Management of Endometriosis

When cells of tissues that are usually found lining the inside cavity of the uterus, start growing outside the organ, for eg over the ovaries/ fallopian tubes/ urinary bladder etc. it is called endometriosis. Painful periods, pain during sex, pain while urinating or bowel movements, heavy bleeding, fatigue, infertility, diarrhea, bloating and nausea are some of the symptoms of endometriosis.

Treating endometriosis usually depends on the following factors

  1. Age
  2. Severity of symptoms
  3. Severity of the disease
  4. Future plans for children

Depending on these the doctor might opt for any of the following treatments

  1. Medication for Pain: If the symptoms are mild, then the doctor will prescribe some pain killers like Nonsteroidal anti-inflammatory drugs. 
  2. Hormone Therapy: Hormone therapy is effective, but once you stop them, your symptoms may come back. Some of the hormone therapies include: 
    • Hormonal Contraceptives: Birth control pills, vaginal rings and patches are useful in controlling the hormones that are responsible for tissue build-up outside the uterine cavity. 
    • Medroxyprogesterone: This drug stops menstruation, which in turn stops the tissue build-up and put an end to all the symptoms. But on the other hand, gaining weight, low bone production and depression are some of the common effects of this drug. 
    • Gonadotropin-releasing hormone (Gn-RH) antagonists and agonists: Hormones that stimulate ovaries are blocked by these. They bring down oestrogen levels and hence prevent menstruation. 
    • Danazol: This drug prevents menstruation and symptoms of endometriosis by blocking the hormones that stimulate the ovaries. However, Danazol may not be a favourable option because of its side effects. The drug should be avoided during pregnancy.
  3. Conservative Surgery: If you are trying to get pregnant, but you already suffer from endometriosis, then removing endometriosis surgically is a viable solution. Your uterus and ovaries will be preserved, which might increase your chances of becoming pregnant. Even suffering from severe pain might find relief from conservative surgery. 
  4. Hysterectomy: In extreme cases, the only way to deal with the symptoms is to completely remove the uterus, ovaries and cervix. Since a hysterectomy means you cannot have children, it is kept as the last option for women in their reproductive age. Hyperpigmentation is a relatively harmless condition, but can affect a person’s self-image and psyche. This skin condition is characterized by dark patches on the skin due to exposure to UV radiation or as a sign of aging. It is caused by an excessive production of melanin in the body. Hyperpigmentation affects people of all ages and with all skin types. However, fair-skinned people are at a higher risk of suffering from this condition than dark skinned people.

This condition can be treated in a number of ways including:

  1. Laser: Laser treatment for hyperpigmentation improves skin tone and boosts collagen production. With advancement in laser technology, scarring is no longer a very big risk with this form of treatment. Depending on the degree of hyperpigmentation argon or carbon dioxide lasers may be used in with short pulses or long pulses. Treatment can take anywhere between 30 to 45 minutes and is relatively pain free. 
  2. Chemical peels: Chemical peels rid the skin of its top most layer and bring new cells to the surface. This gives the skin an even tone and eliminates dark spots and patchiness. A light salicylic acid or glycolic acid peel is ideal for hyperpigmentation treatment. This is often used in cases where the patient’s skin is resistant to over the counter topical treatment. The concentration of active ingredients in the peel depends on the skin tone. Mild skin discolouration and blotchiness can be treated with a superficial peel while age spots and freckles require a medium peel. 
  3. Topical ointments: Topical ointments are usually the first form of treatment for hyperpigmentation. Hydroquinone or retinol are the most commonly prescribed ointments. Broad spectrum sunscreen that offers protection against UVA and UVB rays can also help prevent further sun damage and dark spots. Skin lightening creams with soy or niacinamide as active ingredients also help brighten skin tones. 

Apart from these forms of treatment, there are also a number of home remedies for this condition. Some of these home remedies are:

  1. Rubbing a freshly sliced or the juice of a grated potato on the skin
  2. Applying a combination of cucumber and lemon juice on the skin
  3. A grated papaya face mask
  4. Soak almonds in milk overnight and then skin them and puree with the milk. Apply this mixture on the affected areas. 
  5. A mask of guava and banana pulp

Chronic nightmares are caused by a lot of internal as well as external factors. It can cause sleep disorders and can affect your overall health. Some of the key reasons that contribute to chronic nightmares are mentioned below:

  1. Stress and anxiety: Anxiety can come in many forms. It can happen due to simplest of reasons such as moving to a new place to joining a new job, suffering from trauma to losing a close one in the family, etc. Stress as a result of these can lead to bad dreams and chronic nightmares.

Dislocation refers to an injury, which causes the bones to abnormally separate from each other in the joint, which is the meeting place of the bones. As a matter of fact, calcium tablets and supplements can pose a significant risk, as well. While it is true that calcium does help in the effort of getting heart disease, an amount which is greater than the dietary limit does not. Actually, the excess calcium may increase the risk. This is true when it comes to calcium from supplements and not calcium which is absorbed by the way of food. So, what sort of additional risk is there? A study found that people who receive calcium from a tablet or another similar source have about a 140% greater chance of a heart attack!

How much is too much?

A thousand milligrammes of calcium in excess of the requirement on a daily basis is said to increase the risk of death from CVD or cardiovascular disease by about a fifth. Not a trivial amount, by any measure.

But why does this happen?

The key to answering this question comes from how the calcium is absorbed by the body. When calcium from food is eaten, the rate of it being released is a lot slower as compared to calcium from tablets. Also, when there is excess calcium, it is not absorbed and kept in reserve by the body but is excreted in the form of urine. So, this calcium going through the kidneys can cause some trouble there, as well!

Yes or No?

So, what is to be done in order to make sure one is having a sufficient amount of calcium? Well, there are a lot of sources and consuming things such as dairy products and fish, along with dark leafy vegetables are some great ways to get natural calcium. Only if a person has very low calcium levels should calcium tablets be consumed? Anger is good. It protects you. But intense, uncontrolled anger may destroy valuable relationships. Anger is a result of anxiety and fear. When we assume that our expectations will not be met, we become anxious. When we feel unsure about our own capabilities we become anxious. This anxiety creates fear. Anxiety comes from our beliefs. All beliefs can be modified.

Anger has lots of energy. Avoid arguments/confrontations when you are very angry. When you are very angry you are likely to use rough language which may damage your relations and you are likely to behave impulsively. All this you might regret later on. Following are a few techniques which will help you to manage your anger in difficult situations :

  1. Sit down and have a glass of water, slowly. This will help you to calm down a little.
  2. Avoid or go away from that room or situation. Tell the people concerned, that you are angry and would like to discuss it some other time.
  3. Deep breathing exercises help a lot. Close your eyes, take 10 deep breaths, turn your attention towards your body. See what is happening to your body. Tell your body muscles to relax. If your heart is beating fast, if you are breathing fast, continue deep breathing, till it normalises.
  4. Now understand what is making you angry. Understand your own anxiety and fear related to the issue.
  5. Let the other person know how you are “feeling”. Start your statements, “I feel hurt/insulted/let down/neglected etc. when you do this”, instead of saying “You hurt/neglect me”. When you express your feeling in this manner, the other person does not feel blamed, which makes the other person more receptive.
  6. Focus on the current issue, don’t bring in past situations.
  7. Tell the other person, what you want or expect, instead of telling what you do not want. E.G. Instead of saying “I don’t want you to go out now”, say “I would like you to stay at home today”.
  8. Listen carefully what the other person wants to tell you. Give others a chance to voice their opinions.
  9. Make an eye contact and be firm but polite while communicating your point of view.
  10. Understand everyone’s responsibility in the situation. Try to see, what is your share in the problematic situation? Work on that.
  11. Understand what change you want.
  12. Have realistic expectations from others and also from yourself.

Smoking can cause irreparable damage to your mind and body. Chemicals in the tobacco travel to all parts of the body, leaving no area unaffected. According to available data, smoking causes 30% of all cancer deaths and about 80% deaths from emphysema and bronchitis. In addition to adversely affecting yourself, smoking also has a severe impact on your near and dear ones. According to WHO, about 600,000 people die every year from passive smoking. Out of the deaths that occur due to this, 1/3rd is that of children.

The Indian situation
While 85% of tobacco users worldwide consume it in the form of cigarettes, in India only 13% use it in that form. Additionally, 54% use it in the form of beedis. A study conducted on Indian smokers has revealed that an average of 8.2 cigarettes is smoked by an individual daily.

The study also revealed that the number of cigarettes smoked every year had grown to over 6 trillion. While 1 out of 10 adults dies from tobacco use worldwide, 5% of deaths in women and 20% of deaths in men in India are caused by cigarette and beedi smoking.

What happens when you smoke?
Cigarette smoke is made up of 4000 chemicals that are present either as tiny particles or gases and about 50 are known to lead to cancer, the toxin nicotine being one of them. In addition to nicotine, the chemicals that make up cigarette smoke also include tar and carbon monoxide. Prolonged exposure to these toxins can hamper your body's ability to filter air and clean the lungs. The smoke not only irritates the lungs but also causes excess production of mucus.

It also causes a paralysis of the tiny hair-like structures like cilia that line the airways and are responsible for removing dust and dirt from the organ. Paralysis of these hair-like structures also causes a buildup of mucus and toxic substances, leading to lung congestion. The extra mucus that is produced causes smokers to suffer from the very ubiquitous smoker's cough and chronic bronchitis.

It's also one of the many triggers of asthma, which brings about the narrowing and inflammation of the airways. Long-term exposure to tobacco smoke causes the destruction of the structure of the lung, walls of the airways as well as lung tissue. The result is a condition known as emphysema. Additionally, smoking also leads to cancer of the lung and over 80% lung cancer cases occur due to this habit. If you wish to discuss about any specific problem, you can consult a pulmonologist.

You are in a very serious situation. Losing 20 kgs in 3 months can adversely affect your body. Specially when you are already extremely obese which seem to be causing you lots of complications. Please go to a dietician so that you can get a scientific and customized diet regime.

Hi Ramya U avoid high carbohydrate diet exm. chapati, rice, bread, potato. U can use high protein diet exm. chapati, rice, bread, potato. U take 1 microfruit and 1 cytofresh tablet after breakfast.

hello sir , take high fiber diet.take luke warm water before and after meals.have small and frequent meals.take plenty of fruits. take black tea / black coffee without sugar .do stretching exercise alternate days.don't skip your breakfast and meals.
