According to you provided history your suffering from fistulla in ano or hemorrhoids for this cases are final most step we do is surgery only if that patient is feel guilty and so much excruciating pain and bleeding I dint get your grade (degree) of the fisstula or hemorrhoids but anyway these patients commonly comeout with surgery as if now I suggest you are causing pain i'll take pain medication.

The small tiny lump outside anus can be skin tags, that occur secondary to anal fissure. For better treatment of anal fissure, clinical (physical) examination is very much important to check whether surgical management is necessary or not. Till then, do sitz bath 3-4 times/day and apply anobliss ointment & use any stool softener. It would be best if you visit a general surgeon nearby. (can consult me if in howrah district).

Increasing the fibre in your diet is known to be as effective as injections for preventing further problems 3.
