You need to interrogate the cause of swollen scrotum, locally.

Yes it's quite normal to have different sizes of testes.
But is is still advised to get it checked by a surgeon.

Varicocele with abnormal sperm parameters with infertility means that you can get surgery done and start some medicines for the best possible results. It’s true that nobody can guarantee results after the surgery, but then, it’s the best way to go about it. In fact, you can also get it done laparoscopically.

Surgery is done to improve the sperm count it will not help in increasing the size of testis at this age.

It is entirely normal for one testicle to be bigger than the other.Also, one testicle (usually the left) often hangs lower than the other.

I suggest you to do a semen analysis and doppler study of scrotum.
If you have grade 3 or 4 varicocele then you need treatment. There are no complications if done by an expert.
Till then wear scrotal support and avoid hot baths.

Recurrent testicular swelling can be due to recurrent infection or due to a result of any injury happened in the past, it can also be due to Varicocele. Its serious as prolonged swelling may affect your sperm producing capacity permanently. Antibiotics wont be of help as it will provide only symptomatic improvement.
