What is the treatment?

What are the post-treatment guidelines?

Usually, this might be for a few months. There are modifications needed in your diet and your lifestyle, and you can get back to your routine work after a rest period of 2 to 3 days. Initially, you might be asked to wear a surgical cap when you step outside to avoid exposure to dirt and dust.

How long does it take to recover?

The recovery period is very minimal, and you can carry on your normal duties without any hindrance. You might be asked to take rest for a few days initially and can be advised to refrain from doing heavy work. Hair transplant is a surgical procedure that is used to move hair follicles from one location in the body to another. The part from which the hair follicles are taken is known as the “donor site” and the location where it is moved to is called the “recipient site”. Patients who undergo hair transplants often come across the terms FUE (Follicular Unit Extraction).

Follicular Unit Extraction is a procedure which involves the singular extraction of each hair follicle without any removal of strip of tissue. Random removal of hair follicle takes place which causes minimal and unnoticeable changes in the donor area. Fewer amounts of hair are transplanted as the follicles are removed one at a time which makes it beneficial for the removal of lesser hair as compared to the traditional FUT procedure.

The Procedure

In FUE, singular removal of hair follicles is done using specialized equipment that generally consists of a needle which has a diameter less than one mm. The needle is targeted at the base of the hair follicles. At this spot, a circular incision is made. Another instrument, similar to tweezers, is used to dislodge the hair follicle from the scalp.

Post- Surgery Management:

Stretches are not needed in Follicular Unit Extraction. Additionally, there is almost no post procedural discomfort. Since the occurrence of any scar is absent, healing time is generally less. However, puncture marks which have a diameter of 1 mm or less are common. These marks heal by themselves within 3 to 7 days. It takes three to six months for the hair to look natural once again. The initial scars heal quickly and daily activities can be resumed the very next day.

Advantages:

Perspective of the surgeon:

  1. Lesser manpower, when compared to FUT, is needed i.e. a single doctor with a maximum of two assistants can run a clinic.

Hello, I'm Dr Akhilendra Singh, consultant hair transplant surgeon AKS clinics. I am MD from PGI, Chandigarh and member of an Association of Hair Restoration Surgeon of India.

Hair transplant is a safe and permanent solution for baldness.Today I will discuss technique of hair transplant broadly. There are two techniques only:

One is the older technique which is known as strip techniques
Second technique is follicular unit extraction or FUE.
FUE is the newer and far better technique than the older strip technique.

So, first I will talk about strip technique: In the strip technique, we need to take a strip from the back of the head. Here we need to cut the skin along the hair strict length is approx 20 to 30 centimetres and width is 1 to 1.5 centimetres. Then we need to put suture and sutures are removed after one week.

Drawbacks of the strip technique was that,

We can take Graft only from the back of the head, i.e, the scalp. We cannot take graft we cannot utilize graph from the body like the beard or Chest. So we cannot correct the big bald area in one go.
The second problem with the strip techniques is that it is more invasive because it is cut. There is suture for there is chance of bleeding during surgery. There is a chance of little more discomfort pain after surgery which takes more time to heal.
The third problem with this trick technique that it will definitely leave a permanent linear scar in the back of the head.
Now the second technique of hair transplant is follicular unit extraction.

Here we have extract Graft one by one from the back of the head. We can also extract from beard and the body area with a specialized instrument. In this technique, there is no cut and no sutures. So practically it is painless, is scarless and suture-less technique. Since the inception of the FUE, we are doing a lot of improvement in the technique.

The latest version is direct hair transplant or direct hair implant what we known as DST. In this technique here are taken and implanted side by side so that practically there is no time where grafts are hair grafts are out of body and it increases the survival by many folds. So the bottom line is in the modern FUE or DST techniques.

Advantages of DST:

We can take the number of grafts in one go so we can cover a big bald area in one session.
It is very comfortable for the patient. Healing time is just 24hours or less than that. So he can resume duty from the very next day.
DST technique we are reducing out of body time. We are increasing the result better results, faster results without any downtime.
So there is no drawback of an FUE DST technique. The only thing it is more time-consuming as it is more tedious and needs more experience to operate.

Whenever someone thinks of getting Hair transplant done, there are multiple questions that comes to the mind starting from where should I get it done? which technique should I choose? What will be the side effects? What will be the post of period like?

The most commonly encountered type of hair loss thata a practising hair transplant surgeon sees these days is Androgenetic alopecia which is otherwise called as male pattern or female pattern baldness.

Why do we get it?

Apart from the genetic factors, theer are other factors like lifestyle changes, the stress related factors, the diet, exercise, sleeping factors etc all these affect your hair loss.

Coming on to the treatment, there are lot of non-surgical treatments as well as surgical treatments available for treating the hairloss.

Sometimes people come and ask me, whether "I am a candidate for hair transplant as I am too young" or "too old". So, the answer is yes. Anyone above the age of 18 or 19 till the age of 80-85, depending on the medical conditions, can go ahead for transplantation.

Whenever you visit a consultant plastic surgeon or a dermatologist, ideally first a scalp should be analyzed, certain investigations should be done for you to look into the cause of hairloss. If all is done and you are found to be a candidate for hair transplantation, then another question comes that which technique is suitable?

Your doctor should explain you both the techniques with the benefits and drawbacks of both the techniques. And we choose the technique according to the patient or according to the hair loss he or she is facing.

There are two techniques;

FUE - Follicular Unit Extraction (Punch technique)
FUT - Follicular Unit Transplant (Strip technique)
FUT is usually done for covering larger areas. The donor areas preserved for future settings. The transaction rate is very less because we separate each and every hair follicle under the microscope, taking care of that we don't damage the hair root.

While we talk about FUE, FUE is usually doen for smaller areas. It has other benefits like, it is a stitchless surgery. The post of period is much more comfortable as compared to the FUT technique. Although in FUT is an old techniquen and FUE is a latest technique. It doesn't mean that FUT is out dated. We select the technique acoording to the patient's condition. At times we do both the techniques together if the requirement id more.

How the procedure is done?

Procedure is done under the local anaesthesia. It takes around 7-8 hours to complete the procedure. The harvesting of the follicles is different in both the techniques. In FUT, we take a strip. In FUE, we punch out each and every hair follicle separately whereas the implantation remains the same in both the techniques.

How would be the post of period be like?

You can join your work the next day. We won't recommend any kind of bed rest post procedure. Swelling can be there for 2-3 days, you just need to wear a head band such as a tennis stars wear. BUt the head band is taken off after 2-3 days and the normal shampooing of hair is started. You can take the bath the next day. So, regarding the post of period, it is pretty much comfortable.

Which doctor should I choose? or which centre should I choose?

This is the another question that comes to patient's mind because these days its a big market and everyone is claiming that they are the best. So, in this case,

First check the qualification fo the doctor. The doctor should be a plastic surgeon or a qualified dermatologist with an experince in Hair Transplantation.
Secondly, look at the centre whether that centre has all the facilities required to deal with the emergency which might come up during the procedure.
Thirdly, you should look at the pre-op investigation which doctor has done or not because the Pre-op investigation will cater to what will happen post surgery or during the surgery. If you have been investigated well, nothing wrong is going to happen.
It is very safe and completely painless procedure. Even after the surgery, we will give you pain killers and anti-biotics and you will not face any kind of pain post procedure.

So I ll talk about hair transplantation. Alopecia or baldness is a very common problem nowadays and across younger age group as well as the old age group. Hair transplant offer a permanent solution. In this if basically understood it is the transplantation of hair from one part of the scalp to the other part. In severe cases we may need to use body hair from the axilla or the pubic region of the chest hair even the leg hair, but most commonly in about 99% of the cases we use scalp hair from the back of the head to the front to cover the bald areas. No basically there are two methods for hair transplant. It is either Follicular unit transplantation or Follicular unit exaction. The two methods the implantation are not different that is the putting the hair is almost the same is the method by which we exact the hair is different. So in Follicular unit transplantation or FUT method, we actually take a strip of the hair from the back and the donor area is stitched and then the team of technicians they separate the strip .. the individual follicles from the strip and then they are made ready for the transplantation. Whereas in Follicular Unit Extraction, the individual follicle, the bunch of follicles are extracted and one follicle may contain one hair up to three hair so they are individually extracted and implanted. So there is a myth that floats that FUT is not safe or it leaves a very bad scar and FUE is nonsurgical, so you have to get this very clear that both are surgical methods, FUT or FUE. Hair transplantation is a surgical methods not a nonsurgical procedure and the methods of FUT have improved overtime and we actually now do a trichophytic closure. So the hair the new hair in the scar actually grows through the scar. So it actually obscure the scar. You can hardly make out that a strip of hair is taken from there and in FUE also. There will be punctate scars that will be left behind. So it s just a matter of perception whether what you think is right and it actually . the choice of the procedure depends on the donor area and the recipient area and your level of baldness whether which is used. Sometimes we also use a combination of the two increase the hair yield. Now there are many, you know clinics, which offer that, you know, we offer unlimited hair transplant and see donor area is always limited so there cannot be an unlimited hair transplant and also that if a fair amount of density is expected by the person underdoing the hair transplant, you may will have to go undergo a second session to get a good density. So coverage and density are two different things. So initially we target coverage and if you want more density then of course you can a second procedure can be done. The hair that is transplanted grows .. starts growing in about four months time and the complete result can assessed after eight months to nine months time. And I the follow up period if PRP or Platelet Rich Plasma Theory is done after the hair transplant the hair growth is really better. Some medications are continued after the hair transplant also up to to certain amount of time but it s a procedure that requires six to eight hours if we are targeting a bigger coverage area and it s a procedure done under local anesthesia. When choosing a hair transplant center it is important for you to research and the qualifications of the surgeon or the person doing the hair transplant is very important because a lot of unauthorized centers that have sprung nowadays and you safety is the prime concern. As I said that it is a surgical procedure so things may get complicated so you have to choose a center which is well equipped which is well equipped to deal with emergencies if at all there is a need. Usually there is no need but then you safety is the prime concern. So understand that it is a surgical procedure and it has to be done safely and it has to be result oriented.

There are different patterns of hair loss:

Male and female. most commonly the male patterns of baldness usually I have seen in this clinic, people loose hairline. There is baldness in the crown area. We do different stuff for this thing. For medicational, the thing is manoeveric treatment. It includes derma roller therapy. Titanium 8 needles we roll into the scalp. The main intention is to create microscopic injuries so that the blood circulation increases in the scalp. This brings a lot of blood along with the oxygen, so the dominant follicles will be stimulated and it will just spring the prostration back. The thing is we use low-level laser therapy to stimulate the scalp. It is called the hair grow laser. This is for both men and women. If somebody takes this thing as a manoeuvring treatment the hair will be stimulated to grow back again. You will need several sitting to do this thing. No side effect, nothing. We just have to sit for 20 mins. Under the laser dorm and the laser, the dorm has 91 diode lasers. It is a purely professional one-- no blue, green lights like others.
We also do Platelet Rich Plasma therapy. We have a special machine here in our clinic that is a professional one refrigerated, very cold one. We have to have this machine because the platelets need a cold temperature. If suppose we are doing in a very ordinary machine, the platelets will die, so no use of this thing.

We do stem cell therapy also. We just introduce inside the scalp, stem cells which will stimulate the dormant follicles and it will change the divergent phase into _____ phase. It is follicular unit extraction, then implantation.
First we extract the follicles from the lower area of your scalp and implant into the recipient area. After recovering the follicle from the central position, we go from right to left. So, periodically we put the needles for anaesthesia. So, there are no side effects. Hair transplant, the basic technique is that, we take hairs from permanent zone which is called the donor zone. From there we are planting the hair to the area of baldness. There are two basic technilogies, called as FUT and FUE. In one there is a scar and a stitching, that is molecular transplantation, in which we take a hair bearing strip of skin from the scalp area and in another method we remove the hair with small punches, either .8 or .9mm punches. So, these are the basic two technologies used in hair transplantation.

The next thing is this procedure is done totally under local anesthesia and it is a completely safe procedure. There are various grades of baldness. Basically Hair transplant is suggested for Grade III and above and it is for people who are above 21yrs of age. The steps involved in the procedure are, first, we take out the follicles from the donor area that is permanent area under local anesthesia. Then next thing is we design the hairline and once hairline is designed we give the anesthesia in the front part, then we make holes in that area and implant the hairs. Post-op for around three weeks you have to take care of your hairs. For initial one week there will be pain killer antibiotics, after that you have to continue medicines for the growth of your hair. You will start seeing the results after around 3 months and the full result will come by 9 months to one year.
