Hello hope you are keeping safe 1) from your brief history, there are many things to rule out from - urticaria / hives - photosensitivity - dermatitis (probably due to dust mites/ pollen allergy) - atopy (genetic tendency for allergy) although, it mostly looks like an acute onset of urticaria. 2) here are the few reasons you can develope an itching due to urticaria - dental caries - urinary tract infection - frequent stomach pains - ear infections - any drug intake induced allergy - food allergy -. Allergy to any lotions or other products - any contact allergy 3) you can use - any antihistamines for itching like montelukast, levocetrizine, atarax, cetrizine or a combination therapy - emollients containing calamine + aloe vera - non-soap cleanser for bath 4) please note that these medications are only for symptomatic treatment and the problem will persist if there a root untreated cause. 5) you can do the following investigations. - complete blood routine - esr - ige levels - dental follow up to rule out caries - urine routine 6) you can avoid these foods for a few days - meat and poultry products - dairy products - fish - pineapple - citrus fruits - nuts and chocolate - eggs - soy and wheat items - any self - suspected food 7) the swelling near the anus could mostly be of haemarrhoids / piles.- you can eat foods that relieve constipation, that contain a lot of fibre like legumes, whole grains and cereals, berries, banana, apples, pears,green leafy vegetables - avoid dairy products, breads, meat and processed food - you can apply some anaesthetic ointments like lidocaine gel or creams containing hydrocortisone to relieve the pain and inflammation. - follow sitz bath routine for 20 minutes several times a day - drink more than 10 glasses of water per day, approximately 2-3l. - have some painkillers like paracetamol to relieve the pain and inflammation you should also rule out a possibility of an anal abcess / infections, if present, you should take a course of antibiotics. Hello hope you are doing well 1) I have read your brief history and understand your concern about ezcema. You have also mentioned you have had history of bullous disorder. Please be aware of a few causes that could aggravate your eczema / pemphigoid episodes. - certain drugs like diuretics, antibiotics like pencillin, antidiabetic medications like sitagliptin. - foods like citrus, dairy products, nuts, eggs, gluten, tomatoes and spicy foods need to be avoided unless flare up reduces - dust allergy - family history of atopy - contact allergy to some creams or topicals - dehydration causing dry sensitive skin - hot baths - scrubbing of skin - soaps and other detergents. Please take care to avoid the possible triggering factors. 2) halovate f cream is a potent steroid with an antibacterial coverage, that is beneficial, but I will be able to suggest an appropriate topical only if I see a photo and can grade the severity of the ezcema and decide if you need a short course of oral antibiotics too. Apart from this you can apply - soothing emollients/ emulsifiers like aloevera, liquid paraffin twice daily - antihistamines like cetrizine or atarax to relieve the itching - a medicated non-soap cleanser for bath like cetaphil - multivitamin tablets.(main vit d, zinc, calcium) - potassium permagnate soaks to dry out wet lesions. (antiseptic property) kindly check your vit d and calcium levels as they are directly linked and their deficiency can cause an aggravation of the condition. (both eczema and bullous disorder) kindly titrate your oral steroid dose, according to the severity of the lesion as long time untitrated doses can lead to steroid induced side effects. 3) also follow these measures - wear loose, breathable clothes - do not use strong soaps/ detergents - avoid wet clothes - stay in a cool, well ventilated room - avoid itching to prevent a secondary bacterial skin infection - do not use body scrubbers - do not share personal belongings - sun dry clothes and iron clothes before using to prevent a potential fungal infection for more specific treatment, revert back with a photograph of the lesions.
