What does mean mesenchymal soft tissue lesion in ultrasound of arm report I have a painless lump in my shoulder since 5 months with no symptoms.

It means that the ultrasound can see that there is solid tissue, with texture of mesenchyme (soft tissues other than bone such as muscle, mixed fat) but not beyond that. If the tissue seems harmful and/or it has been suggested, biopsy/tissue sampling and seeing under microscope, will help understand the exact nature of elements causing the lump.
