Gall bladder gets diseased due to an unhealthy diet and lifestyle. This leads to an imbalance of the acid-alkali balance in the body which causes the cholesterol to precipitate in the gall bladder leading to stone formation. Over time, these may get infected and lead to an acute inflammation, pain, fever etc. A stone may move ahead into a narrow passage or the duct and get stuck leading to severe agony. One can avoid formation of gall stones by just avoiding fatty food, junk foods and going behind unhealthy diet. Gastric Cancer - In a nutshell!

Gastric (stomach) cancer occurs when malignant cells form in the lining of the stomach. By far, it is known to be the second most common cause of cancer-related deaths not only in Asia but also worldwide. Though it can affect both male and female populace, it is seen more commonly in men and in people aged 50 years or older.

  1. Type: Gastric cancers can present as one of the following types - 

    1. Adenocarcinoma: Begins in the glandular cells lining the inside of the stomach. This forms a majority of the stomach cancers.
    2. Lymphoma: Begins in immune system cells present in the walls of the stomach. Occurrence of lymphoma, in the stomach, is rare.
    3. Carcinoid Tumor: Begins in hormone producing cells of the stomach.  Occurrence of carcinoid cancer, in the stomach, is rare.
    4. Gastrointestinal Stromal Tumor (GIST): begins in nervous system cells of the stomach. Occurrence of GIST, in the stomach, is rare.
  2. Gender: It affects both male and female populace.

  3. Etiology: The factors that are associated with increased risk of gastric cancer include the following mostly –

    • ‘Helicobacter Pylori’ bacterial infection in the stomach is a common cause of gastric cancer of both the intestinal (expanding) & diffuse (infiltrative) type. Furthermore, studies indicate that high salt intake is synergistic with H. Pylori infection in the manner that it is likely to increase the risk of gastric cancer that is induced by H. Pylori bacteria.
    • Smoking, consuming alcohol, red meat, salty/ smoked/ processed foods, low intake of fruits and vegetables, diets rich in nitroso compounds, eating foods contaminated with aflatoxin fungus etc all.
    • Atrophic gastritis characterized by chronic stomach inflammation is known to increase the risk multi-fold. Chronic gastric inflammation can lead to atrophy of the gastric mucosa, metaplasia, dysplasia and finally carcinoma.
    • History of pernicious anaemia, gastric ulcers, adenomatous gastric polyp etc all.
    • Family history of gastric cancer. Several familial syndromes that have been associated with a pre-disposition to gastric cancer include familial adenomatous polyposis, Lynch syndrome, Peutz-Jeghers syndrome and e-cadherin mutation (diffuse type)
    • Blood group A, Obesity etc all are known to be associated with diffuse or cardia gastric cancer.
    • Low socioeconomic status - persistent lifestyle issues/ irregularities including high stress coupled with an improper diet/ dietary pattern.
    • Epidemiological evidence is indicative of a risk or pre-disposition to gastric/ stomach cancer for people suffering from diabetes mellitus (DM).
    • Very high dose ionizing radiation exposure is an uncommon risk for gastric cancer.
  4. Features: There are often no early stage symptoms. Early stage symptoms, if any, are non-specific and are likely to be ignored, thus delaying the diagnosis most often. Hence, gastric/ stomach cancer is often detected at an advanced stage where the disease is either locally advanced or metastatic. The various presentations (of signs & symptoms), by stage (early or advanced), of gastric cancer are as enumerated below:

    1. Early Stage – can present with one or more of the following non-specific symptoms/ signs -

      • Dyspepsia (Indigestion),
      • Stomach/ Epigastric discomfort,
      • Bloated feeling after eating,
      • Mild Nausea/ Vomiting,
      • Blood in Vomit (Haematemesis),
      • Heartburn,
      • Weight loss (Cachexia)
      • Anaemia,
      • Occult blood in stool/ Melaena,
    2. Advanced Stage – presents with one or more of the following symptoms/ signs -
      • GI Bleeding with black tarry stools (Melaena),
      • Persistent Nausea/ Vomiting,
      • Blood in Vomit (Haematemesis),
      • Early Satiety,
      • Anaemia,
      • Loss of Appetite (Anorexia),
      • Weight loss (Cachexia),
      • Persistent pain in the abdomen,
      • Fluid build-up in the peritoneal cavity (Ascites),
      • Edema of the lower extremities,
      • Liver Enlargement (Hepatomegaly)/ Jaundice,
      • Difficulty swallowing food (Dysphagia)
  5. Screening: Is generally recommended for asymptomatic populations in high incidence areas or as surveillance for high risk individuals. The goal of screening, as usual, is to be able to detect & diagnose gastric cancer at an early stage which is potentially curable. It is mostly endoscopic/ radiologic.
  6. Diagnosis: Following are the diagnostics employed in gastic cancer -​

    • Physical Examination: May be remarkable for palpable abdominal mass, weight loss (cachexia), abdominal distension, ascites, hepatomegaly, lower extremities edema and lymphadenopathy for gastric cancers in the advanced stage. For early gastric cancers, however, physical examination is largely uninformative.
    • Blood: Hb- may be low, ESR – raised, tumor markers CEA & CA-19-9 could be raised sometimes in adenocarcinoma but are not frequently elevated. Abnormal blood test results may be indicative of malignancy, but a follow-up gastroscopy/ biopsy is always the gold standard for accurate diagnosis.
    • Stool: Occult blood may be +ve
    • Barium Meal X-Ray: Could show a filling defect at the site of the carcinoma/ cancer growth.
    • Gastroscopy/ Biopsy: Clinches the diagnosis.
    • Endoscopic Ultrasound: Maximizes tumor staging as it helps determine the depth of invasion of the tumor.
    • CT Scan: Of chest, abdomen & pelvis helps detect metastatic disease, if any, and also helps stage the disease (TNM) appropriately.
    • Bone Scan: Helps detect osseous metastasis (bone mets), if any.
  7. Treatment: Conventional treatment includes surgery, chemotherapy and radiotherapy as contextually appropriate. Surgery (i.e. gastrectomy either sub-total or total), with an adjuvant chemotherapy and/ or radiotherapy as contextually relevant, is the only treatment that is known to cure the disease in light of the prognostic indicators as briefly enumerated in the section below. Chemptherapy and/ or radiation alone cannot be curative. Mostly, it can improve symptoms, and may prolong survival. Simultaneously, an adjunctive or integrative naturopathic treatment with suitable complementary & alternative medicines (CAM) too can help improve clinical outcomes and facilitate recovery as feasible contextually.
  8. Prognosis: For gastric cancer is variable. Preventive measures, earlier diagnosis and right early treatment is key for an effective therapeutic management & better prognosis. Like most other cancers, the chances of cure for an early stage gastric cancer are more. The cure/ recovery chances are influenced by the grade, stage of cancer, recurrence and the patient’s general health & vitality etc all. Distal tumors are known to be cured more often than the proximal ones. Again, intestinal-type gastric cancers are known to have a better treatment outlook in comparison to the diffuse-type gastric cancers.

  9. Prevention: Rightly said, prevention is always a better choice. Although genetic risks are difficult to modify, still an adherence to a relative Mediterranean diet, maintaining an ideal body weight and an active lifestyle with due emphasis on regular exercising, de-stressing and relaxation is highly recommended for reducing the risks of many cancers including gastric cancer. A healthy eating plate comprises essentially a low fat diet, fibre rich foods including whole grain cereals, green leafy vegetables cooked using healthy vegetable oils, fresh fruits of all colours as seasonally available and healthy proteins/ fats including fresh fish, poultry, beans, nuts etc all. The consumption of alcohol, if any, has to be strictly in moderation, and is best avoided in a high risk scenario. Smoking is to be avoided too. Again, red meat, butter, refined grains, sweets, sugary drinks including carbonated beverages and other high calorie foods etc all, if any, are to be taken sparingly. Not only it is important to eat healthy, but also it is equally important to eat properly. Insufficient chewing, eating until full, eating meals within a short time etc all are best avoided so as to ease off digestive burden on the stomach/ other organs in the GI tract. Last but not the least, consumption of clean and filtered water, natural probiotics like freshly prepared yogurt/ butter milk, maintenance of cleanliness & hygiene including oral hygiene etc. all can help guard against H. Pylori infections. Breastfeeding is known to be protective against H. Pylori infections too.

Since 4 days I am getting discomfort while sitting or lying down in bed due to pain in anal rectum. Swelling is there and lot of pain. I am talking pain killers to reduce the pain. Dear lybrate user, you should take the following medications as directed:- 1) 5 drops of homoeopathic sulphur 200 once in a week. 2) 5 drops of homoeopathic aesculus hip 30 at early morning daily before brushing teeth 3) 5 drops of homoeopathic podophyllum 30 before going to bath daily. 4) 5 drops of homoeopathic hamamelis 30, just one hour before dinner daily.

I am 25. Hi Mixing two teaspoonfuls of ghee in a glassful of milk and taking this once a day acts as a liner inside the gut and will heal all the destruction caused by the high flow of Pitta. Very much gas formation after eating food. Very uncomfortable. I am continuously taking medicine for last two weeks LUPIZYME and METROGYL 400. Even after 2 weeks. Its. NOT CURED. This query is regarding my father. As of now his age is 59 years and the problem which I am going to discuss occurred in mid 2015. Previous History: Gas in stomach, mucus formation, piles, cataract. Can not have high doses of medicines hence relies heavily on Homeopathy medicines History of events: April 2015: He underwent Cataract operation and was on medication. Was constantly in touch with doctors because he was not feeling comfortable (weakness, pain etc). Heavy doses of painkillers and antibiotics. May 2015: He was alone in home. Was suffering from fever. In the night when he was asleep at around 12" 0 clock he fell from the bed. He remembers hurting his "neck" and then he became unconscious for about 1-2 hours. When he got back to senses, he recollected himself but again fell unconscious. After some time when he woke up he managed to call others. In the morning he had multiple "vomiting" He felt very weak. But then some how things got controlled. After this event his health deteriorated drastically. Though he is 59 years in age, he seems to be of nearly 70 years. We have consulted various doctors across India but they could not infer anything. Existing Problems: Some time in 3-4 days he feels good and fresh in even standing His neck movement is restricted. He does not keep it down (like reading book) for long hours. His memory has also suffered. He gets tired too early (before this incident he used to work very hard) Medical tests carried out: CT Scan, Blood Tests, ECG etc. Only MRI was left out because he is 'phobiac' about it and will not get it done. As CT Scan reports also did not reflect anything we also did not insisted. Findings: Everything was normal. Doctors also have not said anything. But while doing ECG, the specialist had said "there is something unusual. It is very minute thing, but it is. Though overall it is completely normal" Current Medication: He was on antibiotics and other things till Oct 2015. But since it wasn't working and no one was able to diagnose, so he switched to Homeopathic medicines. The doctor is giving him medicines related to heart problems and he is better than before, but his health is declining. Sorry for the long post, but pls suggest me how to get diagnosed properly. Our focus is on Diagnosis first. Would humbly request your suggestions. Thanks.

Hello, take Berberis vulgaris Q, 15 drops with water thrice daily. Take Cholesterinum 3X , 2 tabs twice daily. Home remedies :- 1. The acidic nature of apple cider vinegar stops the liver from making cholesterol that is responsible for forming the most common type of gallstones. It also plays a key role in dissolving gallstones and alleviating pain. Mix one tablespoon of apple cider vinegar in a glass of apple juice. Drink it whenever you have a gallstone attack. This will significantly ease the pain within 15 minutes. you can add two teaspoons of apple cider vinegar and one teaspoon of lemon juice to a glass of warm water. Drink it on an empty stomach in the morning. Doing this regularly for weeks can dissolve gallstones and prevent pain. 2. Pears can also provide relief from gallstone pain and other symptoms. The compound pectin present in pears helps soften cholesterol-filled gallstones, which in turn get flushed out of your body easily. Mix half a glass of pear juice with half a glass of hot water. Mix in two tablespoons of honey. Drink this juice three times a day for two weeks. peel and de-seed some pears. Cut the pears in small pieces and cook them in water for about 20 minutes. Eat a bowl of the warm cooked pears twice daily for two weeks.

Here are some home remedies to cure acidity.  - Skip the aerated drinks as well as the caffeine. Opt for herbal tea instead.  - Have a glass of lukewarm water everyday.  - Include banana, watermelon and cucumber in your daily diet. Watermelon juice is great for curing acidity.  - Nariyal paani is known to soothe the system if you suffer from acidity.  - Drink a glass of milk - everyday.  - Have your last meal at least two to three hours before you hit the sack.  - Keeping long intervals between meals is another cause for acidity. Have small but regular meals.  - Try to avoid pickles, spicy chutneys, vinegar, etc.  - Boil some mint leaves in water and have a glass of this after meals.

Avoid outside spicy and fried food. Walk about 3-5 kms daily. There should be gap of 2hrs after dinner and sleep. Small and frequent meals is of help chew your food properly at time of lunch and dinner. Keep your blood sugar controlled. Raise your bed about 4" from headend side. Get your gastroendoscopy done. Tab nexpro-l 1 tab empty stomach.

Hello lybrate-user! For colitis there are several dietary restrictions according to the severity of the condition. Basic rules that you have to followis to take easily digestible freshly prepared warm food. Avoid all the raw food i.e salads and fruits. Completely avoid curd and milk products. Along with all the above precautions you should start Ayurvedic treatment to cure your problem.

Take care of your eating habits. If you feel burning in stomach, then you better is to take fruits and reduce regular meals. Sometimes, simply eating less takes care of stomach trouble. Please make it a point that too bitter, too sour, salty, hot, pungent, dry and burning foods are to be avoided as such foods cause distress, misery and disease. Thus deep frying foods or foods taking lot of oil as in gravy vegetables, salty foods, spicy foods, long preserved, frozen foods, junk foods like pizzas burgers chips are to be avoided. Drink two glasses of warm water on empty stomach every morning about 30 minutes before breakfast. Try it! this works amazingly! in addition, walk fast for 30 minutes every day or engage in some service every day. Please also do meditation regularly and sincerely which has been scientifically proved for overall health. Do transcendental meditation by reading iskcon books from the internet vedabase or get the books hard copy from your nearest iskcon center.
