Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to have a persistent back pain as a health issue. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. I met with an accident, my hand got swelled what should I do?

This is inadequate information rule out diabetes & vit. D deficiency or any other metabolic disorder. Physiotherapy usually helps any way take paracetamol 250mg od & sos x 5days caldikind plus (mankind) 1tab odx10days you will need other supportive medicines also. It may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. Do ask for detailed treatment plan. Kindly do not ignore it. It could be beginning of a serious problem.

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

Sir, I have muscles pain in mt legs and 99 fever from approx 2 weeks and when done some tests typhoid comes normal but esr is 56 mm per hour. So what does it mean does this rise in esr and fever is due to pain or something else there are no symptoms except these. Age 19 years male. I am feeling like muscles are torn from inside. You should take Acupressure therapy and take Biochemic Mag phos 200x +Ferrum phos 200x, 4 tab each thrice a day with warm water and take it 5 days than consult private online.

He should take vitamin D , Calcarea Phos 12 x four tablets daily and Calcarea Flour 12 x daily four tablets in the evening for one week . He should also sleep in right posture. Let me know some more details , like constipation, gastric problem, etc. consult on line privately.

I fell from stairs yesterday and have a mild swelling and little bit pain when I move the ankle. I did hot water bag last night and took nucoxia medicine which made me feel it's all well and did exercise in mrng. There is still mild pain, I want to continue gym from tomorrow or asap. Please recommend how to ensure its all fine and start gym asap again.

Hi. Whenever you have a fall or injury resulting in pain and swelling, first 2 days do cold pack, no hot water bag. When the swelling reduces than you can go for hot bag. Secondly you want to do Gym. If the pain is negligible and there is no stress on ankle than you can start with cycle and crosstrainer for cardio and upper body weight training and seated leg exercises like Leg extension and leg curl. Avoid squats with weight, calf raise etc for 1-2 week. After that you can start with one set of exercise restricted before and gradually move to 2 sets and increment your weight (resistance ) back to your regular intensity.

Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

Hi if you have localized back pain with - no neurological symptoms like numbness, paresthesia, weakness in limbs etc, my advice is: - intensive physical therapy initially under supervision of a physiotherapist and then at home. - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know in a week time how you are feeling. I will actually ask you to get x ray of involved region of spine (ap & lateral) and send reports across to me so that I can advise further in a better manner. Do not hesitate to contact me if you need any further assistance.

do situps regularly. move your leg up and down with some weight at ankle by sitting on a high chair 50 times daily. surely you may get relief

Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times.

When I climb stairs. Left leg makes noise in ankle on every step. Is this some ortho problem. Otherwise when I walk it does not show any sound.

This could be due to oa knee &/or obesity. Physiotherapy along with medicines will help. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with a soft bedding on it. Any way take bio d3 max 1tab odx10days ibuprofen 200mg od & sos x 5days do knee exercises. It may have to be further investigated make sure you are not allergic to any of the medicines you are going to take.

Do contrast bath therapy Take hot water in one bowl & cold water in another bowl. First dip your hand in hot water for 3min then in cold water for 1min, keep repeating for 20min, twice daily. Finger splints are available, stationary splint will give your finger complete rest whether with dynamic one you can do work as well

hope it's been diagnosed after MRI of lower spine. if not please get an MRI of LS spine done to rule out other causes of pain in that area. Also stretch exercise and physiotherapy would help you.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Actually when I counsel my patients for a knee replacement surgery, I always see certain things and ask the patients few questions? - during examination and subsequent investigations like x rays, the alignment of the joint needs to be seen, so we have to see whether there is any fixed deformity or a coronal plane alignment, if it is very bad with compromised joint space, then definitely nothing apart from a surgery will work because no medicine can change alignment of a joint - also I need to know how much your mother can walk everyday and what are her analgesic requirements? is she able to do her day to day activities without too much pain, or is that a bother? kindly send me her latest x rays so that I can see and advise accordingly. Try few things for your back pain: - Apply Ice and Heat Heating pads and cold packs can comfort tender trunks. Most doctors recommend using ice for the first 48 hours after an injury -- particularly if there is swelling — and then switching to heat. You can use whichever you find comforting as long as your skin is protected. - Limit Bed Rest Studies show that people with short-term low-back pain who rest feel more pain and have a harder time with daily tasks than those who stay active. -Keep Exercising Activity is often the best medicine for back pain. Simple exercises like walking can be very helpful. But remember to move in moderation, Stay away from strenuous activities like gardening and avoid whatever motion caused the pain in the first place. -Maintain Good Posture most people have poor posture when going about their daily activities, putting unnecessary strain on their backs. -Strengthen Your Core Most people with chronic back pain would benefit from stronger abdominal muscles. If the abdominals are weak, other areas must pick up the slack. When we strengthen the abdominals, it often reduces the strain on the lower back. - Improve Flexibility Too much tension and tightness can cause back pain. By increasing flexibility you need to put an equal load throughout the body from the feet all the way up to the head. -Sleep the Right Way The amount of rest you get is important, and so is the position you get it in. Sleeping in a bad position or on a mattress without support can cause back pain. Back sleepers should put pillows under their knees. Side sleepers should place pillows between their knees to keep their spine in a neutral position. Stomach sleeping causes the neck and head to twist and can put undue stress on the back. -Use Relaxation Techniques Research shows that practices such as meditation, deep breathing and yoga, which help put the mind at rest, can do wonders for the back. -See a Specialist Developing an individualized exercise plan is essential to managing chronic back pain. Some patients need more core strengthening while others benefit mainly from stretching and improving flexibility. Please consult a Orthopedic Physiotherapist for the right exercise plan. Best wishes.

I am 55 years male and I had pain in my lower spine. X ray revealed that I have spondy in l4/5. What medication would you advise. Also my spine has developed parrots beak. Will this be harmful or are there any chances of disability ?

Some degenerative changes like beaks are normal finding in spine with age. They can be related to your back pain with other physical findings and pain pattern and level. Go for physiotherapy consultation.

Plese do an arterial dopler of both the legs. If doppler shows some obstruction. You need to visit a vascular surgeon.

For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.
