Firstly sir ,would like to know, how long have you been diagnosed with lung cancer, and are you on any supportive chemo/ radiotherapy or any domiciliary oxygen support. Depending jpon the grading, of how breathless you are with bouts of exacerbations you have, few medicines can be titrated. If not suffering from any other co- morbities ,can start with doxobid 200 mg twice a day, montair fx once at night, and nebulization with ipravent & budecort [if you feel more breathless]. The life expectancy in a metastatic lung cancer has conventionally been dismal, maybe 6 months. But with newer generation medicines like gefitinib, even metastatic cases can live upto 2-3 years. In any case, I am curious that if the disease has spread to just lymph node, why are you calling it metastatic, it may be addressed with curative intent. Please share your pet ct report for further query.

We need to look at what type of lung cancer it is whether it is small cell carcinoma of lung or non small cell carcinoma of lung
depending on the renal function and the type of malignancies further chemo to be decided.
If the renal function is not reversible then consider non renal toxic chemotherapy and that too in an experienced centres.
