The approximate daily dose of vit. D for children between is 600 i.u. Since vit d is fat soluble vitamin and gets stored in body. Proper dosage is to be decided by child specialist after proper examination. Normally children are given vit d in the form of oral drops. Sachets of 60,000 iu are given in adults after finding vit d levels in the blood. And only if found necessary.

Unfortunately, plant sources of b12 are few, especially in our country. Most vegetarian sources are fortified foods. You may however consider tofu and tempeh. For vegetarians with a b12 deficiency, usually, a supplement will be required. The dose will depend on your health status as well as your b12 levels.
