I have a constant pain at my back and neck. And when I used to write something for at lest 1 hour my shoulders starts paining too. I consulted an spine specialist too, he observed and said lack of vitamin D and gave me 3 medicines namely Brigeta 1gm Sachet (once in a week, fm12 sachets, CCM TABLETS (Calcium Citrate Malate, Vitamin D3 and Folic acid tablets) and Rxplus (long list of composition). Told me to get a next check up after 3 months. Its 5 months now, as long as I was taking the medicine I was feeling fine but again all started same. What should I do?

Warm mustard oil or sesame oil (1 teaspoon) with 3-4 cloves of crushed garlic and 1 tsp of ajwain (carom) seeds until it roasts to a reddish brown colour. Ensure that it does not get burned. Massage this oil on your affected areas. After that do hot fomentation with salt potli (bag). --mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back & neck pains. If you have localized neck and lower back pain with no other symptoms like radiating pain in limbs, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back & neck region while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and neck region and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. Thanks & regards

The lower end of the vertebral column is inflamed and that bone is called coccyx bone and the condition is called Coccydynia. The conservative management would be to keep ice in that inflamed area during one time in a day and also to sit in a hot water bowl where the penetration of the heat would help to reduce the inflammation. And to reduce the weight falling on the inflammed surface we suggest patients to use air cushion pillow (also called as doughnut pillow) where in the buttock's weight will not fall exactly on the painful tail bone and that would automatically reduce the pain. For this the best treatment would be to undergo Ultrasonic therapy in one of the nearby physiotherapy clinics.

Dear your mri is giving you an early warning that this is the time to act. My suggestion is based on following points 1. Straightening of lumbar spine: please see nearby physio clinic and get you musculation examined for tightness, physio will give good treatment to make your lumbar posture correct. 2. Retrolisthesis l5 s1: need to strengthen your back not in gym way but as per physio guidance. As there is no neurological deficit so need not to worry but yes time to act yes you should do your activities but without straining yourself because its easy to say stop activities but by this way we are getting relief from symptom and paying in term of muscular weakness. Please do visit nearby physio clinic for better solution.

Take rest for few days. You may sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. Effects and Diagnosis of Herniated Disc

MS - Orthopaedics, MBBS, fellowship in sports injury and arthroscopy surgery and hip and knee replacement
Orthopedist, Ahmedabad
Effects and Diagnosis of Herniated Disc

A Herniated disc also known as a slip disc or prolapsed disc and it refers to the problem where one of the discs located in between the bones of the vertebrae(backbone) gets damaged and stacks itself upon the nerves. The tender inner portion of the disc gets protruded over the outer ring. Herniated disc causes severe neck pain and back pain. It occurs when the outer ring gets weak and tears down. The most common symptoms include pain and numbness, especially on one side of the body. The pain extends down to the arms and legs. It worsens during nighttime. The muscles become weak. Overweight people are likely to have this disease, as the discs carry extra weight. Old people are vulnerable to a slip disc.

Effects of Herniated Disc:

  1. An untreated case of Herniated disc may cause permanent damage to the nerves.
  2. In certain cases, a Herniated disc is capable of cutting off nerve impulses to the "caudal equine"nerves, present in the lower back and legs. This leads to loss of bladder control or bowel control.
  3. Saddle anaesthesia is another long-term effect. Nerves are compressed by the slip disc, causing loss of sensation in the inner portions of your thighs, the back of your legs and surrounding the rectum region.
  4. Symptoms may improve or worsen.

Diagnosis of Herniated Disc:
Treatment or diagnosis of Herniated disc or slipped disc can be either conservative or surgical in nature. The nature of treatment is determined according to your discomfort level and the kind of pain you are experiencing, or how much the disc has slipped out.

  1. The pain caused by Herniated disc can be relieved by undertaking an exercise schedule, which includes stretching and strengthening the spine and its surrounding muscles.
  2. A physiotherapist should be appointed for recommending the types of exercises.
  3. Taking pain relievers also soothes the pain.
  4. Stronger medicines prescribed by doctors include-muscle relaxants, narcotics to deal with the pain, medicines for nerve pain such as "gabapentin"  or "duloxetine".
  5. In case of the symptoms not subsiding within a span of six weeks, a surgery may be recommended by your doctor. The surgeon may have to remove the damaged part of the disc. This surgery is called a microdiscectomy.
