I am not able to gain my weight even if I am having my food properly. I am having right amount of calories per day.
Yes, you need to increase the quantity of food you eat, but at the same time you have to make sure You are eating healthy. Eat five to six meals a day. Avoid skipping any meal. Increase your portion size. Choose foods that are higher in healthy calories. Do not skip any meal, especially breakfast. Whatever you eat at breakfast is better absorbed and utilized by your system. So don’t forget to take a handful of nuts everyday along with good quality protein, like toned milk and boiled eggs. Obesity - Possible Causes Behind it

Msc. - Food Science
Dietitian/Nutritionist, Bangalore
Obesity - Possible Causes Behind it

According to WHO a third of the world population is obese, obesity is caused by overeating, lack of physical activity, age factor, hereditary and sometimes malfunctioning of the endocrine glands causing excessive fat accumulation. Food is meant to generate energy in the body so as to enable each and every organ to discharge its individual functions. To avoid overeating one has be careful about portion control and eat at a fixed time. Each meal should have a combination of carbohydrate 50 to 60 % of total calories + protein 35 to 40% of total calories and fat 20 to 25% of total calories. Dietary fibre of 25 to 30 grams a day can obtained from whole grain, fruits and vegetables should also be taken in sufficient quantities as it imparts a sense of satiation and reduces the food intake.

Indian diets are usually high in carbohydrates leading to higher incidence of diabetes. This can be countered by increasing proteins in each meal along with fibre, which gives a controlled balance to sugar levels in the body. Use unsaturated vegetable oils like olive, rice bran, soyabean and sunflower oil to maintain and reduce cholesterol and maintain heart health along with Omega-3 from flax seed, nuts (almonds and walnuts) or fish oil supplements, including vegetable juices in your daily diet improves the antioxidant level and fibre in the body and helps in weight loss.

How to make meal plans customized to your BMR

Step 1 - Calculate your BMR

Women's BMR Men's BMR
655 66
+ (9.6* of your weight in kilos) + (13.7* your weight in kg)
+ (1.8*your height in cm) + (5*your height in cm)
- (4.7*your age in years) - (6.8*your age in years)
= ___________ Calories per day = ___________ Calories per day

Step 2 - After calculating your BMR ,and to start weight loss cut 200 kcal from your BMR.

For example if your BMR is 1600 kcal choose a meal plan that is 200 kcal lower I.e is from 1200 to 1400 calorie meal plan.

The main meals should be three meals like your breakfast ,lunch and dinner and the in between meals should be like fruits ,veg juice ,dry fruits, dry bhel , roasted chana, a slice of cheese ,which are not very dense in calories and high in nutrients.

STEP 3 - Eat every two hours with three main meals and three small meals.

STEP 4 - Drink plenty of water through the day at least 8 to 10 glasses.

Exercise - Lack of physical activity means there is no energy output and this results in weight gain over the period of time because as we age our BMR reduces (basal metetabolic rate : that is total number of calories your body requires for normal body functions excluding any activity factor like exercise, BMR is the energy used for metabolic process of maintaining life itself and usually accounts for two thirds of total daily expenditure ). To maintain a healthy weight increasing the BMR is essential and this means increasing the muscle mass over the fat mass as 1 pound of muscle burns 6 calories at rest and 1 pound of fat just 2 calories. Increasing muscle mass can happen only through physical exercise and adequate protein in the diet thereby, increasing fibre and reducing fat in the diet.

Advantages of Physical Activity

  1. Helps to keep digestive system in optimum condition
  2. Improves cardiac rhythm and output
  3. Maintains flexibility of body
  4. Energies blood vessels and does not allow cholesterol deposits and improved heart health as it does not allow fat accumulation
  5. Improves peristaltic action of intestine and rectum and removes constipation
  6. Helps in improving insulin resistance in the body especially in PCOD and PCOS (polycystic ovarian syndrome)
  7. Maintains the sugar levels in diabetic patients

Other Causes of Obesity

  1. Age factor: Obesity is generally prevalent in the middle age as the BMR reduces as a person ages especially if he or she does not exercise, but weight gain can be gender specific as women have more fat mass and the weight gain during pregnancy is common due to increase in fat in the diet, but in recent years there has been a rapid rise in obesity in children and teens due to over indulgence in junk and fast foods, which have high amounts of refined carbohydrates and hydrogenated fats. Heredity also plays an important role in fat accumulation. Children of obese parents do have high chances inheriting obesity from their parents as genetic factor could be a contributing factor rather than excessive food. In such cases physical activity plays a very important role for weight loss
  2. Untreated Hypothyroidism: Hypothyroid causes a drop in the BMR and increase in fat percentage and if untreated can cause weight gain, but once on medication they have improved metabolism.
  3. Drugs: Use of oral contraceptives, steroids, insulin and phenothiazines is generally followed by weight gain to stimulation of appetite.
  4. Alcohol: Alcohol is just empty calories and 1ml of alcohol gives 7 calories and this causes weight gain along with the fat laden food eaten while having alcohol, overeating is usually a consequence of drinking leading to weight gain and drinking coupled with smoking increases the lipid levels in the body affecting the heart health
  5. Fasting and Yo Yo Dieting: The process of losing weight and gaining weight several times throughout a life time is often characterised by increased fatness with each cycle (Krause food and nutrition therapy). Fasting is dropping calories to less than 600 to 800 over a period of time, which causes starvation and breakdown of muscle mass and lowering of BMR and once one gets back to normal diet there is sudden surge in weight which then gets difficult to rid off.
  6. Water: Incredible as it may seem water is a very important catalyst in weight loss, water suppress the appetite naturally and helps metabolise stored fat. Drinking water is best for fluid retention caused due to excessive intake of salt, the more water you take, more it dilutes the excess salt and reduces the fluid retention. Eight to twelve glasses of water in a day is the optimal intake.
  7. Stress: Stress is a another precipitating factor for weight gain which causes damage to the heart. Excessive stress causes an increase in cortisol and thereby, increase in fat percentage in the body and weight gain. An emotionally charged and highly strung nervous system can adversely affect the coronary arteries and deteriorate the heart. If you wish to discuss about any specific problem, you can consult a doctor.

Thanks for the query. There’s no overnight fix when it comes to healthy weight loss, but you can adjust your lifestyle to promote quick, natural weight loss for the benefit of your health and your appearance. Successful, long-term weight management needs to focus on more than the food you eat and the pounds you lose. It needs to focus on your overall health. Reshape your body and your lifestyle by adopting healthy habits and breaking unhealthy ones. Replace all processed foods in your diet with vegetables, fruits, whole grains, low-fat dairy, lean meats, eggs, legumes, nuts, seeds and seafood. Focus on the variety of whole, unprocessed foods you can eat instead of dwelling on the junk food you eliminated from your diet. Eat smaller portions at every meal and avoid going back for seconds. Take a smaller scoop of rice,  half of a piece of chicken instead of a whole piece and one less egg in your morning omelet, for example. Make time for breakfast every morning. Eating breakfast helps prevent overeating and is a habit of people who successfully lose weight. Pack to-go meals instead of eating convenience foods when you’re not home. Store foods like chopped fruits and vegetables or cooked grains, legumes, eggs and meats in your refrigerator so you always have something to grab when you’re in a rush. Buy fresh vegetables and fruits from the grocery store if you forget to bring healthful food from home. When you can’t avoid dining at a restaurant, opt for a salad as a starter, and always ask for the dressing on the side. Clear soups, lean meats, seafood and grain-based dishes like roti, whole grain bread are other good choices. Exercise for 30 to 60 minutes at least three times per week to build muscle and burn more calories. Pilates, strength training, aerobics, swimming, dancing and yoga or a sport like football are ideal choices. If you’re new to exercising, start with brisk walking and jogging. Adopt physically demanding hobbies like bicycling, hiking, jogging, power walking to promote faster weight loss. If you don’t have time for new hobbies, increase your physical activity with things like walking the dog, playing with the kids or jogging instead of driving to a nearby store or taking the stairs instead of the elevator.

I am 17 years old, . The food over here is too oily, and moreover here we don't get enough time to do yoga, arabics. And so I am increasing weight day by day. Now its 68kg And due to this reason I feel so lazy.

B.Sc. - Dietitics / Nutrition, Nutrition Certification,Registered Dietitian
Dietitian/Nutritionist, Delhi
I am 17 years old, . The food over here is too oily, and moreover here we don't get enough time to do yoga, arabics. ...
I understand your concern. First of all avoid that oily food. I will give you a diet chart that will help you stay fit and healthy. Secondly, try to take out 15 mins for yoga everyday. If it is not possible at all, then keep yourself a bit active all day. For example - Take a break of 5 mins after lunch and go for a walk, take stairs whenever it is possible to climb, avoid lifts, avoid any transport and walk or cycle to your place. Small measures go a long way in controlling weight. Some other tips: 1. Watch what you eat – Are you eating too much junk, drink excessive alcohol and eating too much white food and sugar? Please cut it down to as low as you can from tomorrow. Cut down tea/coffee if you are too addicted to it. It is creating havoc inside your body. 2. Lifestyle – Are you a late sleeper and late riser? You can’t imagine what this schedule of yours is doing to your metabolism. It is degrading your digestive system from inside and your looks from outside. 3. Physically active – If you are a couch potato who loves to watch tv whole day then throw that couch of yours in the street and get yourself a treadmill tomorrow. Yes it is as simple as that. Who is stopping you? You have to be physically active all day long if you want to burn those extra calories. Do daily brisk walk, Pranayam, atleast 20 surayanamaskar everyday to start seeing the results. Take break between office work to walk a little or rather just climb some stairs to get that food you ate in lunch have space and heat to digest and absorb. What’s the harm in using your washroom to do a little middle of the day squats. Do that – No one is watching you. The key is to stay physically active all day long, no matter what. 4. Eat healthy and eat on time: Not only eating healthy is important but eating at right time is equally imp too. Have an early breakfast and early dinner and have 4 small meals between breakfast and dinner. To guide you a bit, here you go: A. Breakfast (Between 8-10 a. M) – Have oats/porridge or brown bread with milk and nuts. B. At 11 a. M - Cup of Green Tea c. Mid day meal (around 12 p. M) – Have a bowl of fruits. Citrus fruits are best taken during this hour and absorb well in the body. Have oranges, amla, pineapple etc during this hour. D. Lunch (between 2-3 p. M) – Daal, roti, mix veg, buttermilk and salad. You can also take brown rice with daal in lunch. Try to get your food cooked in olive oil. Lace your salad with apple cider vinegar, black pepper and seeds like flaxseeds or chia seeds to make it nutrition packed lunch. E. Between 4 p. M – 5 p. M – Cup of Green Tea F. Evening (4-6 p. M) – Have some nice sprouts with tomatoes and cucumber. G. Dinner (2 hours before sleeping, preferably before 9 p. M) – Panner & veggies salad with olive oil, Roti or brown rice with Daal. H. Have a glass of warm milk. Weight loss is 80% about what you eat and 20% about exercises. That said, both the things go hand in hand. If you want to lose weight in a more healthy way than be patient and just follow the regimen everyday. Slow and steady wins the race.

Natural way of weight loss is by routine exercise like walking, running, yoga and healthy diet which contains boiled vegetables, more fiber, low carbohydrate and fatty food. Medicines work temporarily for weight loss. Skipping meals will not help. If any medical condition is related then that can be treated.

There are several things you can do to prevent a cold. they are all easy. -Always wash your hands after coughing or sneezing into your hands, blowing your nose, after touching someone who is sick, or when using public restrooms or telephones. -Cover your mouth and nose when you sneeze or cough. -Get plenty of rest/sleep.staying at home will keep you from spreading germs.Avoid people who have a cold or cold symptoms. -Drink plenty of fluids(veg/tomato soups, dal water/coconut water/herbal tea etc), and eat nutritious food(like sprouts/boiled eggs without yolk with milk). Avoid foods or drinks with caffeine which can impair your sleep -Treatment:A cold usually lasts for a week or two. Unless you develop complications, you shouldn't need medical treatment. -Complications like infections of the ears, throat, sinuses or lungs must be treated with antibiotics. -Gargle to keep from getting a sore throat. -Use saline nasal sprays or nasal irrigation. -Tablet nasorest-p once/twice daily may ease breathing. -Do steam inhalation once or twice daily -Cough- For dry cough.Take syrup Alex-D for a dry cough.(1 tsp thrice daily) -Productive cough:it brings up mucus from behind the throat or the lungs. These coughs should not be suppressed to the point that they no longer bring up mucus. Use syrup Alex. -Get chest xray done if cough lasts for more than a week and consult doctor immediately. -Head and body aches- take tablet Dolo-650 mg for fever once or twice daily.

Rheumatoid Arthritis

MBBS, MS - Orthopaedics
Orthopedist, Delhi
Rheumatoid Arthritis
Rheumatoid arthritis

Rheumatoid arthritis is an autoimmune disorder affecting the joints, symmetrically (usually). The joints become swollen, red, hot, painful & tender. The joint movement becomes limited due to pain & swelling
It starts with inflammation of the synovial membrane but may involve the joint capsule and other surrounding soft tissues.

It is a self limiting disease. But by the time it leaves, the patient is crippled due to contractures & deformities.
Acute phase can be managed with medicines.

Commonly used medicines are ibuprofen/paracetamol/aspirin in therapeutic doses. They help to control symptoms of pain and swelling of the joints in most of the cases by their anti inflammatory properties. Fever is also controlled.

Vitamins, minerals, anti oxidents & other supportive medicines are also needed. Omega 3 fatty acids have a very definite role to play in improving the response to the treatment.

Physiotherapy helps in preventing contractures & deformities.

The patient should do general exercises & keep active.

He may have to be further investigated.

In established cases of rheumatoid arthritis dmard (disease modifying drugs) e. G. Methotrexate, hcq, leflunomide, saaz etc. Have to be used.
They all are toxic drugs & can cause bone marrow depression along with other side effects. They should be taken under medical supervision.
Sometimes steroids may have to used.
The dose of these medicines should be adjusted to the minimum required, to control the symptoms.
Blood, urine tests & other investigative procedures should be done periodically to rule out any side effects.
X rays of the affected part should be done to confirm diagnosis & assess the extent of damage.
Diabetes, vitamin d deficiency or any other metabolic disorder should be ruled out. Sometimes they mimic the symptoms & create confusion in diagnosis. Associated disorders like these, most certainly affect recovery adversely.

It should be ascertained that the patient is not allergic to any of the medicines that he is going to take.

Rheumatoid arthritis is a chronic disorder that lasts over many years with acute exacerbation & remissions. The earning capacity of the family is adversely affected. The cost of medical treatment is high. If the patient needs surgery, it is further escalated. Most of the population in india is not covered by any medical insurance. All these factors put together create an atmosphere of depression in the family. Patient feels morally degraded & may even develop suicidal tendency.
In a situatation like this it becomes essential that a moral support is provided to the patient by his friends & relations. Treating doctor has a very definite role to play in keeping up the moral of patient.

diet and workouts go a long way in helping you gain strength to gain weight, you must eat more calories than you burn. When you gain weight, some will be fat and some will be muscle. To gain muscle, you must eat protein. To boost the ratio of muscle to fat gain, you must exercise. To gain muscle and strength, you must engage in resistance exercise. To improve bodyfat percentage and gain weight, you must cycle through phases of gaining muscle and losing fat. I being a both a registered dietitian (rd) and doctor have been successfully helping patients with their problems through a holistic approach using customized therapeutic diet and medications. I will also suggest home remedies. Do reply back for private consultation for rx.
