Surgical procedure is required or not than can be judged after clinically inspecting the case along with radiograph. After removing the milk tooth you might require braces or orthodontic appliances to bring the permanent tooth in proper orientation.

Hello
duration is suspicious. You should get it checked out immediately by a oral surgeon or oral medicine specialist. You can also visit your nearest dental hospital. Generally gums are not the common site for white patches, however it is possible. A physical check is needed to differentiate if this is common or cancerous.

Professional cleaning of teeth n gums r really necessary every 6 months at a dental clinic. Other than that one needs to brush twice daily and you can also use mouthwash with proper dilution to maintain oral hygiene. Dental flosses r recommended for interdental cleaning of plaque and food debris that gets trapped between teeth n create infection.

It is not always required to extract the tooth in case of pain, infact most of the time it can be saved. Visit your dentist to establish the reason behind the pain and the right solution to it.
