Mr. lybrate-user, Thanks for the query. Hypothyroidism is a hormone deficiency disease. In this condition your thyroid gland is not producing enough (nil) thyroxine. This hormone is necessary for day to day functioning of all the metabolic activities in the body. Therefore, it has to be supplemented every day for the rest of life. Hence there is NO CURE. When on treatment with adequate dose of thyroxine, TSH is maintained between 3 to 4 mU/L, at that level all the symptoms will be in check. So if the TSH level is 60 mU/L, means there is uncontrolled hypothyroidism. Thanks.

Hi lybrate-user. You will need to undergo blood tests and xrays to find the cause and severity of the knock knees. If you are facing problems with daily activities or are bothered with the appearance of your legs, you may be advised corrective surgery.

From past two days I am having severe neck pain with light headache and vomiting. Is cervical spondylosis causing this and what are the methods to avoid it.

Yes if upper cervical vertebrae are involved then you may have symptoms like headache, dizziness and neck pain. In physio we have a treatment for cervical spondylitis so I would recommed you to consult any physio and start taking physio modalities and manipulations to reduce pain and increase mobility. I have specially designed one program as I-care specially in cases of neck and back pain which I have pratically applied on many of my patients and received a very good result. The treatment include: 1. Ice massage - Take ice pain perform massage over the area which is painful and have swelling. Do “to” and “fro” motion. Apply it for 8-10 minutes and then relax your body for another 3-5 minutes. 2. Deep massage - Using your finger tips start kneading your affected area. 3 finger kneading or 2 finger kneading or you can use thumb also for kneading. Once done with kneading again relax your body for 3-5 minutes. 3. Now apply any pain relieving ointment like volini or oraflame etc. Do not massage with the ointment. Just apply it gently involving all the area. Do not penetrate it. Once done with application again relax your body for 3-5 minutes and start performing exercises. Exercises: Because of pain and stiffness your neck range starts decreasing so with the entire treatment program we can help you to make your neck mobile and reduce pain to live happily. 1. Position - Sitting Bend your head forward until you feel stretch behind your neck. Hold for 5-10 secs. And relax. Repeat 5 -10 times. Perform same by taking neck backward and then side ways. Perform the exercises slowly until a stretch is felt. Do not over stretch, perform slowly and gently. 2. Position - Sitting Isometrics: Infront of mirror. Place your both hands on your forehead. Apply contradict pressure from hands towards your head and with your head towards your hands. There should not be any neck moment. Instead an isometric contractions should be there in neck. Hold for 5 sec and relax. Repeat it for 5-10 times. Perform same by placing your hands on the back of your head and giving isometric contractions to make posterior muscles stregthening. Then same as with placing one hand on right side and giving pressure from hand to head and vice versa. 3. Position - Sitting Stretching - Bend your head forward and and then gently pull your head further by applying pressure from your hands. A stretch is felt on back of your neck. Same a stretch your side neck muscles. 4. While exhaling push your upper rib down with your one hand and then tilt your head and give a slight stretch by rotating your head to opposite side. Hold for 5 sec and then relax. Repeat for 5-10 secs. 5. Neck and back straight, keep your chin in. Hold for 5 sec and feel a stretch in your neck. Relax. Repeat for 5-10 times. 6. Lie on bed with towel roll under your neck. Press the roll into the floor by using your neck muscles. Hold for 5 sec and relax. Perform 5-10 times. Advice - 1. Use very thin pillow while sleeping. 2. Do not sit for long time with your neck forward bending. 3. Lying on bed and reading book or newspaper should be avoided. 4. If there will be any pain while performing exercises or headache stop doing exercise and consult physio right away. 5. Sit in erect posture without bending your back or neck. 6. Position your desktop so that it should be just infront of your neck you do not have to bend your hands or neck. Please contact me if you face any problem while performing the I-care at home. Yes it can be due to lack of calcium and vitamin d and b12, for which you will require prescription medications and also there are many home remedies for calcium,

Arthroscopy - Things To Remember During and Post Procedure!

Literally translated into two words – arthro (joint) and scopy (to look), arthroscopy refers to a process used to look into a joint. The doctor uses an instrument known as arthroscope which is used to examine a joint in greater detail. What was initially used mainly in diagnosis has gradually grown to include use of instruments through the arthrocope, thereby allowing for some surgical treatments to be done as well.

Indications/uses:

  1. Diagnosis of joint problems like cartilage tears
  2. Repairing joint problems
  3. Remove foreign bodies in a joint
  4. Track response to treatments
  5. Identify disease progression
  6. Used during an open surgery to:
    • Remove fragments or spurs of tissues
    • Shave off sharp edges
    • Collect fluid or tissue samples
    • Remove swollen tissues
    • Take biopsy
    • Extract swollen joint fragments
    • Remove deposits of calcium or other minerals

Procedure:

  1. Usually done as an outpatient procedure
  2. The procedure in terms of what to expect will be explained to the patient
  3. A sedative is given and the area to be studied is usually shaven and topical antiseptic is applied
  4. Vital signs are monitored
  5. Regional or local anethesia is generally used, where only the joint to be studied is injecte with a local anesthetic.
  6. In cases where the patient is undergoing an open surgery, general anesthesia would be given.
  7. The patient’s position would depend on the joint being studied, usually, is made to lie on the back.
  8. A tourniquet is used to restrict blood flow to the particular joint
  9. A small incision is made in the joint space and the instrument is introduced into the joint space. There is a constant slow flow of saline or another irrigant (to which chlorhexidine might be added) to improve visibility and to remove debris and control infection
  10. Other joint tools are used as required during the procedure either to cut off small fragments of tissue or to remove debris or foreign body.
  11. Once all procedures are done, the joint is thoroughly flushed to clear off any debris.
  12. It is closed with stitches after final inspection
  13. During recovery, crutches or splints may be used depending on the patient’s overall condition
  14. Duration of the procedure depends on what actually was done. An inspection could last about 20 minutes while a procedure could take a couple of hours.

After the procedure:

  1. Complications are very rare, maybe painful operated joint for a couple of days
  2. Rest the joint well for a couple of days
  3. Ice, pain killers, and compression bandage may be required
  4. Return to normal activity and exercises to be followed as dictated by the orthopedist.

Shoulder pain can be a result of injury or disease of the shoulder joint. Injury can affect any of the ligaments, bursae, or tendons surrounding the shoulder joint. Injury can also affect the ligaments, cartilage, menisci and bones of the joint. Some minor shoulder pain can be treated at home. Icing the shoulder for 15 to 20 minutes, three or four times a day for two to three days can help reduce pain. Use an ice bag or wrap ice in a towel (putting ice directly on your skin can cause frostbite). Rest the shoulder and avoid any movements that might cause pain. Simple shoulder exercises can help to stretch and strengthen muscles and rotator cuff tendons. A physiotherapist can show you how to do them properly. Most leg pain results from wear and tear, overuse, or injuries in joints or bones or in muscles, ligaments, tendons or other soft tissues. Leg pain can also be caused by blood clots, varicose veins or poor circulation. To relieve mild pain: stay off your leg as much as possible apply an ice pack or bag of frozen peas to the affected area for 15 to 20 minutes three times a day elevate your leg whenever you sit or lie down please go to a physiotherapist if: you have pain during or after walking you have swelling in both legs your pain gets worse your symptoms don't improve after a few days of home treatment you have painful varicose veins if you feel the pain is from muscle cramp then you need to drink a lot of water because poor fluid intake (dehydration) may predispose to cramps also low blood levels of either calcium or magnesium directly cause cramps. I have neck pain how many days I have to wear the soft cervical collar. Any rules to follow.

It needs to be investigated. Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomentation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, inform. Collar is a external support for internal weakness. When you make your neck muscles strong by doing exercises, then the collar will be rejected automatically.

Knee Advice... Avoid sitting Cross legged. Avoid Squatting. Quadriceps Exercises. Core Strenthening Exercises. Hams Stretching. Sports Taping Hot Fomentation.

It may be rotator cuff problem. Since problem is from 1 year, it's best to visit shoulder specialist. If possible, get mri of shoulder with 3 tesla machine and send report.

Hello. Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally. Management: -Stretching or activities that place additional strain on the back are discouraged. -Sleeping with a pillow between the knees while lying on one side may increase comfort. -Lying on your back with a pillow under your knees would also relieves pain and no pillow under your head. -Some people seem to benefit from the use of ice or heat. Take care, do not use a heating pad on ‘high’ or place ice directly on the skin. Medication: Take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day.

Your weight 61 kg is normal. If your backache is not severe then you should do back bending extension exercises regularly. Use a comparatively tough mattress instead of thick spongy or springy ones. Do not bend forwards and avoid lifting heavy objects. If you follow this routinely then there should be regression of your pain and it will prevent any possible pressure on your spinal roots in future. An xray of lumbosacral part of vertebral column will help in finding any space reduction between the vertebrae and possibility of root friction with the discs due to prolapse.
