I' m suffering from frozen shoulder from last five months and my left arm is lifting till 90 degree only with continuous pain. Let me know what to do?

MCh Ortho, ATLS (AIIMS), Diploma In Orthopaedics (D. Ortho), MBBS

Treatment of frozen shoulder is done by exercise and physiotherapy. Sometimes it can take months to get relief. In severe cases manipulation of the shoulder is done under anesthesia to give quicker relief. Ice packs immediately before and after physio will help reduce the pain during physio. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr. Manoj kumar khemani

I am female 52 years old suffer from back pain since 2 years.

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist,
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Back pain at this age is usually due to degenerative changes in spine which can be changes in disc, canal, ligaments etc. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation and if required a mri of lower back may also be required but a lot of patients actually improve with well done conservative treatment regimen. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

My wife is 44 years old. She had tennis elbow pain since last 3 month. She has taken medicine like spade ap since last one month, but temporary relief is felt but if she goes to some home work this paid spurs up. Please advise.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Tennis elbow or lateral epicondylitis is inflammatory pathology around the outer epicondyle of elbow, actually it is a degenerative tear in ecrb tendon. My advice for her is: - intensive ice packs - tennis elbow brace for sometime - take a short course anti-inflammatory medication which suits you. - physical therapy like ultrasonics and laser therapy if available. Let me know in 2 weeks time whether your symptoms are improving or not. Also try to avoid all activities which increase your pain. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr Akshay Kumar Saxena

I am 25 years old. I have slip disc. Please suggest me what can I do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist,
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned slip disc which is actually mri finding. I actually need to understand what are your clinical problems in termas of back pain, leg pain and other neurological symptoms like numbness, weakness or paresthesias. If there is only localized pain in back region with no other associated symptoms like radiating pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc in case you are having back pain. If there is no improvement in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. Do not hesitate to contact me if you need any further assistance.

I have this problem for the past few years. There are thoughts that I feel has happened and ask the people involved they don't remember, this has happens many a times. I have stopped studies so my capacity of studying and remembering think have become quiet hard. Yes I can remember for some time that too just for the exam and that's the time brain also works a bit in studying else I can't even go beyond a page when it comes to studies any solution to all of this please. Research-based strategies to boost your memory and keep it strong 7 Strategies for Remembering 1.Become interested in what you're learning. We're all better remembering what interests us. Few people, for example, have a difficult time remembering the names of people they find attractive. If you're not intrinsically interested in what you're learning or trying to remember, you must find a way to become so. 2.Find a way to leverage your visual memory. You'll be surprised by how much more this will enable you to remember. For example, imagine you're at a party and are introduced to five people in quick succession. How can you quickly memorize their names? Pick out a single defining visual characteristic of each person and connect it to a visual representation of their name, preferably through an action of some kind. Remember: Memory is predominantly visual. 3.Create a mental memory tree. If you're trying to memorize a large number of facts, find a way to relate them in your mind visually with a memory tree. Construct big branches first, then leaves. Branches and leaves should carry labels that are personally meaningful to you in some way, and the organization of the facts (" leaves") should be logical. 4.Associate what you're trying to learn with what you already know. It seems the more mental connections we have to a piece of information, the more successful we'll be in remembering it. This is why using mnemonics. Write out the items to be memorized over and over and over. 5.When reading for retention, summarize each paragraph in the margin. This requires you to think about what you're reading, recycle it, and teach it to yourself again. Even take the concepts you're learning and reason forward with them; apply them to imagined novel situations, which creates more neural connections to reinforce the memory. 6.Do most of your studying in the afternoon. Though you may identify yourself as a" morning person" or" evening person" at least one Study. Suggests your ability to memorize isn't influenced as much by what time of day you perceive yourself to be most alert but by the time of day you actually study—afternoon appearing to be the best. 7.Get adequate sleep to consolidate and retain memories. Not just at night after you've studied but the day, you study as well. Far better to do this than to stay up cramming all night for an exam. 10 Study Tips to Improve Your Learning Study Tip 1: Underlining Underlining is one of the simplest and best known study tips. It’s easy to highlight the most significant parts of what you’re reading. One key sentence per paragraph and a few important phrases here and there. You can only retain a certain amount so it’s best to retain the most important information. Study Tip 2: Make your own note and taking notes is one of the most widespread study skills out there. Essentially the aim of note-taking is to summarise lectures or articles in your own words so you can easily remember the ideas. Study Tip 3: Mind mapping A good Mind map can save you many hours of study and further consolidate your knowledge for your exams. Mind Maps are an extremely versatile tools. They can be used for brainstorming, outlining essays or study topics and for general exam preparation, ExamTime offers the ability to create Mind Maps quickly and easily which makes them the ideal tool when it comes to exams. Study Tip 4: Flash cards:- Using these are a particularly effective method of learning when trying to assimilate different facts, dates, formulas or vocabulary. Subjects such as History, Physics, Maths, Chemistry, Geography or any language are made much easier if you incorporate Flashcards in to your study. Study Tip 5: Case Studies Sometimes it can be difficult to grasp the implications of some theories. This is where studying case studies can be a big help. Case studies can help you visualise a theory and place it in a more familiar and realistic context. This is especially useful in business or law subjects. Study Tip 6: Quizzes are an excellent way to review study notes in the weeks and days before an exam. Quizzes can show where your strengths and weaknesses are, so it allows you to focus your efforts more precisely. Moreover, if you share your Study Quiz with your classmates and test each other as much as possible you can discover even more details and areas you may have overlooked. So before any exam, make sure you create and share a bunch of different Quizzes with your Friends. Study Tip 7: Brainstorming This is another study technique that is ideal for studying with friends and/or classmates. Brainstorming is a great way to expand every possible idea out of any topic. Just get a bunch of friends together and shoot the breeze, there are no wrong answers when brainstorming – just talk and capture the ideas, you can review afterward. Study Tip 8: Mnemonic Rules Mnemonics are especially useful when memorising lists and sets. Mnemonics rules basically work by associating certain concepts with other concepts that are more familiar to us. There are many different ways to make mnemonics and these can be individual to the person. Study Tip 9: ORGANISE your study One of the most effective study skills is also one of the most often overlooked; this is organising your study. Creating a TIME TABLE gives you goals and a time in which to achieve them. Having a study timetable as you study is greatly motivational Study Tip 10: Drawing Many people find it easier to recall images rather than text that is why they are better able to memorise concepts if they associate them with pictures or drawings. If you find answer helpful please click on “helpful” tab for knowing my efforts are useful CONCENTRATION TIPS Here’s some tips to get into a state of deep concentration where work / or studies flow easily so that you can do well in examinations/ and or do your work well. 1) Cut Off the Noise Getting into a state of concentration can take at least fifteen minutes. If you are getting distracted every five, you can’t possibly focus entirely on your work. Request that people don’t interrupt you when working on a big project. If you are required to answer phones and drop-in’s immediately, schedule work when the office is less busy. 2) Structure Your Environment The place you work can have an impact on your ability to focus. Try to locate yourself so you are facing potential distractions such as doors, phones or windows. This way you can take a glance to assess sounds that would otherwise break your focus. 3) Clarify Objectives Know what your goal is clearly before you start. If you aren’t sure what the end result is, the confusion will make it impossible to focus. 4) Divide Blobs. If you have a large project that needs work, clearly identify a path that you will use to get started working on it. If the sequence of actions isn’t obvious, it will be difficult to concentrate. Taking a few minutes to plan not only your end result, but the order you will complete any steps, can save hours in wasted thinking. 5) Know the Rules Get clear on what the guidelines are for the task ahead. What level of quality do you need? What standards do you need to follow? What constraints are there? If the rules aren’t clear from the outset, you will slip out of concentration as you ponder them later. 6) Set a Deadline Deadlines have both advantages and disadvantages when trying to force concentration. A deadline can make it easier to forget the non-essential and speed up your working time. If you give yourself only an hour to design a logo, you will keep it simple and avoid fiddling with extravagant designs. Time limits have disadvantages when they cause you to worry about the time you have left instead of the task itself. 7) Break down Roadblocks Roadblocks occur whenever you hit a tricky problem in your work. This can happen when you run out of ideas or your focus wavers. Break down roadblocks by brainstorming or planning on a piece of paper. Writing out your thought processes can keep you focused even if you might become frustrated. 8) Isolate Yourself Become a hermit and stay away from other people if you want to get work done. Unless your work is based on other people they will only break your focus. Create a private space and refuse to talk to anyone until your work is finished. Put a sign on your door to steer away drop-ins and don’t answer your phone. 9) Healthy Body, Sharper Mind What you put into your body affects the way you concentrate. Nobody would expect peak performance if they showed up drunk to work. But if you allow yourself to get chronic sleep deprivation, overuse stimulants like caffeine or eat dense, fatty foods your concentration will suffer. Try to cut out one of your unhealthy habits for just thirty days to see if there is a difference in your energy levels. I’ve found even small steps can create dramatic changes in my ability to focus. 10) Be Patient Before I write an article, I often sit at my desk for a fifteen or twenty minutes before I put finger on the keyboard. During this time I feel a strong urge to leave or do something else. But I know that if I am patient, I’ll stumble upon an idea to write about and enter a state of flow. Without a little patience, you can’t take advantage of flow when it rushed through you. If you need strong concentration I recommend periods of 90-120 minutes. Any less than that and you will waste too much time getting started before the flow can continue. More than this is possible to sustain focus, but you will probably benefit from a quick break. These are the general guidelines for concentration, studying well or doing your job. Regards.

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days. For further guidance and treatment.
