Yes, smoking can hamper sleep. Since you seem to have identified issues from childhood, have you thought of seeking the help of a counselor? If you have skewed your biorhythm by this extent, then you will need to give yourself some time before it returns to the normal cycle. Even if you feel less anxious visibly, there could be underlying factors to consider that may disturb sleep. You need to check on your prenatal history and early childhood for influencing factors. You must combine medication with therapy. This is extremely important for your anxiety and depression. Life is full of problems as a norm. Any attempts to escape will never help: you must learn to face these problems every day with good success. The counselor will help you in this regard. I also strongly believe that every crisis offers an opportunity for change, if you seek it and grab the opportunity. Please meet with a counselor and work on your emotions, particularly anger and fear. Exercise is very important but you have to build on it developmentally. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend yoga classes etc. Watch sitcoms on tv or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least 30 minutes but not in the scorching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in god to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love. Learn all about emotions and how to handle them and that will get you out of the depression rather easily and quickly. A counselor is there only to facilitate you, all the hard work must come from you, and your cooperation with that person is very critical for your success. Be positive every day and learn to be content with what you have. Whatever happens please cooperate with the therapy and do not discontinue until the condition is completely resolved. The lack of sleep, i.e. Sleep for less than 6 hours per night, is directly linked to about 86 diseases! so let us first focus on that. In your case particularly, do not sleep during the day; don’t even lie down. Go for regular exercise (breathing through your nostrils only) at a fixed time (at about 6 am) every day, whether you sleep or not. You must develop this habit of sleeping by 10 pm latest every night and wake up at a uniform time by 6 am every morning, even if you do not get sleep. If you do not get sleep, just lie down and if you have a bed lamp (or you could get one) lie down and read something light but not exciting. Your eyes will tire and you will fall asleep. If you like you may also put on light and soft music with the sleep function on. Depending on whether you are a visual or auditory person, you may watch tv in a lying down position until you feel sleepy. Have a warm bath before going to bed (for some people a cold bath at night seems to induce sleep! if you are one of them, please do so). Place the head side of your bed in a north-south position. The room must be as dark as possible and there should be no electronic items that give off even a light led glow. There must be ample ventilation in the room. Avoid strong odors, except lavender, on your body or in the room, or on your sheets and clothes. There should not be any live plants in your bedroom. Sleep with the lightest of clothes or without any clothing. When lying down to sleep, lie down on your left ear as much as possible. Do not sleep directly under an overhead fan. You must eat a good breakfast, a fairly light lunch, and an early dinner, which should be a very light meal. Whatever you eat in the other meals, have only a pure carbohydrate meal at night with any vegetables, and preferably before 8 pm. Do not drink much liquids after or near about dinner time. If you know emotional freedom technique or eft (check this out either on youtube or google), then please practice this too for sleep induction. Self-hypnosis and slow relaxation techniques, at night will be tremendously useful. Acupuncture or acupressure will both be very useful too. You can also try the 4-7-8 breathing pattern, which is to breathe in for four counts, hold your breath for 7 counts and exhale with a whoosh sound for 8 counts. This is to be done four times at a time first thing in the morning and last thing at night, and any number of times during the day. While doing this exercise place the tip of your tongue on the roof of your mouth. The effects will be seen after a few days. Pray last thing at night before you sleep.

I don't think it is unipolar depression and is more serious than that. You need to go for counseling too if you want to feel better and even recover. It is so much better and you would have developed a lot of coping mechanisms by now and may have been able to conduct yourself to the point of even enjoying life to a certain extent. Even now it is not too late, although it might be a little more difficult. Nature and the way we have been made, have fortunately built-in certain structures that let us live close to normal lives, especially up to our teens. After that, if the little signs that there is some problem were not noticed and dealt with, then the issues will come up for the last time seeking a resolution in late adolescence. But for some reason it was not handled well or in time, and therefore this situation where it has now become a permanent negative trait that has to be dealt with more aggressively. Now it is time to seek counseling in tandem with the medication; and you are to keep with the counseling until there is a resolution. Be cooperative and follow all the directives and requirements and you will see a gradual but sure improvement. You must be active and work towards your reformation. Many lifestyle changes need to be made to progress away for the condition. When you are depressed, you will feel lazy though I would like to call it lethargy. There is an inertia and lack of energy to even think, let alone act. You are required to literally do the opposite of what you feel like doing. Depression makes you want to withdraw, lie down, sleep, become lethargic, slow down, make you too lazy to even think etc: exactly like you described. So, when I suggested doing the opposite, you will need to fight this disease. It is necessary to know what is causing tension and sleep problems. To feel better consult a psychiatrist about medication. If you are experiencing side effects of medication talk to your psychiatrist about it. Consult a psychologist and explain about tension and sleep problems. The most effective solution for dealing with anxiety for me personally was waking up early. I am not a morning person but once I tried this, I was surprised how good this made me feel. It helped me find peace and calm myself down. Having so much time in my hand I could do small things that made me happy and I felt a sense of accomplishment just by waking up early. I feel the most anxious during the night- so sleeping early really helped me tackle it to a great extent. Meditation was another thing I tried when my anxiety was at its peak. Trust me- for someone who thinks a lot, it is no joke to shut your mind down and focus on your breathing. I could concentrate for very few minutes in the beginning but my thoughts kept spiraling back. So listening to some meditation music while I meditate helped me increase concentration. It helped me analyse my recurring thoughts and gave me more clarity about them. Mostly during such times I don’t feel like interacting with anyone- thinking that nobody will really understand how I feel. It’s great if you can talk it out and rant everything going on in your mind to someone who cares to listen. Taking it all out really helps. If you’re someone who is not comfortable with addressing this with anyone, then you can try writing down all that is going on in your mind. Once you take it all out in any manner, you will feel relieved. If you feel like you are stuck between your thoughts and feel very helpless, just do something. Take a walk, call up your friends and talk about something random, play with your pet - just do something to break that cycle that is going on in your head. Do anything that will distract you even for sometime. Take action without overthinking anything. Just go for it. Being positive and thinking right is almost impossible during such times. One of the main reasons I avoid talking to people during this time is because they may feel bad/ give some positive shit which is not what I want. You know yourself the best- try starting small and appreciating small wins. Having a motivational self-talk session/ looking in the mirror and giving yourself some positivity might help. Appreciate yourself for how far you have come and remind yourself about your strengths. Try to get a balance of the basic things that need to be right- sleep, food, health and water level. Once you get these things in check it will show better results over time. Having any kind of physical activity or a workout routine will help you fight anxiety. Doing this releases endorphins which trigger a positive feeling in the body. It will also help you reduce stress, improve sleep and help in fighting with anxiety and depression. If all the above things do not help and you are still stuck with being anxious, then there is just one way out. Confrontation. Ask yourself what is troubling you and analyse your thoughts. Address it and not run away from it. You have to feel the anxiety, be curious about it and seek answers and find out what is really going on. There will always be an underlying message or something you’re trying to run away from and are too weak to confront that. No matter how much you run away from it, sooner or later you will have to face it. This is a long process and you need to be strong and have patience to tackle it. Lastly, know that you are not alone. This feeling is just temporary. Think of it as your body’s way to communicate something with you. We feel this only when we have lost perspective and when we are lacking inspiration. We have to just find ways to get out of the drama going on in our heads and gain perspective. Anxiety is something most of us go through at different levels. We are just too weak to talk about it. It is a part of being human- you are not alone.

There are multiple issues. Let's go one by one: 1. Depression: symptoms of tiredness, increased sleepiness and no response on watching porn which amounts to lack of sexual arousal are all well established signs of depression. So, even now I suspect that you have ongoing depression which needs to be managed by either change in meds (if escitalopram is not working ). 2. Porn: watching porn alters your motivation system (dopamine system) so much that you no more feel motivated to do anything productive. It also alters the way you imagine about real sex and even the way you perceive women as well as damages your own self image. So, avoiding all the visual cues and situations which trigger indulgence in further bingeing is strongly recommended. 3. Other striking feature is regarding weight. With your height being 5.1 feet, 77 kgs falls under category of obesity. That could be contributing to your lethargy and depression has been strongly associated with obesity. So, I would recommend cutting down on your weight as one of the first things to focus on. Strongly recommend consulting a psychiatrist before any alteration of medications. Sir, please get yourself evaluated by an allergy specialist thoroughly and only after clearance from them consult a psychiatrist. Subsequently as you said that most of symptoms came under control by taking sertralin earlier, it may well be psychological. Only difference now is that even sertralin is not working and you may be well with change of medications.

Seems there is an inherent issue in your serotnergic transmission in the mind (brain), hence meds work and on stopping symptoms relapse. You have to make up your mind in regular continuous treatment like diabetes, hypertension etc. Also face the weight issues of medications with strict dietary schedule and life style modification. Good luck.

Hello, there are much better medications available for ocd that shows respose within a month. Also read exposure and response prevention therapy and apply it on yourself with the help of your family member or psychiatrist. You do not need to carry this problem when such promising and quick treatment is available.

Remember one thing there is no easy way to be successful, if you wants to be successful you have to gone through so much odd, can go through failure also, it's not all that easy, keep calm do your work, focus on it, success will come but be prepare for the odd also, keep it mind, read motivational stories it will help.
