Dear doctor, Please help and advice. Dear Mr. lybrate-user, Mammogram is just one investigation for breast cancer. If it says malignant, then we need to do a fnac /biopsy of the lump. We also need to do a xray mammogram, a xray chest and a sonography of the abdomen and bone scan to stage the disease. If the disease is localized only to the breast and/or the axilla (armpit), it can be completely removed.

You should do a mammography of bilateral breast. If the lump is highly suspicious, an fnac from the lump should be performed.

How many cigs have to be smoked to get cancer or how long you have to be smoking to get cancer?

Dear sir there no mathematical Co relation, but smokers are twenty times more at risk than non smokers and even passive smokers, those who live in company of smokers are at risk of getting cancer which may be about five times higher than completely nonsmokers.

I wad detected with Prostate Cancer in Nov '15 (PSA 41 - Gleason 9 - T3N0M0) and underwent 28 sittings radiation followed by quarterly Hormone Therapy (inj Eligard 22.5).Post radiation PSA checked in Mar '16 was down to 1.60 and when checked in June '16 down to 1.20.Hormone Therapy to lat 2 years- Your expert views about treatment and condition requested. Hi Mr. lybrate-user, I appreciate you seeking opinion. As far as I understand, this injection attempts to slow down the growth of prostate cancer by lowering the levels of male hormone testosterone. Its good to note that your PSA levels have consistently decreased from as high as 41 ng/ mL in Nov 15' to as low as 1.2 ng/ mL in June 16' However, sometimes one may still have prostate cancer despite a normal PSA value. A PET CT scan, at this juncture, can help provide correct status as regards the progression of the disease. Apart from consulting your primary oncologist with the diagnostics, I would advise that you connect with an expert oncologist specializing in genito-urinary cancers for a second opinion. Benign Prostrate Hyperplasia And Its Symptoms!

Benign Prostrate Hyperplasia (BPH) is a condition which results in the enlargement of the prostate gland. The prostrate grand is situated near the Urethra (a tube which helps flush out the urine from the bladder). Generally, the chance of developing BPH increases after one crosses the age of 50. But it is yet unknown as to why some men experience more severe symptoms than others.

Causes of BPH
This condition generally occurs because of old age and affects almost all men above the age of 75. It occurs because of the various hormonal changes and changes in cell growth that the body goes through, as one becomes old. Sometimes BPH can set in due to genetics. If BPH sets in due to genetically reasons, it usually is quite severe and affects men before they reach 60.

Symptoms of BPH
Quite a few men who develop BPH, experience no symptoms at all. But when symptoms of BPH, known as lower urinary tract symptoms (LUT) start, they can be either mild or very severe. The severity of the symptoms is not related to the extent of the enlargement. Many a times men with only a mild enlargement complain about severe symptoms, while men with a highly enlarged prostate gland have not complained about any discomfort faced. 

Symptoms of BPH tend to worsen due to cold weather and also because of physical and emotional overexertion. There are certain medicines, which should be avoided if you suffer from BPH, as they have a tendency to worsen your symptoms, for example diphenhydramine, pseudoephedrine, oxymetazoline spray and other antidepressants.

The symptoms of BPH are related to bladder emptying and issues with bladder storage.

Symptoms related to the urine drainage from the bladder are:

  1. Strain while urinating
  2. Weak urine flow
  3. Some dribbling after urination
  4. Sudden urge to urinate
  5. Pain while urinating

Symptoms related to storage of urine in the bladder are:

  1. Waking during the night to urinate
  2. Urinating frequently during the day and at night
  3. Sudden urge to urinate, which may be hard to control

It however, has to be kept in mind that these symptoms may not primarily occur due to prostate enlargement, but are the result of other conditions like urinary tract infections, prostatitis, prostate cancer, neurological disorders and even diabetes. Thus, it is essential that you visit a doctor and get the cause for these symptoms diagnosed properly.

Hello Concern Dr. I would like to advice from you , everyday I drink three or more times coffee with sugar mix is it Ok for health ? I have Sugar under control I used Zoform-1000 MG tablet everyday one time after dinner only. Previous two years before I had faced Cancer hematologist surgery done, but I am alright now.

Hi lybrate-user, I appreciate you seeking help. In the given context as you describe, I would advise that you minimize or avoid tea/ coffee etc all. Try cut down on sugar, salt, spicy, oily, greasy and other high calorie food stuff including fast foods. Adopt a mediterranean diet pattern instead. Stay well hydrated. You need to stop smoking immediately and consult a chest physician, it can be due to lung inflammation or cardiac causes (angina). Get ecg, cxr, echo and blood tests to find the reason. If you can not stop smoking on your own, consult a psychiatrist for anti-craving drugs.

My father has a tumor in their food pipe. Which is about 25 to 35 cm. Can you please suggest me what can we do.
He needs the following tests 1) barium swallow (not advised by many doctors nowadays but has its own utility) 2) endoscopy for biopsy 3) ct scan of the chest. This will help staging the disease. If it is early stage, then upfront surgery can be done. If its locally advanced then first chemoradiation is given followed by surgery. Please show it to a surgical oncologist near you or any of us surgical oncologists on the forum.

  1. According to Ayurveda, having fresh foods can help you to prevent cancer. Consuming more of green leafy vegetables and fresh fruits may prove to be helpful.

Not necessarily all breast cancer are genetic. Most of them are due to life style related or due to hormonal imbalance. Still you should do breast self examination on monthly basis five days after periods are completed and after forty or may be earlier you can go for annual mammography.

I am seeing my internal cheeks membrane seems to harden and I can feel a lump in my cheeks my skin has darkened I consume alcohol on a daily basis can these symptoms be a cancer symptom if yes how much time do I have and if at all this can be cured with chemo without sideffects on face.

Is multiple myeloma curable my father is 71 years old and has been detected of multiple myeloma he is been under going chemotherapy since april his 5th cycle is going on how many more chemo he has to take his reports as showing good response.

Multiple myeloma is a incurable but treatable disease. Means just like diabetes it can not be cured but we can keep it in control with chemotherapy. Number of chemotherapy cycles depends on type of chemotherapy regimen used and whether bone marrow transplant planned. Prognosis depends on various pathological factors and response to chemotherapy.

I am suffering from cancer on abdominal muscle, which was removed by surgery and radiation given on September 2012. Spindle cell sarcoma. Till now, I am under medical review every 6 months, I am afraid whether it will come again. What shall I do to stop re occurrence again.

Well Mr. Lybrate-user. I appreciate you seeking help. As I understand, the prognosis stands better usually in surgeries with clean margins. A clean surgery can, at times, be difficult especially for sarcomas in the abdominal muscle considering proximity of the other abdominal organs, and surgeries in such cases are most likely to be followed by chemotherapy/ radiation as deems appropriate by the oncologist (s). Apart from the above-mentioned, stage and grade of the cancer could also impact the chances of recurrence. Nevertheless, having already gone through the treatment procedure successfully and having already completed over three and a half years of disease free survival, I would only encourage you to stay positive, hopeful and brave as ever. Do see your doc/ oncologists for medical review regularly as advised. Besides the guidelines from your doc (s) which you need to follow strictly, I would advise you to stop tobacco smoking & restrict alcohol consumption, if any. Try maintaining a good health by being physically active, consume healthy/ nutritious diet (preferably mediterranean) with emphasis on plenty of leafy greens and fresh fruits, stay well hydrated, adopt a healthy lifestyle and de-stress regularly. A relaxed and stress free mind is also known to have positive impacts on health. In this connection, I would advise you to practise relaxation techniques like yogasanas (like'savasana'), meditation, listening to soothing music, social interactions, attending or viewing spiritual programmes etc all to stay relaxed and happy. The above-mentioned dietary and lifestyle recommendations may help lower the risks of recurrence or other cancers if not stop it completely. Do take care and I wish you all good health. Sincerely.

Dear mam, breast cancer is lifestyle related disease sedentary lifestyle, fatty food, lack of exercise, apart from this it could be related to hormonal imbalance due to some types of hormonal supplements, late menarchy or menopause,late marriage and late or no conception, not breast feeding and lastly it could be genetic. Abnormal sleeping habits may lead to personality change and psychological disorders but not really directly related to breast cancer.
