How To Manage A Patient Recovering From Brain Surgery?

Undergoing Brain Surgery can be a very traumatic experience, and it is common for many patients recovering from brain surgery to face depression, spells of dizziness, confusion and weakness post the surgery. It can be very critical that family members and friends talk to the patient and be empathetic towards them.

It takes approximately 12-18 months for the brain to heal after a brain surgery and slowly and gradually the patient will regain all his normal functions and get back to his daily routine. However in that time they need the complete support and understanding of their families, as well may need help from therapists. This will help the patient in gaining back their independence as well as confidence in their abilities.

Here are some tips that will help you deal with a patient recovering from Brain Surgery:

  1. After brain surgery, a person may feel disoriented and have some speech or understanding disability for a while. Family members and friends are advised to take pause when talking to the patient, so that he/she can easily understand the conversation. Speaking slowly is not recommended, as patients may recognize it and have an emotional outburst or feel hurt.

  2. Caretakers and family members should also keep reminding the conversation topic at different points to the patient, so that it is easier for them to participate in the conversation.

  3. Family members should also not react adversely in case of emotional outbursts, instead show love and patience to a person recovering from brain surgery.

  4. Caregivers should make sure that the person recovering from brain surgery gets enough sleep and rest to recuperate.

  5. People interacting with someone recovering from brain surgery should understand that the person's ability to learn and remember will improve daily, and any lapses in attention by the patient are not caused by any act of obstinacy. Your care and understanding will be essential for a person to recover.

  6. Family members should also make sure just to give enough care and not smother the brain surgery survivor. It is essential for them to regain their confidence and a sense of competence.

Family members should take the patient for a neuropsychological examination--after treatment, 6 months later, and a year later to see if they are recovering properly. Caregivers should watch out for emotional outbursts, like rage, uncontrollable laughter, withdrawal and depression. In case of such symptoms it is advisable to take the patient for a check up with medical practitioner.

Conditions related to the eating patterns or behavior of an individual which leave a negative impact on the health of the person are known as eating disorders. These can lead to the deficiency of sufficient nutrition in the body. The heart, the digestive system, the bones, and the teeth and the mouth are the body parts that generally get affected due to this condition.

Although the root cause is not known exactly, there are several factors which can be held responsible for causing this type of disorders likes:

1. Genetics-A person, who has parents or siblings suffering from eating disorders, has higher chances of suffering from it as well.

2. Society- In order to fit into the society, people often find it very necessary to look the best, which means being thin. Peer pressure and the worry about what people say fuels the desire, more like desperation in a person to become thin, which may lead to an eating disorder.

3. Emotional and psychological health- Problems directly linked with the psychological and emotional health of a person, like low self-esteem, impulsive behavior, relationship associated trouble and perfectionism can also lead to eating disorders.

3. StressAnyone who is going through any kind of emotional/mental stress has high chances of getting an eating disorder.

4. Family history-People who have a family history of this condition, i.e. their parents or siblings had suffered from it earlier, have higher chances of getting any kind of eating disorder.

5. Other mental disorders-Obsessive compulsive disorder, depression and anxiety are some mental disorders which may trigger this condition in an individual.

Any form of regular exercise is a must for a healthy lifestyle. While some people prefer to work out in a gym, others like the calmness of their own house where they can practice yoga or a walk in the park. Each style of exercise has its own pros and cons. Let's take a look at working out in the gym vs yoga and regular walks.

Benefits of yoga and walking over going to gym:

Good for both - the mind and the body
Yoga is a lifestyle and not merely an exercise routine. Thus, it not only builds and tones muscles but also strengthens your lung capacity, digestive system etc. It is also said to increase the ability of the mind to focus and attentiveness.
You could walk on a treadmill in the gym but taking a walk outdoors does more good to your body. Being with nature when you walk outdoors is known to be an effective stress buster and helps you calm your senses. Walking can also help improve your moods and is a form of treatment for depression as well.

Does not need any special equipment
With yoga, there is no need for weights or special shoes. All you need is a flat, firm surface which could be the carpet in your bedroom or your lawn outdoors. A reason most gym enthusiasts prefer a strenuous workout to yoga and walking is faster weight loss. These techniques do help lose weight but the process is slow. However, since this weight has been lost gradually, it is more likely to stay off.

Almost all of us have experienced bloating, gas or an upset stomach. Yet some people experience this problem at a chronic level. Gas occurs in the stomach and intestines due to breakdown of food into energy. All of us pass gas, however some people do more than others. An average person passes gas 5-20 times a day.

An upset stomach can often cause problems and can be a difficult battle for many people. Although the symptoms of gas problems often relate to an improper diet, yet other factors may be also responsible.

Causes and Symptoms of Gas Problems

  1. Swallowing air: The swallowed air which is not burped out, passes through the digestive system and is released through the opening of the anus. Hiccups can also occur if you have swallowed excessive air.
  2. Diet: The reaction of different foods varies from one person to another. Also foods that lead to odorous gas may also vary from person to person. Yet, spicy foods and dairy products are the most common causes of gas. Example - Beans, cabbage, onions, radishes, eggs, carbonated drinks, sugar, packaged foods etc.
  3. Medications: Certain medicines and supplements, including prescription drugs, can lead to gas problems and bloating. Further, medications which change the hormonal balance in the body can also cause bloating.
  4. Medical problems: Certain medical conditions such as Irritable bowel syndrome or bowel obstruction can also lead to an upset stomach.

Common symptoms of gas problems include gas pain, cramping, bloating, diarrhoea and constipation. In the worst of cases, gas and bloating indicate colon cancer. Such people experience rapid weight loss, anaemia and blood in stools.

Treatment
Modifying your lifestyle, in particular your dietary practices, is essential to a healthy recovery.

  • Avoiding foods such as spaghetti, oats, bran, barley, banana, bread etc. which contain high fibre content is important.
  • You can opt for over the counter medications after consulting your physician. Homeopathy and Ayurveda can be also opted as alternative forms of treatment.

Pre-diabetes is a serious medical condition which puts you at higher risk for getting type 2 diabetes. But, there are a number of things that you can do, changes that you can make like avoiding certain foods, controlling the portions of your meal and the number of hours you exercise in a week that will help you prevent type 2 diabetes.

Without pulling any punches, let us say this very clearly that any healthy changes that you make during pre-diabetes will save you from an extremely serious disease called diabetes which kills due to various complications.

Type 2 Diabetes and Pre-Diabetes

  • What happens when you have type 2 diabetes is that your body does not produce enough insulin, the hormone that metabolizes sugar or is not able to use it properly. Your blood sugar therefore, shoots up alarmingly causing all the diabetes symptoms we already know of.
  • Pre-diabetes is a condition when your fasting blood sugar level is above normal. It is diagnosed through a blood test after you have fasted overnight.
  • Normal fasting sugar is 60 to 99 milligrams per deciliter and in pre-diabetes, it is 100 to 125 mg/dl, whereas diabetes sugar is 126 mg/dl or higher on 2 occasions.
  • If you have pre-diabetes, your doctor will counsel you about developing a lifestyle plan to reduce your risk of getting type 2 diabetes.
  • Most recommendations hover around losing 5-10 percent of your body weight if you are obese, more exercise and control of your portion size.

Portion size control for pre-diabetics

  • You will be advised to cut your calorie intake by 250 to 500 hundred calories per day if you are overweight.
  • Certain foods which are basically carbohydrates will go off your plate forever. Carbohydrates are high glycemic index foods as they convert to sugar very fast in the body and trigger insulin.

  • Pre-diabetics should keep the calories from carbs to the minimum. They should get their maximum calories from proteins and fats.
  • Which means their plates should have more lean meats and eggs, less fats and minimum amounts of carbs, like wheat.
  • This has a number of benefits for blood glucose control. Eating more fat and protein-based foods will limit the uptake of sugar from your bloodstream. Fat is insulin-neutral and so are proteins. This means they don’t trigger insulin.
  • Since an increased waist size is associated with decreased sensitivity to insulin, keeping your calories down by eating moderate portions of food is the best way to stop progression to full-blown diabetes.

Managing carbohydrate intake

  • Pre-diabetes means that your body doesn’t tolerate carbohydrates. Think of it like this, in a meal, having half a banana is all your body can tolerate in terms of carbs.
  • Use can also use the plate method which involves dividing up your plate into sections for different food groups. It helps you to keep control of portion sizes as well as helping you to enjoy a well balanced diet.

Paan which is prepared using tender green betel leaf and areca nut is very popular and widely consumed in many countries of South and South East Asia such as India, Bangladesh, Pakistan, Indonesia, and Malaysia. There are many variations to the preparation, some might contain tobacco, slaked lime paste also known as choona and a paste that is known as kattha which is a breath freshener.

The betel leaves used to make paan have a range of healing and curative benefits. The leaves contain vitamins such as niacin, riboflavin, vitamin C and many more. Chewing paan can, therefore, have a good impact on your health.

  1. Digestive Health: The chewing action when consuming paan helps to stimulate the salivary glands. This causes the secretion of saliva which contains the enzymes necessary to ensure that the food is properly broken down. This aids in a good digestion. The drawback of surgical approaches (such as vasectomy), especially patient compliance and the low success rates with condoms has spurred research on hormonal contraceptive dosage forms. There is a dearth of investigations in the development of pharmaceutical preparations. Evolving technology in the 21st century as well as improvement in living standards further underline the need for new male contraception approaches, especially those that entail new drug delivery methods.

Here are few areas where male contraception is being researched and used in some countries

Hormonal contraception for men is possible, and we are at the threshold of an important breakthrough. Combined testosterone plus progestin administration is more effective and safer than testosterone alone. Combination of testosterone plus an anti-androgenic progestin has several advantages over other formulations. In combined therapy, single injection formulation may have better compliance. 

  • Conventional male contraception 
  • Abstinence (doesn't work well)
  • Masturbation
  • Male condom
  • Vasectomy

There are other methods being developed but not sure they are there yet. But for guys? Their options are stuck in a time warp. If a man wants to take pregnancy prevention into his own hands, his choice basically comes down to condoms, a vasectomy, withdrawal, or abstinence. That’s why it's so amazing that scientists are finally developing some real advancements when it comes to male contraception.

Researchers writing in the April issue of the Open Access Journal Contraception published a rundown of the top emerging options. A few hold real promise, particularly a daily or weekly pill that would deliver a dose of artificial hormones to a guy’s bloodstream, which would then act on reproductive hormones to stop sperm from being produced. Like the female hormonal pill, the male hormonal pill would be reversible. But also like the female hormonal pill, there appear to be side effects, among them acne, weight gain, and even trickier to work around, changes in testosterone levels that trigger a plunge in libido.

Non-hormonal techniques are also being developed, particularly a vaccine that immunizes men with antibodies to halt sperm production. This so-called male birth-control shot is encouraging, because it targets sperm directly (rather than targeting other hormones in the body) and doesn’t have the testosterone-lowering side effects of a hormonal pill. Each injection would last for long intervals (experts aren’t yet sure how long), but the pregnancy-preventing effects would be reversible, if and when a guy decides he’s ready to be a dad.

So when can you expect to see men rushing out to the pharmacy counter to pick up their new birth control Rx? “I think we may see a novel male contraceptive within 10-12 years, That may seem far off, but hey at least it’s finally within sight.

Women do not normally visit a gynaecologist, until they are suffering from any problem. Visiting a gynaecologist at regular intervals is important for good health.

Following are the 4 common reasons to visit a gynaecologist:

  1. Itchy vagina - Vaginal itching is very commonly ignored as most women consider it to be embarrassing, and as something that does not require any attention. However vaginal itching may be an indication of something more serious. Usually vaginal itching is a sign of fungal infection or a symptom of a sexually transmitted disease. In the worst-case scenario, it can even be a sign of vulvar cancer (a cancer of the vulva, which is the external part of your vagina).
  2. Painful sex - Most women feel that it is normal to feel a little bit of pain in your vagina during sex. However, the truth is that pain during sex is not at all normal. If you feel pain while indulging in sexual activity, there are chances of you suffering from either vaginal dryness or a mild infection, which if left untreated can become worse. Some of these infections can even be STDs such as herpes, gonorrhoea etc.
  3. Lump in your breast - A lump in the breast, whether or not accompanied by a bloody discharge from your nipples, is most likely a warning sign of cancer. You should also watch out for any type of pain in your breasts or even an abnormal growth as all of these indicate the development of cancerous cells in the breasts.
  4. Considerable bleeding during periods - If you bleed heavily during your periods, so much so that you have to change your sanitary napkin 2-3 times in just one to two hours, it's a cause of concern. If this heavy menstrual flow is also accompanied with shortness of breath and rapid heart rate, you may be suffering from anaemia. In extreme cases (which is very rare), this can lead to extreme blood loss, which may require a blood transfusion.

Importance of Pre-natal screening

More than 90% of pregnancies continue towards a healthy outcome. However, to have a normal pregnancy one must have a retrospective diagnosis for the same and this invariably may be associated with multiple questions from both the parents and close family members. Hence pre-natal screening in several forms allows early prediction of possible problems which may or may not be genetic/inherited. Cases are thus segregated as either high risk or low risk pregnancies and with the aid of specialized FMF- UK accredited softwares we are able to offer the  surveillance required on an individual basis. 

Maintained international standards of fetal care.

First trimester screening is a combination of fetal ultrasound and maternal blood tests. The ultrasound is done between 11- 13 +6 weeks and is performed by a FMF accredited operator. The blood test is performed on FMF approved bio-chemical assay systems and measures two hormones, beta- HCG and PAPP-A. 

The combination of the nuchal translucency and the blood test is known as the combined first trimester screening test which has a 93-95% sensitivity in predicting the risk of chromosomal abnormalities, the most common of which is Down syndrome.

A specific set of guidelines has been formulated for performing the nuchal translucency , cervical, anomaly and fetal well being scans including fetal Dopplers. A scan done as per guidelines helps calculate the risks for aneuploidies (chromosomal abnormalities) and prognosticate the pregnancy for the risk of pre-eclampsia and/or pre-term labour in order to continue a stress free pregnancy both for the mother and near and dear ones!

The need for a CVS or an Amniocentesis

The results of the first trimester combined screening test are available in a weeks time. It provides us with the risks (probability) for the fetus having any chromosomal abnormality. As per guideline, an invasive prenatal test in the form of CVS (chorionic villous sampling) 10-12 weeks or amniocentesis , beyond 16 weeks is offered as a diagnostic test if the risks are higher than 1 in 150. Though these tests carry a risk of miscarriage , 0.5-1%, they are 100% diagnostic for certain chromosomal abnormalities. It is for you to decide whether or not the risk of having a chromosomal abnormality is high enough to warrant having an invasive test.

Yes it is possible. You need to consult an endocrinologist who will bring your blood sugars to optimal levels pre-conception with regular monitoring daily blood sugar levels and also HbA1c.
