV tests should be performed at the time of delivery and labor and this is a must in women who have not been documented for HIV. The results of the test should be available within an hour of the test and the testing should be available 24 hours. In case the results are positive then, infant postnatal antiretroviral and intrapartum drug prophylaxis should be started immediately.

Women who have not been able to get tested for HIV at the time of labor and delivery are suggested expedited screening for HIV. Their screening can be done immediately postpartum or their babies should undergo screening. In case the infant and mother, both are positive, then infant antiretroviral drug prophylaxis should be started immediately. These mothers should avoid breastfeeding their babies, until the supplemental HIV tests are negative. In infants with positive HIV, prophylaxis should be discontinued and antiretroviral drug therapy should be started.

In case of acute HIV infection during pregnancy, that is in the intrapartum period, or during breastfeeding, initial testing can be performed with an antigen/antibody combination immunoassay. If the supplemental test is negative, then an additional test which is the virologic test (DNA, RNA) are necessary for the diagnosis of the HIV infection. If the mother is HIV positive, then this information must be documented in the infant's medical record and also communicated to the infant's care provider.

The knowledge of an antenatal maternal HIV infection allows the:

  1. Women with HIV infection to get the correct antiretroviral therapy along with prophylaxis for the infections, which might occur due to the immunocompromised state of the body. This also prevents and decreases the risk of transmission to their partners.
  2. When there is provision for antiretroviral therapy for the mother during pregnancy and labor along with antiretroviral drug prophylaxis for the baby there is less risk of perinatal transmission of the HIV.
  3. The HIV-infected women should be counseled for a cesarean delivery. The option of an elective cesarean reduces the perinatal transmission of the HIV.
  4. The HIV women should be counseled about the risk of breastfeeding. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.
