Breakfast - One slice of bread + one egg + sprouts. Lunch one roti + salad bowl + dal bowl. Dinner - Rice bowl + Veg grill + beans bowl. This is just one example of how you can reduce carbs i.e by having equal amount of veg/salad/dal/sprouts/protiens etc. Also if you want to reduce carbs, cut down all maida and replace with multigrain atta. Stop white rice and take hand pound rice/brown rice/millets. This way you can reduce carbs and replace them with good protiens or complex carbs.

In t2 dm there is insulin resistance but there are no antibodies against it. Insulin resistance can be reduced by maintaining near normal body weight and a bmi of 23kg/m2. Exercising for more than 1 h a day is a good method of improving insulin resistance. Drugs like metformin and pioglitazone also help in reducing the resistance.

In t2dm there are no antibodies produced against insulin, it is the insulin resistance that is the culprit along with improper production of insulin by beta cells of pancreas. You might a drug like metformin 500 mg twice a day in addition to insulin which would reduce insulin resistance and help insulin action. Plus your strict diet and exercising for more than 1 h a day should continue.

It means sugar is passing in urine. Normally it should not. Your body is utilizing sugar to produce energy. Ketones are produced when body is not able utilize glucose because of deficiency of insulin, and energy is produced by metabolism of fats.
You need to increase the dose of insulin.

Insulin resistance invariably develops in obese and overweight patients. This also is a sign that in future that person is likely to develop diabetes. In your case with a BMI of 35.56 kgs/M2, you are already in grade 2 obesity. That is the reason for developing Insulin Resistance (IR). Weight resuction can help reduce IR and also postpone development of diabetes (if already not developed). Thanks.
