Symptoms and Treatment for Chronic Pain

Hi. I am Dr. Shantanu Mullick from Navi Mumbai, Pain Clear Clinic. Today, the whole world is suffering from chronic pain.

Million patients they are suffering from different types of chronic pain like back pain, neck pain and cervical spondylosis. Mainly, due to their lifestyle disorders but they are also suffering from different complicated pains like cancer pain, but most of the patients they are suffering from chronic pain with frozen shoulder, knee pain, back pain. They are going for surgery but many times these surgeries may not help in that way. That is why this newer therapy, intreventional pain therapy which is nowadays throughout the world and very much effective to support this kind of chronic pain to rule out the disease as well as to treat the level of the root cause.

Normally, the patients who are suffering from the back pain, many times I have seen that after regular therapy , physiotherapy, exercise, they are not getting relief and they are going for the surgery. In those cases, interventional pain therapy will help the to rule out the cause. We do treat these patients according to MRI reports but all these treatments are done without surgery. So many times, surgery may not help these kind of back pains and in cervical pains. So in those pains, interventional pain therapy, without damaging the anatomy helps a lot. So if you see the pain where we can see the patient is suffering from disc pain. We can see here the nerve root coming there and the disc is bulge and we commonly call it as slip disc problem or disc hernation. In those cases, the surgeons are getting the bones, there is a lamenectomy but without doing surgery as per MRI report we are under forescopic guidance in the operation theatre we are going inside the disc and treating them without any surgery and day care.

So patients automatically get very good relief and after that we are able to support the patients with physiotherapy exercises and this way we can help different types of patients suffering with back pain, neck pain. On the other hand, in case of cancer pain also when the patients are not getting relief with the morphine or from very strong pain killers, we can support them with interventional pain therapy to block their pains permanently working in the spinal cord.

Another, dreadful pain condition is trigeminal neuralgia where patient is suffering from pain in the face usually the pain comes where patients complain of sharp shooting, electric shock kind of pain. It will appear on one side of the face. Actually the trigeminal nerve is coming from the brain and it is supplying the nerves of the face in three ways. Usually it happens in one side and those patients initially are treated with medical therapy and any times they are not getting support in those cases we are treating with radiofrequency ablation therapy. So different types of new treatments have for this chronic pain where patients without knowing their disease, they keep on taking pain killers and damaging their other organs of the body so that is why pain management nowadays is a very very effective subject and separate speciality to support the chronic pains those who are suffering from long types of disease.

Dr. Shantanu Mallick is a distinguished Physician in Navi Mumbai. During his career of 22 years, he has won much laurels and appreciation. His speciality of dealing with chronic pain has made him one of the most sought after physicians in Mumbai. After completing his schooling, he joined R.G. medical College in Kolkata. He completed his MBBS in 1995. After that he completed his Diploma in Anaesthesia and Pain Management in 1999 from MGM College in Navi Mumbai. He then became the first Physician in Konkan Belt to get the Fellowship in Interventional Pain Practice (FIPP) degree from World Institute of Pain, UK. As a pain physician, he has a lot of experience. He worked at Pioneer in Interventional Pain therapy in Navi Mumbai.

He also worked as a Consultant Pain Physician at Fortis Hospital & MGM Hospital and DY Patil Medical College in Navi Mumbai. Chronic Pain - Ways You Can Cope With It!

There are numerous people worldwide who suffer from chronic pain. The experience can be quite annoying and painful, not only for the affected person but also for their loved ones and family members. Coping with the situation can be quite a challenge. Chronic pain can leave a person physically as well as emotionally exhausted. Thus, proper management is needed to deal with the situation better.

Causes of chronic pain
Chronic pain can be due to numerous reasons such as

  1. Use of wrong footwear (especially high heels in case of women).
  2. Injury due to rigorous and strenuous exercise.
  3. Unhealthy lifestyle habits, poor posture.
  4. Chronic pain can also result from a traumatic injury.
  5. Medical conditions such as Fibromyalgia (a condition characterized by muscular pain, especially in the shoulder and neck region), AIDS, Cancer, Arthritis (Osteoarthritis and Rheumatoid Arthritis), Multiple sclerosis, to name a few, can give rise to chronic pain. Obese people are at a greater risk of suffering from chronic pain.
  6. In some people, incorrect sleeping posture and the wrong choice of sleeping mattress can cause chronic pain.
  7. With age, a person is likely to suffer from chronic pain (age related problems and disorders).

The effectiveness of the treatment depends on the severity as well as the factors responsible for the chronic pain.

  1. Medications such as NSAID (Nonsteroidal Anti-Inflammatory Drugs), Tylenol, provide relief in case the pain is not very severe.
  2. In severe cases, doctors may recommend the use of corticosteroid injection as well as powerful painkillers such as Fentanyl, Lortab or Roxicet.
  3. Certain antidepressants and antianxiety drugs such as Duloxetine and Valium respectively may also be used to improve the condition.
  4. Physiotherapy and Bioelectric therapy (the bioelectric therapy works by blocking the pain messages or signals to the brain, thereby relieving the pain) also bear fruitful results.

In addition to the above treatment and medications, certain changes and modifications in lifestyle habits can also help to cope with the chronic pain better.

  1. People with chronic pain should avoid stress, anxiety, and depression as much as they can. These are nemesis that can aggravate the pain further. Meditation, yoga, breathing exercise, are great stress busters and mood enhancers.
  2. Obesity is one of the underlying factors responsible for chronic pain. Maintain a healthy body weight. Eat healthy and nutritious diet, exercise regularly (even if it is only for 30 minutes). For obese people with chronic pain, rigorous exercise can prove to be harmful. Indulge in light to moderate exercise. Seek an expert help if necessary.
  3. Do not compromise on your sleep. Sound sleep can work wonders for the chronic pain. Follow a healthy sleeping routine.
  4. Refrain from smoking and drinking. These can make the situation worse.
  5. Gentle massages can help relieve the pain to some extent. Again, it is advised to seek an expert guidance.
  6. Alternate medications like Chiropractic and Acupuncture have helped many patients with chronic pain.
  7. Avoid doing very strenuous physical activities.
  8. Join a support group. You will not only feel better but will also be able to deal with the situation better.

Middle Back Pain - Probable Reasons Behind It!

Middle back pain indicates a discomfort or pain in the junction of the rib cage and the spine. This area is also known as the thoracic spine. The level of pain can vary from chronic, acute to mild depending on the nature of the underlying cause. With a right diagnosis, there are plenty of treatment options available to arrest the pain and aiming at a permanent cure.

Probable reasons for middle back pain?

  1. The lifting of heavy objects repeatedly or improperly can cause damage to the blood vessels and muscle tissue resulting in frequent pain in the back area.
  2. Discs are located in between the spinal vertebrae. They act as a cushion against shock and help in physical movement. An injured disc results in leaking of the liquid nucleus in its outer casting. This results in a strain in the surrounding tissues, ligaments and nerves pose a serious threat to the spine. A disc injury poses severe pain and discomfort in the middle back region.
  3. Often high impact accidents or violent falls can result in a vertebrae fracture. This kind of fracture necessarily has a co-relation with spine fracture and can result in intense middle back pain. In certain cases, a fractured vertebra can affect the spinal cord as well.
  4. Patients with osteoporosis can suffer from middle back pain too. This is a condition where the failure to regenerate old bone by the body poses a threat to the individual.
  5. People who are aged often faces middle back pain due to a condition such as the spinal stenosis- narrowing of the spinal cord.
  6. A condition known as the spondylitis that leads to inflammation of the spinal joints might lead to pain in the middle back area.
  7. Problems such as scoliosis can result in middle back pain. This happens due to the disruption of the weight distribution of the spine.
  8. Even tumors can upset the basic architecture of the spine leading to pressure on the tissues, nerves, and ligaments that result in middle back pain.

Symptoms of middle back pain:
Some of the typical symptoms of middle back pain include muscle spasms, sleeping problems, the stiffness of muscle in the morning, fatigue, weakness, tenderness, numbness in the back area, headache etc.

Probable causes, detection, and cure:
Some of the probable causes of back pain include obesity, family history, pregnancy, sedentary lifestyle etc. Over the counter medicine, heat and ice treatment, massage etc can be initially tried to get relief from the pain. If this does not fetch any result, a doctor must be immediately consulted. A Doctor first goes for a physical exam to understand the origin of the problem. He can recommend imagery tests such as MRI scan, CT scan etc. to further ensure the root cause of the problem. Once this is done, a doctor might suggest minimally invasive procedures such as fusion, discectomy, and microdiscectomy.

Spondylitis includes swelling of the vertebra. It happens because of wear and tear of the ligament and bones found in your cervical spine, which is in your neck.   4. Acupuncture: Acupuncture is a highly effective treatment used to mitigate back and neck pain. Little needles, about the extent of a human hair, are embedded into particular points on the back. Every needle might be whirled electrically or warmed to improve the impact of the treatment. Acupuncture works by prompting the body to deliver chemicals that decrease pain.

Causes Behind Nerve Pain And How To Control It?

Nerve pain or peripheral neuropathy, as it is medically known, is characterized by pain, numbness and weakness that arise out of nerve damage. This pain is usually restricted to the feet and the hands.

What are the common causes?

Diabetes is the most common cause for this condition. However, other causes, such as injuries, infections and prolonged exposure to certain toxins can also trigger nerve pain.

How can you control nerve pain?

Nerve pain can be managed and taken care of at home taking into consideration the points that have been mentioned below:

  1. Topical Painkillers: Certain over the counter ointments and creams can help relieve nerve pain. These medications act as local anesthetics; that is they numb the area of the pain. Capsaicin, a derivative of chili peppers, is one of the major ingredients used in these medicines. Others use botanical oils as the major ingredients.
  2. Painkillers: There are painkillers available OTC which can curb nerve pain. However, painkillers such as ibuprofen, aspirin and acetaminophen do not really work well for severe pain of the nerves.
  3. Supplements: The lack of Vitamin B12 is considered to be a very important cause, and at times, a catalyst for nerve pain. In a case such as this, Vitamin B12 supplements or injections might be administered. Although more research needs to go into this, but certain supplements, especially gamma-linolenic and alpha-lipoic acid can help soothe nerve pain caused by diabetes.
  4. Acupuncture: This time honored Chinese approach is a proven alleviator of nerve pain. Acupuncture releases chemicals which numb the pain, or prevent the pain signals from travelling to the brain.
  5. Physical Therapy and Massages: Physical Therapy can especially heal and strengthen weak muscles that can stem from acute nerve damage and pain. Though more evidence is needed, according to a few sufferers of this condition, massages can be helpful with the tremendously painful muscle spasms that can so often result from nerve pain.
  6. Relaxation Techniques: In combination with the above mentioned approaches, relaxation techniques can go a long way in relieving debilitating neural pain. Besides being helpful with dealing and reducing a few stress phases of life which can cause nerve pain, these techniques might actually root out the actual pain itself.
  7. Lifestyle Modifications: This one’s always a winner! Eating a well-balanced diet that guarantees all nutrients in uniform amounts can never go wrong. Regular exercises for about 30-45 minutes can remedy different types of pain; nerve pain being one of them. Try and limit alcohol intake (a peg once a week won’t harm one, but binge drinking can play the ultimate spoilsport and make all health plans go topsy-turvy!). Abstain from smoking or chewing tobacco as this is usually the primary contributor towards any medical condition or disorder. 
  8. Interventional pain therapy: It is newer mode of pain therapy for many neuropathic pain conditions, where regular pain therapies don't work. It is done in operation theater under fleuroscopy guidance as per MRI or other investigation report with clinical examination.

Hi Last month I tried to commit suicide by having 40 sleeping pills and 6 pain killer. My hubby saw me after 4 hr n took me to hospital. There doc cleaned my stomach and started iv. Now I'm feeling too much head ache I'm not able to bare it. Wat can it be.

First of all don't kill yourself. Life is only once. It's up-to you how you take or accept things. You can lead your life without expecting anything from others. You will be happy. Do your duties only but don't expect anything from others. Be happy giving others. One day you will start getting love of others.

Actually if you are not ready to go for surgery, you can try with PRP therapy or platelet rich plasma therapy. It may help you.
