Gallstones: Causes and The Reason To Opt For Surgery

Gallstones: Causes and The Reason To Opt For Surgery

How and why are gallstones formed?

The bile in your body contains enough chemicals to dissolve cholesterol excreted by your liver. In case your liver starts excreting more cholesterol than your bile has the capability to dissolve, the excess cholesterol crystallizes and forms stones. Bile may become concentrated if the gallbladder doesn't empty in the time leading to the formation of gallstones. A number of factors can increase the risks of formation of gallstones like:

  •  high cholesterol
  •  being obese/overweight
  •  being an American Indian or Mexican American
  •  being female
  •  falling in the age group of 60 or above
  •  a family history of gallstones
  •  rapid weight loss
  •  diabetes
  •  consumption of cholesterol-lowering medications

When should you consider undergoing surgery?

In some cases gallstones that are detected during ct scan or ultrasound (without any symptoms) don't require surgery. If you do not feel aggravating pain in your upper right abdomen or other signs and symptoms, then one can postpone the surgical process according to the need. In few instances, it has been seen that small and solitary gallstones can be dissolved with oral medication like ursodiol and chenodiol. However, you need to opt for surgery to remove the gallbladder necessarily in the following cases-

  •  sudden inflammation or infection in the gallbladder
  •  if you experience recurring instances of intense pain due to the gallstones
  •  certain growths known as polyps develop in your gallbladder which is larger than 1 cm in size and begin to give rise to numerous symptoms.
  •  if you have pancreatitis, which is a condition involving inflammation of pancreas due to gallstones
  •  if the wall of your gallbladder gets calcified
  •  your immune system is damaged
  •  you are experiencing symptoms and your gallbladder has ceased to function

When I went to toilet today, I observed that my stool is in brown color. I could not see any content of blood. It is little brownish in color and went normally without any pain. The urine color is normal only. What is the reason for this?

MBBS, Member of the American College of Physicians
General Physician, Kolkata
The most common cause is dehydration. Try drinking lots of fluids, 3-4L/day. If the problem persists even after that, contact me, it might be an intestinal problem.

Recently I got pain at anal cavity. I went to hospital. Doctor said it was a infection called fistula. How can I overcome this.

Hi, generally fistula doesn't cause pain. It may be a fissure or a perianal abscess. Just consult your doctor again. Get a Trans anal Ultrasound done to get a proper diagnosis.

Ayurvedic Remedies For Thyroid Disorder!

Thyroid has become a common ailment among most women in India nowadays. It is triggered due to the presence of impurities in the blood, which lead to hormonal imbalances and therefore, a number of symptoms, including anorexia, unhealthy weight gain, loss of energy, weakness and fatigue are experienced. But in case of hyperthyroid, you will notice an increased metabolic activity, a rise in pulse rate, loss of weight and palpitation.

Irrespective of whether you are suffering from hyperthyroid or hypothyroid, Ayurvedic treatment has some viable solutions that are aimed at elevating your health condition.

Ayurveda and thyroid

Though there is no direct mention of thyroid in the Ayurvedic texts, there are major implications that hint towards a disease like thyroid that is caused due to imbalance of body fluid and its probable treatments. Thyroid imbalance can be controlled by the following treatment regimen that includes:

1. Dietary regulations: As per Charaka, thyroid doesn’t affect those individuals who take milk in adequate quantities. In addition, you must take moong seeds, bengal gram seeds, sugarcane, cucumber, barley and old rice in optimum amounts for prevention of thyroid. In case you are already suffering from thyroid imbalance, then you must also aim at having a considerable amount of milk on a regular basis.

2. Herbs for regulating thyroid: The herb named Purple Mountain Ebony is purposely helpful in the treatment as well as proper functioning of the thyroid gland. In addition to this herb, you must also intake Punarnava, Brahmi, Jatamansi, Gokshura and Shilajita for alleviating your thyroid problems.

3. Home remedies: If there is swelling in any part of the body caused due to thyroid imbalances, then it can be properly treated by applying a fine paste of Jalakumbhi over the affected area. The juice of this paste can be administered in a small quantity of 22 grams in a span of 24 hours. It helps to increase the amount of iodine in the body which helps to fix various problems in the body. If you are experiencing a lack of energy and weakness, you can use coconut oil for boosting the sluggish metabolism of the body.

4. Yoga for treating thyroid: There are various Yogic postures, which are believed to help in the maintenance of the thyroid gland. You can practice sun salutation, fish pose and shoulder stand for alleviating your condition and get rid of all types of health issues caused from thyroid imbalances.

If you wish to get a personalised evaluation of your health condition, then it is imperative to pay a visit to an expert who can recommend proper Ayurvedic treatment methods for you.

Hi my wife has thyroid enlargement. Recent report says thyroid gland is grown. Also it touches the throat pipe. Can anyone pl suggest to control the growth of thyroid.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Null, Thanks for the query. The reason thyroid grows is because of hypofunction, there is an extra amount of TSH (Thyroid Stimulating Hormone) secreted, that makes the gland size go up. This invariably signifies low thyroxine hormone levels leading to hypothyroidism. Usual cause is iodine deficiency. There is no mention of her thyroid hormone levels (T3, T4 & TSH), as it is important from the point of view of treatment. She may also have to undergo an Ultra Sono graphy of the gland to what kind of changes are present and if there are any nodules inside. Please get these tests done and come back. Then I will be able to guide specifically. Also please tell me if she has any symptoms related to either low thyroxine or high thyroxine. Thanks.

Thyroid - Does It Impact Sex Life?

The thyroid gland is a tiny gland that produces the hormones which affect the body's metabolic process. When these hormones are produced in less or more quantities, it may result in an imbalance of the thyroid gland, leading to a range of thyroid issues including hypothyroidism, hyperthyroidism and others. Overproduction or underproduction of the thyroid hormones can also cause a variety of other symptoms and conditions. While this affects the state of one's general well being, health and weight, it also has an effect on the patient's libido.

Let us learn more about the link between thyroid and sex:

  • Thyroid in Men: Men who are suffering from hyperthyroidism or hypothyroidism as well as related conditions like goitre or Garves Disease, have complained of sex related problems. These problems are known to affect at least 50% to 60% of the patients, as per various medical reports. The sex related issues include premature ejaculation, low sex drive, delayed ejaculation, erectile dysfunction, and others.
  • Thyroid in Women: It has been found that almost 10% of women over the age of 50 suffer from some form or the other of thyroid hormone imbalance. Apart from fatigue, depression and muscle ache as well as weight alteration, depending on the type of thyroid problem, thyroid patients who are women have also experienced problems when it comes to libido. Sexual dysfunction and arousal problems are high on the list of these issues.
  • Hormonal Treatment: One of the most common ways of treating this sex related thyroid problems for thyroid patients is through the use of hormonal therapy. Besides getting the testosterone, estrogen and other androgen levels checked, the patient must also get a full hormonal evaluation along with a check-up of the adrenal function. This can help in pointing any anomalies and the hormones that will require replacement with the help of appropriate medication.
  • Optimal Drug Treatment: A T4 only drug or Levothyroxine can lead to a resolution of sexual problems that one encounters as a part of thyroid problems. There are many doctors who may switch to a T3 drug once the libido is restored and brought back to normal.
  • Supplements: Men may require testosterone supplements, while women may need estrogen or progesterone supplements to tackle the sexual problems in such cases. These can be rendered as a patch or through an injection.
  • Sex Therapy: In many cases, sex therapy considered as a popular and effective form of addressing the problems and helping the patients get around the same. This is especially helpful if depression is one of the symptoms of the thyroid disorder.

A thorough physical check-up also helps in treating such problems for thyroid patients. If you wish to discuss about any specific problem, you can consult a Sexologist.

Mr. lybrate-user, Thanks for the query. Your TSH level is high. It falls in the sub-clinical hypothyroidism. In this condition usually not treated. Only when TSH goes above 10 mU/L But if a person has troublesome symptoms even when TSH is lower than 10 mU/L, then treatment is initiated. I am taking Thyroxine sodium 100 mcg 1 tablet daily for thyroid, no tested again Which results: Total triiodothyronine T3: 1.17 Total thyroxine :10.50 Thyroid Stimulating hormones: 6.57 (0.35-5.50) So my question is :should I continue the same dose or increase or decrease my dose, please reply. Thanks in advance.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, Thanks for the query. Please let me know the age of the patient plus are there any symptoms present related to hypothyroidism? Unless those details are known, it is difficult to say whether dose needs to be increased. Usually when on treatment TSh should be maintained around 3 to4 mU/L, particularly in adults. In older people a slightly higher level of TSH does not really matter. In fact it offers certain protection. So please answer my questions. Thanks.

It's difficult to judge in this case. It can be unrelated also. Please talk to your doctor who has been treating him and discuss about it. What exactly was the diagnosis 1 year back. You can also update all the reports for clarification. You can also take a second opinion if you are not convinced with the explanation from the doctor. Kindly follow up regularly and continue the treatment. Please upload the exact reports.

My thyroid tsh result is 0.13ulu/mL (0.45-4.5) and t4 is 1.06 ng/dl. I am taking 100 mcg tablet for a day. After the blood result ,please mention my dose.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, Thanks for query. It appears the present dose a bit high. You can cut it to 87.5 mcg per day. On treatment TSH should be around 3 to 4 mU/L, so you can reduce the dose as mentioned and then check TSH after 8 weeks. To see if that has helped. Thanks.

Recently I got to know in an ultrasound test that I am having a gallstone and a kidney stone. But there is mild discomfort in the right side of my back. Kidney stone is 4 mm and gall stone is 9 mm. Is this size a reason to worry and how soon should it be operated? Meanwhile what precautions are to be taken?

Mbbs, MD - General Medicine, DM - Nephrology
Nephrologist, Ahmedabad
4 mm kidney stone probably come out without any operation. U need to take 3 to 4 lit fluid intake and low salt diet. For fall stone it's needs gall bladder removal if it's causing pain.
