Someone in serious state of depression needs immediate help and attention as they may have risks such as suicidal ideation. Kindly consult psychiatrist as they may consider hospitalization to improve the condition until the condition has improved to the extent that he or she is no longer suicidal. Drug addiction is a condition, which is abnormal or pathological and occurs due to excess use of alcohol, drugs, smoking or other such addictive substances. The different effects of addiction are acute drug use, drug-seeking behavior, relapsing vulnerability, slow response to natural stimuli, and last but not the least, low motivation in life.

Drug addiction is regarded as a community disease as it leads to crimes, violence, failure in school, child-abuse, the loss of productivity and domestic violence. That is why it is very important to eradicate this problem from the society; especially the teens should be educated on the same.

Madatya in Ayurveda
In Ayurveda, the problems of drug addiction is dealt under the heading of Madatya, meaning ‘intoxication’. The state of individual occurs due to continuous use of drug, alcohol, tobacco or any substance of high intoxication. Madatya is relative to the third and fourth stage in the addiction process i.e., drug dependence and drug abuse. Madatya kills the will to live a happy and healthy life because it is a total opposite of ‘ojas’, which means a source of energy. Madatya and wellness can’t go hand-in-hand in a body.

Addiction and doshas
The Ayurvedic states that the doshas are aggravated by the overdose of drugs into one’s body. They are Vata (Air), Kapha (Water), Pitta (Fire) or a combination of all three doshas termed as Tridosha.

Treatments in Ayurveda
In Madatya, Ayurveda balances the aggravated doshas. It first detects the doshas, and then treats them. Ayurveda uses herbal remedies to treat the symptoms, discards the toxins (ama) collected in the body and motivates and regenerates cells and tissues. There is a phase in the de-addiction process where the patient becomes confused because their intelligence level drops due to drug abuse. Ayurveda treats this problem with the use of herbs like Yashtimadhu (Glycyrrhizaglabra), Brahmi (Bacopamonnieri), Shankhpushpi (Convolvulus pluricaulis) and Guduchi (Tinosporacordifolia).

In Ayurveda, treatment of Sattva Guna, meaning the quality of goodness, is done to promote mental health. The patients are made to follow strict healthy practices like consumption of natural vegetarian food, practice Yoga, and meditation and follow a daily routine of less anxiety and stress. Activities like wearing neat and clean clothes, walking and spending time in natural flowery ambiance with good friends, listening to good music and all other forms of natural relaxants and entertainment are also a part of treatment. These will boost the soul and mind of the patients and make them grateful to God for their lives.

A family member is under treatment for schizoaffective disorder since 10 yrs. Lithium carbazepine risperidone clonazepam apripazole n ativan lorazepam for wen trouble sleeping. She has developed slowness of movement almost walks like penguin n feels rigidness tightness in hips n thigh thus trouble sitting n walking. Her psychiatrist told to do tests serum lithium n carbamazepine were under normal levels Vit b12 normal Only calcium slight lower than normal n vit d3 low Ortho consultation raised query abt extrapyramidal side effects of antipsychotic medication but psychiatrist refuted it. Ortho den gave gabapentin n mecobalamin n ca+vit d improved her walking n stiffness to an extent. I wanted to ask what is the reason for her walking problem n why did she get relief from gabapentin. We were asked to follow up after 15 days. If still stiffness n walking prob to refer neuro physician coz ortho cause was ruled out. I would like to know whats going on with her cause our docs didnt clearly explain d situation? N I cant afford to change docs.

Hi lybrate-user, I would like to say that the symptoms of slowness of movement what you have mentioned in a patient who is on Rispeidone and aripiprazole is most probably EPS (extra pyramidal Side effects) unless proved otherwise. There may be underlying or Co morbid Parkinsons disease. But in this position its difficult to diagnose as the Pt is on antipsychotic. Gabapentin has no role in decreasing antipsychotic induced slowness in movements. But its often used as a treatment of Akathesia which is also a kind of EPS. So indirectly gabapentin may decrease Slowness of gaits by working through GABA neurotransmitter system. I would suggest to change the antipsychotics if possible after discussing with your doctor. Preferably Quitiapine may be a good choice. Or anticholinergic medicine can be added to alleviate the side effects. Thanks.

Hi, When someone feels depressed it is important that they recognize the need to seek help. It could be from someone they trust within their own personal sphere but more importantly from a professional person. It is not uncommon for us to experience distress in life due to unreasonable remarks or actions of people around us. However, one should learn to be resilient and confident in one's self so as not to succumb emotionally. Please do visit a psychiatrist for further help. For depression there are treatment available in the form of medications and psychological therapies which are effective.

Dear user, medicine never get you out from ocd and depression. Its your will power that will help you. Don’t get dependent on medicines, change your life style, add some activities to your routine. Do yoga, exercises, walks etc. You need regular counseling sessions (CBT). Call me and discuss all the things in detail, tc.

Since you are already on medication I suggest you talk to your treating Psychiatrist and convey your current symptoms to them. It is possible that some changes in medication dosages will help improve the symptoms. As they are familiar with your history it is always better to have a discussion with them regarding this situation. Do not discontinue your medication or make any changes without consulting your treating Psychiatrist. You can try yoga, meditation, deep breathing and progressive muscular relaxation to help you feel better.

Repetitive tasks, intrusive and repetitive thoughts, checking, counting, disproportionate excess need for order and cleanliness, as well as anxiety when not able to do the same are signs of obsessive compulsive disorder. Levodopa - How To Maximize Its Affects For Parkinson's?

If you are being treated for Parkinson's disease. How can you maximize the effect of Levodopa? The effect of levodopa can be maximized by increasing its absorption from the digestive system. In particular, taking levodopa on an empty stomach is very important. 

  1. Take on an empty stomach: Do not eat anything 1 hour before taking levodopa. Do not eat anything for at least 30 minutes and if possible 1 hour after taking it. Take the pill with 2 glasses of water so that it can dissolve. If taking the levodopa on an empty stomach makes you nauseous, confirm that your medication has the right amount of carbidopa in relation to the levodopa (25%). If you continue to feel nauseous, you can eat a piece of white bread around the time that you take levodopa. Do not apply any butter, yogurt, cheese or similar high protein spreads. Protein can drastically decrease the absorption of levodopa. 
  2. Get your constipation treated: This is critically important too. Being constipated can slow down the passage of levodopa through the digestive system. In most patients, drinking adequate water and taking stool softeners is helpful. 
  3. Get your ulcer treated: It can be difficult to recognize that you have a gastric ulcer. However, if you have a burning sensation in your stomach at any time, or belch frequently, you may have a gastric ulcer. If you think you have an ulcer you should get tested (there is a simple breath test now) and treated with antibiotics (not antacids!) 
  4. Do not take levodopa with iron tablets: The iron binds to levodopa and prevents its absorption. Keep a gap of at least two hours between these two medications. 
  5. Try taking levodopa with orange juice and carbonated water (soda): Mix equal quantities of orange juice and carbonated water (soda). Take your regular levodopa with a glass of this preparation instead of water. The carbonated water hastens disintegration of the tablet, and the acidity of both things helps in levodopa absorption. There are special tablets of levodopa (dispersible levodopa, Madopar) which can be dissolved in this preparation. Talk to your doctor about it.

Eating Disorders - Are You Suffering From Any One Of Them?

Eating disorders can be defined as conditions that influence your eating behaviour significantly and which may give rise to serious health issues. Eating disorders are associated with behaviours like compulsively counting calories, puking after meals, living on a rigid diet etc. which often leads to a feeling of self-loathing and obsessive thoughts about food and weight.

Here are the three most common eating disorders and the symptoms by which they can be identified.

1. Anorexia nervosa - This disorder is considered to be a mortally dangerous condition in which the body weight reduces drastically. An individual is terrified about gaining weight and can take extreme steps to maintain their weight and shape, often leading to starvation, vomiting and many other health problems.

Common symptoms include:

  1. Reducing the amount of food to be consumed to cut down the calories
  2. Exercising beyond capacity to burn calories
  3. Recurrently monitoring your shape, weight and appearance
  4. Skipping meals
  5. Avoiding social gatherings where you will be offered food
  6. Irregular menstrual cycles in women
  7. Erectile dysfunction in men

2. Bulimia nervosa - Mostly observed among people in their mid-teens, Bulimia nervosa is a serious type of eating disorder associated with episodes of bingeing and purging with the individual being unable to regulate their eating habits. Moreover, the person suffering from this disease often feels guilty and horror-struck about weight gain and forces himself/herself to vomit.

Normally, people suffering from this condition exhibit the following habits:

  1. Binge eating
  2. Use of laxatives or throw up forcibly to get rid of calories
  3. Feeling exhausted and guilty
  4. Experiencing irregularity in their menstrual cycles
  5. The inability to maintain normal weight, in spite of trying to eliminate calories

3. Binge-eating disorder - This disorder is characterised by overeating, eating even when you don't feel hungry and continuing to eat even after feeling full. People suffering from this disorder cultivate a feeling of guilt inside them for overeating but do not try to nullify its effect by vomiting or exercising intensely. Some of the most prominent symptoms of this disorder include:

  1. Eating alone to hide the embarrassment caused due to overeating
  2. Eating quicker than normal people
  3. Being overweight
  4. Binge eating at minimum one instance every week

Attachment Disorder - What Causes It?

While most children share a strong bond with their parents, especially the mother, some infants are unable to establish lasting relationships with their parents or caregivers. This is known as attachment disorder. It is a rare yet serious condition as it affects the child’s relationships at a later stage and also triggers delinquent behavior. Children, who have been abandoned, orphaned or are victims of abuse are the most vulnerable to this condition.

What causes attachment disorder?
The exact cause for attachment disorder is not yet understood. However, researchers have put together a number of viable hypotheses for this. One such hypothesis suggests that when needs of a child are ignored or when he begins to feel unwanted, the child begins to expect hostility, negativity and rejection. This is a learnt response and the child begins to feel that his needs are unwanted. With time, these responses make the child untrusting and he withdraws into himself to avoid social contact. This affects the development in the brain and causes attachment problems that affect the child’s personality and future relationships.

Risks associated with it
Some children are at a higher risk of attachment disorder than others. This includes

  1. Children living in orphanages or other such institutes
  2. Children whose caregivers are changed frequently
  3. Children who have had prolonged hospitalization
  4. Neglected children
  5. Children who have suffered from physical, sexual or verbal abuse
  6. Children living in extreme poverty
  7. Children born to parents with mental conditions or anger management problems
  8. Children born to a mother suffering from post partum depression

Symptoms
There are two types of attachment disorders; inhibited and disinhibited.

  1. Inhibited attachment disorder: Children suffering from inhibited attachment disorder are likely to be:
    • Detached
    • Withdrawn
    • Unresponsive to comforting ie. they shun all types of relationships and generally do not get along with their peers or caregivers.
  2. Disinhibited attachment disorder: On the other hand, children suffering from disinhibited attachment are likely to:
    • Seek attention
    • Display inappropriate behavior
    • Frequently break social boundaries
    • Be inappropriately familial and attached to selective figures.

Additionally, children suffering from either type of attachment disorder are manipulative, destructive, moody and lack compassion and remorse.
Effects of attachment disorder
If not managed properly in its early stages, this disorder can continue into adulthood. This leads to:

  1. Relationship problems with peers, friends and family members
  2. Low self esteem
  3. Depression
  4. Anger management
  5. Academic problems
  6. Unemployment
  7. They are also at a high risk for drug abuse and alcohol addiction.

I want to know if alcoholism and that too desi one can confidently stop drinking and refuse even when alone. He was in mumbai and never drank but now gone out of town and got again into it. And once starts drinking will not stop until his liver issue begins or does not lose all. Is there a confident solution towards this.

Dear lybrate-user, It is nice of you to think about alcohol use in Jagdish. May be, Jagdish can distract and postpone his craving for alcohol in Mumbai. Such distractions may not be available outside Mumbai. So he gets tempted and drinks. Once he lapses, it becomes a vicious cycle. It is better that he discusses these lapses, relapses and craving with a psychiatrist in Mumbai when he is sober itself. Sometime he needs to take anti craving tablets for 1 year or so along with relapse prevention strategies. You can help him in these consultations as co therapist.

Am facing issue in lack of presence of mind and concentration .Always attention and patience is not there while taking with others. Always absence of mind. This issue is there past a year .as suggested by doctor taken lexcita 5 mg for 3 months but no change please suggest some medicine.

Dear user, Anxiety disorders are a category of mental disorders characterized by feelings of anxiety and fear, where anxiety is a worry about future events and fear is a reaction to current events. These feelings may cause physical symptoms, such as a racing heart and shakiness. There are a number of anxiety disorders: including generalized anxiety disorder, a specific phobia, social anxiety disorder, separation anxiety disorder, agoraphobia, and panic disorder among others. Dear Lybrate user: Anxiety medications have their place. Prescription medications are effective last resorts, they're acceptable in combination with psychotherapy. But on their own, medicine becomes a crutch - something you rely on to cure your anxiety without actually making it possible to control your anxiety. When you say within in a month it sounds like that month is really important for you to not have anxiety and that might be causing you more anxiety. In terms of controlling withdrawal symptoms I recommend you to seek a clinical Psychologist and resolve the underline issues for what causing you anxiety at the first place. I had undergone all cardiac test, ECG, doppler, STRESS TEST etc. As I was feeling restless. Following that I had developed stress, so sleeping problems appeared. For the last 4-5 month I am taking antidepressant. For last few weeks I am having palpitations. I am having palpitations problem for the last 15-20 years. Dear lybrate-user, Have you tested for thyroid functions also? You have been having palpitations since 15-20 years. It was treated with Mirtaz 7.5 mg since 4-5 months. With treatment, you were better - less restlessness, better sleep and less stress. So Mirtaz is not the cause for frequent palps. If you consult your psychiatrist again, she/he will increase the dose, or change the antidepressant, or add another drug or therapy.
