What is the treatment?

Hymenoplasty is a surgery performed to restore a ruptured hymen. Hymen is a symbol of purity in many cultures whereas scientific evidences suggest that this thin membrane was meant to keep out infection in the vagina. Although a hymen is a sign of virginity, it can get ruptured due to non-sexual reasons such as running, jumping or any other form of athletic activities; some women are also born without a hymen!

Hymenoplasty can be done in two ways – if the remnants of the hymen are present, then it can be stitched back together. The second option is to reconstruct a new hymen using tissues from the patient’s body. Along with restoring the hymen, you can also get the vaginal walls tightened. This surgery will repair the hymen, but you will experience bleeding and discomfort the next time it ruptures.

Although a lot of conservative societies emphasize the need for a virgin bride, this procedure has gained immense popularity in the less conservative societies such as in the United States and Europe.

Hymenoplasty is a very safe procedure, but you need to be very careful when you pick a doctor. Since this is done in an intimate area of the body, you need to make sure that you are very comfortable with the doctor. Do your research and choose a doctor who has sufficient experience and expertise in performing the procedure.

How is the treatment done?

First of all, you will need to schedule a consultation with the doctor to discuss the procedure and make your expectations clear and concise. The doctor will give you an insight about the surgery along with instructions to prepare for hymenoplasty.

  • The basic technique - If there are remains of the hymen, it can be stitched back together. A local or sometimes a general anesthesia will be administered to prevent pain and discomfort. The surgeon will then sew the torn parts together – the stitches are dissolvable. The entire surgery takes around 30 to 40 minutes and it is usually done on an outpatient basis.
  • The Alloplant technique – This technique is followed when the remnants cannot be stitched back together. The surgeon will insert a biomaterial into the vagina and this tear-through material will function as the hymen. This procedure takes around two hours and you will be under local anesthesia.
  • Hymen reconstruction – In this procedure, the surgeon will create a new hymen using tissues from the lip of the vagina. However, this method will require you to abstain from sexual intercourse for a minimum of three months.

Who is eligible for the treatment? (When is the treatment done?)

Usually, women seek hymenoplasty for cultural and religious reasons. In most cases, the person would have lost her virginity in an intercourse and wants to restore it before her marriage. In some cases, the girl would have lost her hymen due to non-sexual activities, but yet again due to the religious or social norms, they find it imperative to repair the hymen.

The suitable candidates are –

  • Those who have ruptured their hymen due to sex or other activities
  • Those who are above 18 years of age
  • Women who are healthy and don’t suffer from any serious illnesses

A gynecological exam is also necessary to determine if your hymen can be restored or not.

Who is not eligible for the treatment?

Women who suffer from any venereal diseases or have genital cancer are not eligible for this treatment.

What are the post-treatment guidelines?

The patient will be observed for a few hours in the recovery room after the treatment and if there are no complications, she will be relieved. The individual can resume work after 3 to 4 days. However, it is advised to begin moderate activities only after 10 days.

Are the results of the treatment permanent?

What are the alternatives to the treatment?

There are a couple of other alternatives to hymenoplasty such as –

Not many people are aware of the term Hymenoplasty. For some people, an intact hymen has a great significance (often an indication of a female virginity). However, you don't always need an intercourse for the hymen sheath (a thin tissue membrane that surrounds the external opening of the vagina) to break. Activities such as horse riding, gymnastics, swimming, to name a few, can play a pivotal role bringing about the rupture of the hymen. For long, people believed that a hymen, once ruptured, can never be restored. With the advancement of technology, this fact is slowly getting transformed into a myth. With Hymenoplasty (also called as a Hymen reconstruction surgery) one can achieve the impossible i.e reconstruction of a ruptured hymen surgically. Hymenoplasty also comes as a blessing for women born without a hymen. In this case, a Hymenoplasty aids in creating a new hymen, making life a lot easier for the concerned person. Procedure involved in a Hymenoplasty Before opting for a Hymenoplasty, one should have a proper understanding of the surgical procedure, including the pros and the cons. Thus, a consultation with the surgeon (should be experienced with impressive credentials and success rate) is of prime importance. Once you are fully prepared (physically, mentally, emotionally, and financially), the surgeon will fix a date for the surgery. Precautions one must take The following precautionary measures can work wonders, ensuring a better recovery sans any complications. Physical activities such as strenuous exercise, swimming, aerobics should be avoided for at least a few weeks or as recommended by the doctor. Do not indulge in sex immediately after Hymenoplasty. In most cases, doctors advise four weeks break from sexual intercourse following the hymen restoration surgery. Going for Epsom salt baths (sitting on it) at least twice or thrice a week can work wonders, speeding up the healing process.

What is Hymen? A hymen is a thin pinkish membrane that partially blocks the vaginal entrance. It is situated just inside the vagina about half an inch inside. It is in fact part of the vulva which is the gateway to vagina. The function of the hymen is unknown but has been proposed as a vaginal barrier against external sources of infection until puberty. What is Hymenoplasty? Hymenoplasty, which is also referred to as hymenorrhaphy, hymen reconstruction, hymen repair, hymen restoration, hymen surgery, and revirgination, is the most popular term describing surgery to reestablish the integrity of the hymen. Who Needs A Hymenoplasty? None. Repair has no direct medical benefit. Absence of the hymen or failure to bleed is not necessarily a sign of previous intercourse. Studies have shown that over 40% did not bleed at first intercourse and intact hymen has been reported in 52% with prior intercourse. Also the procedure of hymenoplasty does not guarantee bleeding on intercourse. There are many reasons why a woman might consider undergoing a hymenoplasty procedure. Some of the reasons for having hymenoplasty include: Cultural or religious reasons. In some cultures, having an intact hymen is very significant as it acts as a signifier of the chastity of a woman. Whether during sexual activity or otherwise, a woman s hymen can tear. Having cosmetic surgery to restore the hymen is a way of proving virginity and chastity. Psychological healing. If a woman has suffered rape or sexual abuse, it is very likely that her hymen will have torn. As a way to repair the psychological damage of this kind of abuse, some women choose to have hymen surgery to restore them both physically and mentally to the time before the abuse o Injury to the hymen. For some women, having an intact hymen that becomes broken during a first sexual experience is very important. In some instances, the hymen can tear before intercourse. Use of tampons and vigorous exercise can all tear the hymen, and in the case of a hymen being torn because of these activities, hymenoplasty offers a surgical solution. Revirgination. When a woman begins a relationship with a new partner but has already broken her hymen during sexual intercourse, she may wish to have her hymen surgically restructured. Timing of Hymenoplasty The timing of anticipated coitus may play a role in selecting a surgical approach. For patients who desire structural integrity and need the hymen to be intact on visual inspection, the repair should be performed 3 months before the consummation of marital vows. However, when the timing of coitus is known, and the patient desires bleeding but visual integrity is not needed, surgery is often best scheduled 3 weeks before the date. Surgical procedure The choice of procedure is decided by the surgeon based on desired outcome and time till marriage. The most common procedures are flap technique, Surgical adhesion, Luminal reduction and suture only techniques. It is performed as a day care procedure under local anesthesia with sedation or preferably under General anesthesia due to the anxiety of the procedure involved. All techniques achieve the same goal of forming adhesions between hymen remnants and narrowing the introitus. What is the Recovery After Hymenoplasty? Mild discomfort is treated with oral pain medication for a few days. Wound care consists of gentle irrigation after urination and patting after defecation. You can take shower the next day, and should not use public pools, hot tubs, and saunas for 4 weeks. If you start your menstrual cycle, it is recommended you avoid using tampons for your next 1-2 cycles to avoid irritation of the incisions while they are healing. At least couple of days should be taken off work or arrange to work from home during this time. Aggressive physical activity should be avoided for at least a week or two after surgery. Sexual intercourse should be avoided for approximately 4 weeks after a hymenoplasty. By week 6 to 8, the healing is complete, and no restrictions are needed. Risks and complications Hymenoplasty is a safe procedure and other than the usual anesthesia risks no major complications exist. Because of the use of small dissolving suture there may be mild bleeding from the surgical wound, possibly caused by walking or other activities. Failure to bleed on intercourse and repair breakdown are the only surgical complications. Do I need accompanying person? If you are major (> 18 years) there is no need of accompanying person. The secrecy and confidentiality is strictly maintained Is there scar or sign of operation? No, there are no signs visible after surgery. Initial few weeks till the sutures get absorbed there will be sutures seen on visual inspection. Once you have recovered full, you will see an intact hymen like natural one without any scar mark Can it be combined with other procedures? Hymenoplasty can be combined with labioplasty for vulval shaping or vaginoplasty when there is vaginal laxity.

Contrary to what most girls have been led to believe about the hymen being a flat tissue that covers the vagina and is ripped during penetrative intercourse, the truth is that hymen is a fringe of tissue around the vaginal opening. It is not an intact tissue draping across the vaginal wall because without any outlet for the menstrual blood, girls would get periods only after they lose their virginity. It is elastic and stretchy in most cases. Vaginal delivery makes the hymen less visible by smoothening and stretching it. Despite popular belief, the pain women experience when they have sex for the first time is not due to your hymen breaking, but because your vaginal muscles are too tense and/or you're going at it too rough and fast without adequate lubrication. Simple exercises such as walking, riding bicycles, doing splits as well as horseback riding may rip your hymen long before you have had intercourse. Hymens should not be perceived as the indicator of sexual activity because they are extremely stretchy and stay with you your entire life. Even sexually active people have hymens. It's important to do perform Kejal's exercise I. E.pelvic floor exercises 4 weekd after normal delivery to regain the pelvic floor muscle tone. So start learning and doing post'natal pelvic floor exercises to tighten up her loose vagina by daily doing 30 minutes exercises.
