Infertility is a widespread problem. for about one in five infertile couples the problem lies solely in the male partner and in another quarter, both partners have problems. In most cases, there are no obious symptoms or signs of infertility. Intercourse, erection and ejaculation will usually happen without difficulty. The quantity and appearance of the ejaculated semen generally appears normal to the naked eye. Medical tests are needed to find out if a man is infertile. Male infertility is usually caused by problems that affact either sperm production or sperm transport. Through medical testing, the doctor may be able to find the cause of the problem.

Hypothyroidism - Can It Hamper Your IVF Chances?

The thyroid gland is a small butterfly-shaped gland located at the base of the neck. This is responsible for producing the thyroid hormone that in turn helps regulate metabolism and other bodily functions. The production of less than normal amounts of thyroid hormone is termed as hypothyroidism. Stress, food intolerance, an unhealthy lifestyle and immune system disorders are some of the factors that could contribute to hypothyroidism.

Symptoms of hypothyroidism include

  1. Fatigue
  2. Weight gain
  3. Eczema
  4. Cramps and menstrual irregularities

In mild cases, there may be no visible effects on the person but in severe cases, it can reduce ovulation or cause the ovulation to stop completely. This can result in infertility. Hypothyroidism can also put a woman at a high risk of having a miscarriage.

Infertility can be treated in a number of ways. One of the most common amongst them is IVF or In Vitro Fertilization. This procedure involves stimulating the woman’s ovaries to produce eggs, harvesting them and combining them with sperm cells from the male partner in a laboratory. This increases the chances of forming an embryo. 2 or 3 of the healthiest embryos formed are then reinserted into the woman’s womb. When the embryo is successfully implanted, the woman may then carry the pregnancy to full term.

Many women who undergo IVF treatment give birth to twins or triplets. IVF typically has a very high success rate but when it comes to women suffering from hypothyroidism, this may not be the best course of treatment. This is because hypothyroidism can also prevent the embryos from being successfully implanted in the uterus.

For this reason, a thyroid test is essential before inserting the embryos into the uterus. This can be done with a simple blood test to measure the level of thyroid hormone in the blood. This test is known as a TSH test. Normal TSH levels range from 0.3 to 3.0 mIU/L. If the levels are unacceptable, medication will be prescribed to help regulate them. Dietary changes may also be required. Once the levels are within the normal range, the embryos can be inserted.

Women suffering from hypothyroidism must have regular checkups for the entire duration of their pregnancy. This will help doctors identify potential problems early and deal with them before they can affect the growing fetus. Even after giving birth, the woman should get her thyroid levels checked regularly while she is nursing the baby.

Age 25, male (History: h/o stroke 10 yr old, recovered 70% but on bp medication for last 10 years, diagnosed as type 2 DM @ 19 age since that taking insulin and oral diabetes medicines) From last three months he has uncontrollable blood sugar despite maintaining all diabetes workout. ( diet, exercise, sleeping, medication etc) 1. Insugen 50/50  40 unit morning + 40 unit noons 2. Metformin 1000+ teneligliptin 20 (noon) 3.NPH 26 unit before sleep (10 pm) (added just 2 weeks before) Hba1c is 9.6 , after starting bed time NPH, FBS is now getting 140 to 160 ranges but high ppbs (280-320) range despite eating less and highly fluctuations in RBS (RBS always high)    Life style: same food timetable (low calorie) /fixed moderate exercise schedule/no alcohol/no smoking/etc. What should do now? His oral medications seem not working is there any chance that he has type 1 diabetes instead of 2. If that's so how confirm that.(He is obese, so the first doctor immediately diagnosed as type 2) I searched net and found that c-peptide test will do that, so to get correct insulin production should the external insulin need to stop one day before that test. Or is there any test to check the cause of hyperglycemia. Please ask if need any further information.

Hello, Thanks for the query. I have gone through the details given. It is clear that at present the diabetes is totally uncontrolled. Because he is obese ((BMI 32.57 kgs/Sq meter), doctor must have concluded he has T2 DM. But it is better if you get C-peptide test plus GAD antibodies, to confirm whether there T1 DM. I have a few questions: 1) What is his routine diet? In detail like from early mornong till he goes to sleep. 2) What is the amount of daily exercise being done both interms of time spent and what activity is done? 3) Have the glucose levels remained high right from beginning or has this been noticed recently? 4) Has he been obese all along? 5) What are creatinine, urea and microalbun values? Please come back with these clarifications, then I may be able to guide. Thanks.

Hi, I have started with diabetics. It was noticed 3 weeks back when I came to know that im 5 weeks pregnant. But after 2-3 days I got bleeding and had to go for D&C. Doctor told due to sudden increase in sugar I got this abortion. My dad family member are having sugar problem. But my parents do not have. Even three months back when I checked I did not have any sugar problem. Now I want to know what are the fruits and foods I can take since I have high sugar which sometimes reached above 280 also. I had prescribed to take glycomet sr 500 1-1/2 tablets before food only.

1.no alcohol 2. Reduce body wt 3. No smoking/ tobacco 4. Diet - no ghee/ butter, have mix of vegetable oils - mustard, til, ground nut, olive oil, have more green vegetables and fruits, have whole grain atta, no fried. Fast. Spicy / processed/ junk food. Less sugar, potato, rice 5. 30 mts brisk walk daily 6. Deep breathing exercise for 10 mts daily 7. Meditation daily for 10 mts. 6-8 hrs of sleep at night 8.
