I have few black patches on my nose and cheeks from the last year, many creams tried but not effective, which medicine and cream I should try for patches, I am 38 years old.

Your most probable diagnosis is melasma.in some cases it is resistant. 70-80% improvement is quite satisfactory result. Always 50 plus spf sunscreen every 3-4 hour. Use kojic acid based cream at night on spot. Tranexemic acid tablet sometimes helps but doctor must confirm that it is not contraindicated to you. Q swithed .laser and glycolic chemical peel sometimes help.
