Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance. If you keep in touch personally. I ate prawns last night. After one hour I observed some rash with bumps of skin with slight itching sensation on my shoulders, chest and back. I thought may be due to any mosquito bites first. But even after observing the same symptoms in morning, I thought may be due to some allergy. My stomach is also feeling uncomfortable since the food was not digested so far (as I feeling. Its due to allergy by proteins of prawn. Avoid it next time .apply coconut oil with camphor all over body, take antihistamine tablet if rash not releived or visit physically to doctor.

Preferably, therapy for PE (Premature ejaculation). Should be conducted under the supervision of a health professional trained in sexual dysfunction. Both partners must participate responsibly in the therapeutic program. Treatment of PE requires patience, dedication and commitment by both partners, and the therapist must convey this message to both. The first part of therapy requires both partners to avoid intercourse for a period of several weeks. This period of abstinence is helpful in relieving any troublesome performance anxiety on the part of the man that may interfere with therapy. Behavioral techniques, taught either individually, conjointly, or in groups, are effective in the therapy of PE. A preliminary stage of all treatment is termed "sensate focus" and involves the man's concentration on the process of sexual arousal and orgasm. He should learn each step in the process, most particularly the moment prior to the "point of no return. The sexual partner participates in the process, maintaining an awareness of the patient's sensations and how close he is to ejaculating. At this point, two techniques are commonly used: • The "stop and start" technique. This approach involves sexual stimulation until the man recognizes that he is about to ejaculate. At this time, the stimulation is discontinued for about thirty seconds and then resumed. This sequence of events is repeated until ejaculation is desired by both partners, with stimulation continuing until ejaculation occurs.• The "squeeze" technique. This approach involves sexual stimulation, usually by the sexual partner, until the man recognizes that he is about to ejaculate. At this time stimulation ceases. The patient or his partner gently squeezes the end of the penis at the junction of the glans penis (tip of the penis) with the shaft. The squeezing is continued for several seconds. Sexual stimulation is withheld for about 30 seconds and then resumed. This sequence of events is repeated by the patient alone or with the assistance of his partner until ejaculation is desired. At this point stimulation is continued until the man ejaculates. The patient and his partner should be advised against trying any of the many unproven remedies that are available either over the counter or popularized on the Internet. Certain prescription medications, especially antidepressants that produce delayed ejaculation as a side effect, may be useful as therapeutic adjuncts. Recently, the use of a class of drugs known as selective serotonin receptor inhibitors (SSRIs) has shown promise in the treatment of premature ejaculation. The SSRIs prolong the time it takes the man to ejaculate by as much as 30 minutes. The SSRIs most commonly used to treat PE are sertraline and fluoxetine. Potential complications Premature ejaculation that takes place before the man's penis enters the woman's vagina will interfere with conception, if the couple is planning a pregnancy. Continued lack of ejaculatory control may lead to sexual dissatisfaction for either or both members of the couple. It may become a source of marital tension, disturbed interpersonal relationships, and eventual separation or divorce.

Normal level of vit D is between between 20-50 ng/ml. A level less than 20 ng/ml indicates Vitamin D deficiency. Vitamin D3 is essential for healthy bone, teeth, muscle, cell growth, immunity,neuro muscular functioning and overall health. 1. Expose yourself to sunlight for 15-20 minutes daily in the morning 2. Take Milk, cheese, egg yolk, Fish (salmon/mackerel/ trout/sardines/tuna) cod liver oil, Mushroom, cereals, orange juice. 3. If you have deficiency of Vitamin D you have to take Vitamin D supplement 60000IU/week fo r 3 months after consulting physician.

You could start with some exercises for the shoulder. Exercise - start with shrugging of shoulder up to 10 tines. Exercises - gentle pendular movements exercise - ststretching of shoulder and neck will help. For your neck you need to do isometric exercises. Placing your hand on your forehead pushing your forehead against the hands. Gentlyneck movement exercises. All these will help you. You also need to improve on posture while sitting standing.

First treat constipation do 1.Take 2/ 3 glass of warm water in the morning 2.Take little more water in the day 3.Eat more fruits and green vegetables 4.No fried/ spicy/ junk/ processed/ fast food, no maida products 5.No tea/ coffee/ cold drinks 6.No alcohol 7.Reduce wt 8.Daily morning walk for 30 mts 9.Deep breathing exercise for 10 mts daily 10.Fiber Foods that are high in insoluble fiber include whole grains, fruits, and vegetables. Try wheat bran, brown rice, or whole grain bread. Soluble fiber- Prunes and figs can be added to breakfast or eaten as a snack. 12.Take a glass of hot milk with sugar at night before sleeping For medicine contact on private consultation Good luck.

Earlier I had a fungal infection and I had prescribed medicine by doctor at the time of taking medicine its ok but when I stop irritation again occur in bump and side of private parts and also in armpit. So please prescribed medicine so that I can get rid of irritation.

You should take Ayurvedic treatment as below 1-Purim capsule 1 bd Neemol tab 2 bd 2- Anant salsa syrup 3 tsf bd 3- Six-c ointment for local application Avoid spicy, fast food and sour items. Adopt healthy life style. If you have constipation should take issabgol husk 2 tsf with warm milk or water.

Are you constipated? Or having frequent diarrhoea? both are motion problem. Will you be little specific? diabetics do have gastroparesis - slowed down movements of gi tract, which causes constipation. I am damn sure your diabetologist / doctor has spoken to you about it if he heard it from you. You talk to him. In the mean time take oral fluid (water - 3 litre daily), high fibre diet. Laxatives if required. Continue exercise and dietary management as suggested by your treating doctor and also medicines. Monitor blood sugar periodically and also check regularly to find that you are not developing any complication of diabetes. All answers you shall get from your treating doctor.
