My 4 year old daughter is not speaking to anyone outside home since last one year. At home she is very active, speaks very well to us, very talkative. She is going to play outside home with friends since last 4 months but doesn't talk to them but play with them. No one heard her voice. My neighbors daily says to her why are you not speaking. We also tell her everyday to speak but she says I feel shy. She has taken admission in school in nursery class. Since last 3 days she is going to school but never speaks there, never eat her lunch, never drink water. Her teacher try to make her eat but she doesn't open her mouth. We are a nuclear family, husband,wife and daughter. I am working woman. My husband always fights with me. I scold my daughter many times when she did any mistakes. One day I went to a sadhu baba with my daughter when she was 3 years old to solve our conflict .then he put his hand on my head ,said something in my ear. Seeing this my daughter screamed loudly. What made my daughter so much shy and scared I don't understand. Please help me. What should I do.

It appears like you have panic disorder and you are right now on correct medications. May be dose needs titration. If you are tolerating well paeroxetine 12.5 and if if you have tried it for more than 1 month. Better to increase the dose to 25 mg once at night. In case of any anticipatory anxiety and when. i am dealing with mental illness from last 8 years and tried treatments from all top notch psychiatrist's of the city .ate kilo's of medicines which are some serious, snri's, tricyclic ,tetracyclines, maoi's, clonazepam, alprazolam, etizolam,lamotrigine, sodium valproate, lithium, quetiapine, aripiprazole, olanzapine etc. But these all medicines and psychiatrist's failed to bring my mental well being back. I am dealing with anhedonia, apathy ,disordered thinking, inability to do goal directed activities ,bed ridden ,i don't brush my teeth, I don't bath ,i am doing nothing in my life ,i have no career and super hopeless about my future ,dull facial expression, highly demotivated, suicidal (i have attempted twice ,one with carbon monoxide poisoning and second with 50 mg clonazepam+200 mg zolpidem+750 ml alcohol bottle+20 cigarettes, my purpose was to die from respiratory depression, but I failed ).now to whichever doctor I will go ,i will ask him will you give me ect ,ketamine therapy ,rtms or not.

congratulations for continuing your interest in making your life better. Ect is an option - not tried so far. But it is not a one time treatment. It has to be given under anesthesia, may be 6-12 times. Only few psychiatrists will have ect facility. Ketamine and rtms - very very few psychiatrist will have it.
