Thepray can give you a safe space to work towards resolving your concerns and make progress towards a healthier future. Please consult with your psychiatrist as your symptoms are not in remission. Stopping with them can exacerbate your thoughts and mood.

Fluoxetine shows it's effect at different doses in different individuals. One need to start at 20 mg per day, after breakfast, increase to 40 mg after 2 weeks. If needed, you can further increase by 20 mg every week, maximum up to 80 mg per day.

The treatment you are getting still need to be optimized and dose of fluvoxamine should be increased to 300 mg per day. Clonazepam need to be tapered off and stopped gradually. If needed a medicine from tca class can be added as an augmentation strategy or as an add on.

Doses need to be titrated as all of the anti OCD medications are also antidepressants. Apart from that you need exercise regularly so that your body also naturally tries to cope with depression by releasing endorphins during exercise. Endorphins are natural antidepressants and make mood better. Avoid caffeinated drinks as they will worsen anxiety and depression. Do some breathing exercises and relaxation techniques.

Mirtazapine (nassa not ssri) is far better in inducing sleep compared to venlafaxine. Venlafaxine is not superior to mirtazapine in treating adhd, ocd and anxiety. A ssri (e.g. Paroxetine, slight helpful in sleep induction too) would be a good choice if you want to shift from mirtazapine.
