General Physician,
Erectile dysfunction (ED) is a condition in which man has trouble in keeping or getting an erection. It is a common male sexual dysfunction. It becomes more common as you get older but it is not a part of aging. MECHANISM OF ERECTION An erection starts in the brain. Physical and mental stimulation is required for an erection to begin. This stimulation causes nerves in the brain to send chemical messages to nerves which are present in the penis which results in the free flow of blood into the penis. This leads to high pressure in the penis which in turn traps the blood within both corpora cavernosa and then the penis expand. This causes the penis to sustain an erection. When the inflow of blood is stopped, then it opens outflow channels and then penis becomes soft. Then an erection is said to be reversed. CAUSES OF ERECTILE DYSFUNCTION Erection is a complex process which involves hormones, brain, emotions, muscles, nerves and blood vessels. Problem with any one of these can lead to erectile dysfunction. Sometimes stress and mental health can also lead to erectile dysfunction. Combination of Physical and Psychological issues can lead to erectile dysfunction. Some medicines like antidepressants can also cause this and if you are taking any medication you have to discuss with your treating doctor to change or stop medicine if possible PHYSICAL CAUSES OF ED •Heart disease, high blood pressure, High cholesterol. •Parkinson's disease, obesity, alcoholism, atherosclerosis. •Use of tobacco, surgeries that affect the pelvic area or spinal cord. •Treatments for prostate cancer. PSYCHOLOGICAL ISSUES OF ED: Depression, anxiety, stress and other mental health conditions are some of the psychological issues which causes erectile dysfunction. SYMPTOMS OF ERECTILE DYSFUNCTION •Trouble in getting erection •Difficulty in keeping erection •Reduced sexual desire To Prevent ERECTILE DYSFUNCTION By decreasing the chances of developing several diseases like diabetes, heart disease and hypertension, chances of developing erectile dysfunction can be decreased to some extent. By eating healthy diet, erectile dysfunction can be managed. By doing Exercise daily and by avoiding smoking, risk for developing erectile dysfunction can be decreased. If you do not have any diseases it is only your mind that is preventing your erection. So you need to be calm and stop worrying about it and keep on doing normally.

Hello Hyperhydrosis is a genetically determined condition and it's difficult to reduce sweating by using medication. Localised hyperhidrosis can be controlled by blocking sweat glands. I prescribe aldry lotion to my patients and have seen excellent response on palms soles and axilla. You can try this. Take vitamin b complex tablet daily for 3 months. Thank you.

Dietitian/Nutritionist, Mumbai
Self-care such as exercising, losing weight, not wearing tight clothes, elevating your legs, and avoiding long periods of standing or sitting can ease pain and prevent varicose veins from getting worse. Shedding excess pounds takes unnecessary pressure off your veins. What you eat can help, too. Follow a low-salt diet as prescribed by a registered dietitian to prevent swelling caused from water retention. Compression stockings can also be prescribed based on your condition do reply back for medication prescriptions. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

Compulsive Sexual Behaviour - How to Manage It?

MD - Psychiatry, MBBS
Psychiatrist, Pune
Compulsive Sexual Behaviour - How to Manage It?

One of the emerging psychiatric disorder that has significant medical and psychiatric consequences is compulsive sexual behaviour or hypersexuality, is an obsession with sexual thoughts in which people cannot manage their sexual behaviour. It may involve a commonly enjoyable sexual experience as self-stimulation which becomes an obsession. The person suffering from the condition may also get involved in sexual behaviours that are outside the bounds of commonly accepted conduct like paying for sex or having extramarital affairs.

Untreated compulsive sexual behaviour damages one’s self-esteem, health, job, relationships and career. However, with treatment and self-help, one can manage compulsive sexual behaviour and learn to manage one’s urge. Males are more likely to be unfaithful than females because they have stronger sexual impulses and weaker self-control. Compulsive sexual behaviour symptoms vary in type and severity. During intense sexual impulses, one feels as if they're beyond control. This is the first and the most common symptom as the patient uses compulsive sexual behaviour as an escape route for problems like loneliness, depression, anxiety and stress. One has trouble establishing and maintaining emotional closeness, even if one is married or in a committed relationship.

The treatment for compulsive sexual behaviour involves psychotherapy and medications. A primary goal of treatment is to help one manage urges and reduce excessive behaviours while maintaining healthy sexual activities. If one has compulsive sexual behaviour, one may need treatment for mental health condition. People with compulsive sexual behaviour often have alcohol or drug abuse problems or a mood disorder such as depression.

Counselling sessions can help one learn how to manage one’s compulsive sexual behaviour. Intensive treatment programs for compulsive sexual behaviours focus on identification of core triggers and beliefs about sexual addiction. It assists in development of healthier choices and coping skills to minimize urges and deal with the preoccupation of sexual addiction. Psychodynamic psychotherapy can prove to be very useful as it focuses on increasing one’s awareness of unconscious thoughts and behaviours, and developing new insights into their motivations. Resolving conflicts using this therapy is also recommended. The other kind of treatment program is cognitive behavioural therapy. This therapy helps one to identify unhealthy, negative beliefs and behaviours and replace them with healthy, positive ones. Family therapy and couples therapy may restore trust, minimize shame and guilt thus establishing a healthy sexual relationship between partners.

Medications include anti-depressants like selective serotonin reuptake inhibitors, mood stabilizers and anti-androgens have also been used to treat compulsive sexual behaviour. Anti-androgens are prescribed as it reduces the biological effects of sex hormones in men thus reducing sexual urges. Luteinizing hormone is also prescribed as it reduces obsessive sexual thoughts by reducing the production of testosterone. Anti-seizure medications, naltrexone, and medications which decrease male hormones have been found to decrease the compulsive urges and impulses associated with sexual addictions for some sufferers. If you wish to discuss about any specific problem, you can consult a psychiatrist.

I'm taking insulin 25 morning n 18 at night also taking Jake's 50/500 once sugar remains 190/260 pp n also taking thyrocouy 50. N blood pressie 100/140 taking arbitel 40 sir suggest me.

BHMS, MD - Alternate Medicine
Homeopath, Nagpur
Dear sir if you have type 2 diabetes than this is due to insulin resistance and not insulin deficiency. So taking an additional insulin some times that's why fails to control sugar. In our experience many diabetic patients not only could get better sugar control but in many cases they could get read of insulin doses too. So if you feel we can plan appropriate treatment strategies for you.

There is evidence that certain dietary supplements and vitamins can help with certain types of pain. Fish oil is often used to reduce pain associated with swelling. Topical capsaicin, derived from chili peppers, may help with arthritis, diabetic nerve pain, and other conditions. There's evidence that glucosamine can help relieve moderate to severe pain from osteoarthritis in the knee. Do reply back for private consultation. And detailed treatment plan.
