I have pain near buttock n anal. But it feels like there is a tumor over there. When I touch it pain. I can feel the difference between both buttocks. What it could be. What type of doctor I should be concerned.in my precious query I did not mention anything about any kind of tumor. please help.

MBBS, MS - General Surgery

Anal pain also known as proctalgia common causes:- 1.anal fissure-An anal fissure is a small tear in the skin of the anus that can be caused by passing a large or hard poo. Symptoms of an anal fissure can include: a severe, sharp pain when doing a poo a burning or gnawing pain that lasts several hours after doing a poo rectal bleeding – you may notice a small amount of blood on the toilet paper after you wipe Anal fissures can be very painful, but many heal on their own in a few weeks. Increasing the amount of fibre in your diet, drinking plenty of fluids and taking laxatives and over-the-counter painkillers can help. If the pain persists, you may need special ointment that relaxes the ring of muscle around your anus. Occasionally, surgery may be needed to help the fissure heal. 2.Haemorrhoids (piles) Haemorrhoids (piles) are swellings containing enlarged blood vessels that are found inside or around the bottom. They're often thought to be caused by straining on the toilet as a result of prolonged constipation. In many cases, haemorrhoids don't cause symptoms. When symptoms do occur, they may include: bleeding after doing a poo an itchy bottom feeling like there's a lump in or around your anus soreness and redness around your anus anal pain, if the blood supply to the haemorrhoid becomes blocked or interrupted – for example, by a blood clot The symptoms often pass after a few days. Increasing the amount of fibre in your diet, drinking plenty of fluids and taking laxatives and over-the-counter painkillers can help. If the blood supply to the haemorrhoid has been blocked by a clot, a simple procedure can be carried out to remove the clot under local anaesthetic (where the area is numbed). 3.Anal fistulas and abscesses An anal fistula is a small tunnel that develops between the end of the bowel and the skin near the anus. It's usually caused by an infection near the anus resulting in a collection of pus (an abscess). Symptoms of an anal fistula or abscess can include: a constant, throbbing pain that may be worse when you sit down skin irritation around the anus passing pus or blood when you poo swelling and redness around your anus a high temperature (fever) Your GP may prescribe antibiotics if an abscess is picked up early on. If it persists, it may need to be drained in hospital, possibly under general anaesthetic (while you're asleep). If a fistula develops, surgery will usually be needed because they rarely heal by themselves Less common causes of anal pain When to get medical advice Many common causes of anal pain will improve with simple self-care treatments, so you don't always need to see your GP. But it's a good idea to see your GP if: your pain is severe your pain doesn't improve after a few days you also experience rectal bleeding Don't feel embarrassed to see your GP – anal pain is a common problem that they're used to seeing. Your GP can try to work out what the problem is and give you treatment advice. They'll probably ask to see your bottom and may carry out a rectal examination (where they gently insert a gloved finger into your bottom) to check for any abnormalities. If the cause is not immediately obvious, they may refer you to a specialist for advice and further tests.

Diet containing lot of green vegetables & roughage and fluids, avoiding spices & chillies, will do a lot of good. A regular physical will also exercise. Don't worry about fluid that comes out. It is the extra mucous. If there is painful defecation and / or blood during defecation, the fissure might be active and that will need to be seen by a surgeon, physically.

Hi. Thanks for sending query in lybrate. I understand your concern. From your description the duration of the problem is not clear to me, neither you have mentioned anything about bleeding per rectum or weight loss, still I will give you most probable differentials: 1) prolapsed rectum 2) prolapsed hemorrhoids 3) a polyp or less likely a lower rectal growth kindly visit a surgeon/surgical gastroenterologist and get yourself evaluated clinically by doing per rectal examination, proctoscopy, colonoscopy and also a cect if the treating consultant feels the need for the above. If the diagnosis is haemorrhoids then open/stapler haemorrhoidectomy as per your treating consultant's advise. If it is a rectal prolapse which looks more probable repair through abdominal approach or perineal approach or laparoscopic repair as per your treating surgeon. The names of few methods are delorme's method, altemeire's method, lahaut's operation, anal encircling by steel wires or laparoscopic repair. If there is a polyp a biopsy has to be sent either colonoscopic or from ot. If biopsy shows malignant potential polypectomy with apr where the rectum, sigmoid and soome extent of descending colon will be resected and a permanent colostomy to be done. If colonoscopy shows multiple polyps then total proctocolectomy with ileoanal anastomosis to be done as polyps are premalignant. If unfortunately it turns out to be a carcinoma rectum apr has to be done. Hope the consultation was beneficial regards.

Hello doctor.I m worried about a small lump I developed at the opening of my anus which was result of me passing a dry and hard stool with pain.Pls advice how it can b treated. The pain has got lighter in the last one day.I applied an ointment. Thanks

Fellowship of the Royal College of Surgeons (FRCS), MS - Urology, MBBS
Urologist, Ahmedabad
Dear , This is very likely to be perianal haematoma which occurs sometimes after passage of hard faeces. If pain has decreased then it would probably subside. It is better if you take Sitz bath once or twice a day: sitting in a tub full of lukewarm water with a few drops of dettol in it. This will help to subside it quickly. However if pain increase and takes throbbing character then a course of antibiotics should be taken. If pain or throbbing becomes severe enough to disturb your sleep at night then contact a proctologist or a general surgeon. Dr. Kiran Mehta.

GERD / Heartburn - 3 Reasons Why You Should Not Ignore it

MD - Paediatrics, MBBS
Pediatrician, Faridabad
GERD / Heartburn - 3 Reasons Why You Should Not Ignore it

Heartburn is a common symptom experienced by many people, but is often overlooked as indigestion. Besides making you uncomfortable, this can be a sign of a much bigger health problem. While the occasional heartburn can be treated with antacids, chronic heartburn can be a sign of gastroesophageal reflux disease (GERD) or acid reflux.

Here are three important reasons you should not ignore heartburn

  1. It can promote tooth decay: The acid regurgitated might not necessarily stop in the esophagus. It can often also make its way into your mouth. When this happens frequently it can promote tooth decay and wear out enamel. This can also leave your mouth with a sour taste. Acid reflux induced tooth erosion can easily be prevented by the early treatment of acid reflux, but in most cases is noticed only after the damage has been done.
  2. It can cause cancer of the esophagus: Chronic acid reflux that persists for years can lead to changes in the cells of the esophagus. This can lead to a cancerous condition known as Barrett's esophagus. The condition has no other symptoms can if left untreated can develop into cancer of the esophagus. Women are at a higher risk of this form of cancer than men. Though esophageal cancer is relatively rare, it is not something to be trifled with. The survival rate of esophageal cancer is very low in comparison to other forms of cancer. Only 15-20% patients live longer than 5 years after this condition is diagnosed.
  3. It can lead to breathing problems: Heartburn is caused by stomach acid flowing back into the esophagus. If it is inhaled after regurgitation, it can worsen asthmatic conditions and pneumonia. At the same time, treating acid reflux with medication like PPIs can also increase the risk of pneumonia as it can promote bacterial growth and suppress coughing. Thus, when consulting a doctor for acid reflux treatment ensure that you mention any existing asthmatic conditions so that you can be treated with your lung functioning in mind.

In addition, acid reflux can also impact a person's health and quality of life. To prevent heartburn people often find themselves avoiding certain foods. This can impact their social lives and in turn make them withdrawing into themselves. A person's sleep can also be affected by heartburn. Hence, it is important to treat heart burn as early as possible and not wait for it to become a chronic problem.
