Mr. lybrate-user, Thanks for the query, In long standing diabetes peripheral neuropathy is a commonly occurring complication. This is particularly seen if the blood glucose control is not good enough. Plus it is observed even when there is a fairly well controlled diabetes. Since there is no mention of your blood glucose levels and HbA1c%, it is not possible to know how well is the condition controlled. Tramadol does help in giving some pain relief. But there are some other drugs like Gabapentine and Pragabalin which could be useful. But most important is strict blood glucose control with fasting < 100 mg, PP 160 to 170 mg & HbA1c% < 6.8. In addition your BMI is 29 kgs/Sq Meter, which is very close to grade 1 obesity. This itself is a major contributor for improper control of diabetes and high BP. Plus it is possible that due uncontrolled hypothyroidism there is excess weight gain. Your ideal weight should be around 59 kgs, as against that you are now 79 kgs (20 kgs excess). When one is on adequate dosage of levothyroxine TSH is usually maintained around 3 to 4 mU/L, at that all the symptoms are well in check. Since there is no mention of details of glucose levels like fasting, PP & HbA1c%, it is difficult to comment. A level in the range of 325 mg is too high, suggestive of totally uncontrolled diabetes. Which means there is an urgent need at updating the drug treatment, plus giving a serious look at diet and exercise and weight control. Even BP will be well controlled when there is weight reduction, plus initially there is again a need for adding some suitable drug. In this communication we are supposed to give only general guidelines, no drugs can be prescribed.
