How can we find breast cancer in first stage? And how can we react to the sickness.

DNB, MBBS
Oncologist, Faridabad
Dear sir, regular self breast examination five days after the periods are over, is the key to diagnose in early stages. If over 40 years then she can get mammography done in addition to self and clinical breast examination. Most often it presents as painless lump, so do not ignore any lumps in breast.

I have pain under my right breast. It became worst while taking deep breath or changing positions while sleeping. And also pain starts at the middle. Does this pain relates to ribs or sternum. What it is. Is it serious. Please advice..

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Hello, Thanks for the query. Based on the explanation given it is difficult give a clear opinion as to why the breast pain is occurring. It needs to be properly examined and if needed some investigations may need to be done. Therefore, my suggestion is better to consult a local competent surgeon and based on his advise further investigations should be done. Do not neglect. Usually cancerous problems are painless. But please get properly examined. Thanks.

Could you please suggest weather radiology is best or chemotherapy for treatment of cancer.

MS ( General Surgery)
Oncologist, Mandsaur
Radiotherapy & chemotherapy are two ways to treat cancer. Few cancers are more sensitive to radiotherapy while others are radio resistant. Same way few are chemotherapy resistant while other cancers are chemo sensitive. So it depends on nature of cancer and accordingly it is decided to treat cancer by chemotherapy, radiotherapy or by both modalities. Nothing like which modality is better, in few cancer radiotherapy is good while in others chemotherapy is better.

Hi lybrate-user, I appreciate you seeking help. Well, GERD can possibly be one of the reasons that can trigger allergic asthma, and cause shortness of breath. Medicines apart, as advised by your doc, you need make sure your diet/ nutrition, lifestyle including bowel habits etc. All are streamlined. Learn to relax and keep your stress levels low. Its advisable to eat in moderation and less than your full capacity than eating to your full. Try avoiding tea, coffee, spices, chilies, pickles, extremely hot & cold/ chilled foods. Do consume citrus fruits/ vegetables rich in vitamin C as they help strengthen your immunity. Stay well hydrated and consume at least 6 to 8 glasses of clean & filtered water daily. Warm saline gargling, at least once daily, shall help protect you from throat related afflictions that often trigger asthma in certain people. Mostly, fresh air, dry climate, regular exercise help too. Daily consumption of ginger tea helps a lot too, unless you are allergic to it. Post meals, try sitting in 'Vajrasana' yogasana for about 10 minutes. Learning well & practicing yogasanas like bhujangasana, shavasana, and pranayams like kapalbhati, anulom-viloma, bhramari etc on a daily basis are highly beneficial in this condition. Hope this helps. Do take care, and all the very best. Sincerely

What are the complications which can be possible after stomach cancer surgery? Please tell me in detail.

Post Doctoral Research (Ph.D.) (A.M) (Oncology), Integrative Oncology for Physicians (MSKCC, N.Y, USA), Doctor of Natural Medicine (N.D/ N.M.D), Ayurveda (I) Cert., Advanced Strategic Management (APSM), B.E (Computer Sc. & Engg.), Clinically Relevant Herb-Drug Interactions (CME) - (Cine-Med Inc. USA)
Alternative Medicine Specialist, Bhubaneswar
Hi lybrate-user, I appreciate you seeking help. Well, as far as I know and understand, following are the probable complications and side effects of a stomach cancer surgery - - being a difficult surgery often, there can be bleeding from the surgery, blood clots etc all. There can also be damage to nearby organs during the surgical procedure. - the patient will usually not be allowed to eat/drink anything for at least a few days post total or partial gastrectomy. This allows time for the digestive tract to heal properly. - while recovering from surgery, one may have side effects like nausea, heartburn, pain in abdomen, diarrhea etc all especially after taking food. Dumping of food into the intestines happens too quickly after eating. The side effects often get better over time though. - one will require to eat smaller meals, more frequently. This is dependent upon how much of the stomach is actually removed. - as a surgical removal (total/ partial) can interfere with vitamins absorption, one may require to be on vitamin supplements as appropriate. Last but not the least, having said the above-mentioned, I would advise you nevertheless to get in touch with an experienced/ expert onco surgeon specializing in gastric cancers, who can educate you more on the risks/ complications versus the benefits involved. You can take a decision accordingly. Also, I would like to emphatically add hereby that an integrative treatment (peri-operative) with natural/ alternative medicines can certainly help fasten recovery and get over convalescence soon. Hope this helps

14 Tips to Help You Recuperate from Prostate Operation

Fellowship of the Royal College of Surgeons (FRCS), MS, MBBS
Urologist, Delhi
14 Tips to Help You Recuperate from Prostate Operation

A prostate operation involves the removal of a part of the prostate gland or sometimes the removal of the entire prostate gland. Such a surgery is a serious one and several recovery tips must be followed by a patient to ensure quick and successful healing.

Here are some important tips for recuperating from a prostate operation:

  1. After the surgery, you need to spend up to three days in the hospital for initial recovery. You might be given fluids, which go directly into your veins until you fully recover from anesthesia.
  2. After the operation, normal urination will be disrupted in the beginning, as the urethra stays swollen. Urinating gets a little difficult and uncomfortable for a few days after the removal of the catheter.
  3. You may feel fatigued and may feel under the weather for some weeks after a prostate operation. So you must take things easy for at least two months.
  4. Avoid lifting or moving heavy objects for some weeks following the surgery. Do not perform any exercise, which causes strain. You should ask your friends and family members to carry out chores for you.
  5. After becoming slightly stable you should start gentle exercises. Walking is an effective remedy, which helps in blood circulation and lowers the risk of getting blood clots in the legs.
  6. You should drink lots of water during the recovery stage. This will reduce the risk of getting a urinary tract infection and also help in clearing any trace of blood from your urine.
  7. For improving bladder control, you should perform several pelvic floor exercises.
  8. In case of any kind of pain, you can take over the counter painkillers to ease the pain.
  9. It will take you a time span of three to six weeks for total recovery after a prostate operation. You should consult a surgeon about the best time to return to normal daily activities.
  10. You can get back to work soon, but it depends on your nature of work. Office jobs are not much of a problem, but in case of physical labour, more time is needed before you can resume your duties.
  11. Activities such as driving can be started quite soon after initial recovery. However, it varies from person to person. Some people may get back to driving within two weeks while some may need a month.
  12. You have to wait for 6 to 8 weeks after a prostate operation before getting back to sexual activities.
  13. In case you experience high fever, pain during urination or the inability to urinate, you must consult your doctor immediately.
  14. It takes a person several weeks to recover from a prostate operation completely. Initially, total rest is required. But eventually, he can return to normal activities and lead a healthy life.

Breast Cancer - 4 Common Signs!

DNB, DGO, MBBS
Gynaecologist, Faridabad
Breast Cancer - 4 Common Signs!

Breast cancer is a type of a cancerous infection that develops in the skin cells of the breast. This condition is more commonly diagnosed after you have skin cancer and is more common in women than men. There are certain signs to identify this disease, which are as follows.

1. Breast lumps - Lumps are patches of skin that occur due to the thickening and swelling of the skin. Breast lumps is a very common phenomenon for women but that does not always lead to a breast cancer; however, frequent and reoccurring instances of the same increases the risk of breast cancer. A breast lump is easily identifiable through the naked eye as it looks different from the surrounding tissues.

2. Blood discharges - The nipple is one of the most sensitive areas in the human body. In case of women, it is meant to emit milk during pregnancy, in order to feed the infant externally. A probable symptom of breast cancer can also be a frequent discharge of blood from the nipple without any visible damage or injury.

3. Breast changes - If you are experiencing any visible changes in the size, appearance or shape of the breast, then it is an indicative sign that you may have breast cancer. Thus, it is of paramount importance that you observe, touch and physically examine your breast regularly for changes. If there is any major change detected then it is advisable to consult a doctor.

4. Inverted nipple - The tip of the nipple is generally bulging and pointing outwards both in cases of men and women. But sometimes the tip of the nipple is pushed inside due to excessive suction caused by the skin tissues present inside the breast around the nipple area. This suction forces the nipple to grow inwards which is abnormal and might harm the breast and skin in various ways.

However, apart from these main symptoms, some other signs are also there that increase your chances of being diagnosed with breast cancer. They are redness or pitting of the skin over your breast like an orange, peeling or scaling of the pigmented area around the nipple, dimpling of the skin on the breasts, and such others.

Sir I have a half marble size lump near my neck which is not painful is it thyroid or any other disorder?

MBBS
General Physician, Ahmedabad
Hi, the location of lump is important for diagnosis. In the neck area except thyroid, lymphnodes are also important. For perfect diagnosis, sonography is useful and if possible biopsy is last one.

What You Should Know About Breast Lump Detection

MD - Radio Diagnosis/Radiology, MBBS
Radiologist, Mumbai
What You Should Know About Breast Lump Detection

A breast lump is a restricted swelling, projection, lump or knot in the breast that doesn't feel like a breast tissue. There are diverse reasons why breast lumps occur.

Not all lumps are cancer. These can also be breast conditions that are not harmful and which can be easily curable.Knots that feel harder or are not the same as the rest of the breast need to be checked. This kind of irregularity might be an indication of breast cancer.

A self-examination should be your starting point. This is how you can detect a lump on your own:

Step 1: Begin by taking a look at your breast in the mirror. Keep your shoulders straight and your arms on your hips.

This is what you need to search for:

  1. Breasts that are their typical size, shape, and shading
  2. Breasts that are uniformly formed without distortion or swelling

In the event that you see any of the changes mentioned below, convey them to your doctor:

  1. Dimpling, puckering, or protruding of the skin
  2. A nipple that is not in its initial position
  3. Redness, rash or swelling

Step 2: Now, raise your arms and look for the changes mentioned above.

Step 3: While you're in front of the mirror, search for any indications of liquid or blood discharge from the nipples.

Step 4: Next, examine your breasts while resting.Use Your right hand to feel your left breast and use your left hand to feel your right breast. Cover the whole breast from your collarbone to the highest point of your mid-region, and from your armpit to your cleavage to search for any lumps.

Step 5:Examine your breasts while either standing or sitting. Many women find that the easiest approach to examine their breasts is when their skin is wet, so this step could be done while taking a bath as well.

Diagnosis:

  • Mammogram: Mammography is a technique using X-rays to diagnose and locate tumours of the breasts.
  • Breast ultrasound: Breast ultrasound utilises sound waves to create pictures of the breasts from the inside.
  • Breast MRI:This involves using an effective and attractive field, and radio frequency pulses to create photos of the insides of the breasts.
  • Ultrasound-guided biopsy: During this sort of biopsy, utilising ultrasound imaging to discover the bump, a radiologist will give you anesthesia and afterward insert a needle into the lump to evacuate some tissue for assessment under a magnifying lens. Stereotactic biopsy and an X-ray-guided biopsy may also be used.

Possible treatments:

  1. In case the knot turns out to be cancerous, surgery is typically performed.
  2. You may have a few discussions with different doctors for additional treatment, including radiation treatment and chemotherapy or hormone treatment.

I am suffering from fibroadenoma since a long time but I have been taking homeopathy medicine since last six months. The time I take medicine my tumour is in small size but if I stop taking it, my tumour grows. What should I do to remove the tumour completely except having a surgery?

M.CH - Surgical Oncology
Oncologist, Visakhapatnam
Dear there is no other option than getting it removed surgically for complete relief. There is always a chance given your age and description that the lump may subside by itself over a period of time (usually years). But it also possible that it may not regress on its own. If it is causing you distress, surgical removal is ideal.

You can follow test psa. And take dutas- 4 1 od after meal. Vitamin c from vegetables was associated with lower risk. Vegetables highest in vitamin c include bell peppers, broccoli, brussels sprouts, kohlrabi, snow or snap peas, cauliflower, kale and tomato or vegetable juices. Foods higher in zinc also seemed to be beneficial. These include oysters, crab, baked beans, duck, lamb and beef (lean). Interestingly, a high fruit intake didn't affect the risk of bph. And the role of total protein was unclear. Some studies demonstrated an increased risk in men who ate red meat every day compared with men who only ate it once a week, but other studies found a decreased risk in men with a high total protein intake. Finally, the article found no evidence that dietary or herbal supplements were useful in preventing or treating bph. Take note, gentlemen: a healthy prostate is yet another reason to exercise, avoid obesity, eat your vegetables, keep fat intake low and enjoy an occasional drink.
