Apply Hot Fomentation twice daily.
Avoid bending in front.
Postural Correction- Sit Tall, Walk Tall.
Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. START EXERCISES WHEN YOUR PAIN REDUCE.
Wear lumber spine belt.
Apply volini cream before sleeping.
Don't lift heavy things.
Don't do forward bending.

Get your x ray done.

you have nerve compression on your l4l5 area thats the main reason for radiating pain. and that is the sciatica. don't lift heavy weight. you need rest. after getting free from the pain you can go for exs. surgery not required just go for physiotherapy sessions it will definitely help you out.
