Multimodal Pain Relief After Total Knee Replacement

Knee replacement can be extremely painful. Previously, opioids or narcotics were administered for pain relief. But excessive addition of opioids is not exactly effective for controlling pain.

Multimodal pain management has become an important part of the perioperative care of patients undergoing total joint replacement. The principle of multimodal therapy is to use interventions that target several different steps of the pain pathway, allowing more effective pain control with fewer side effects. Many different protocols have shown clinical benefit. The goal of this review is to provide a concise overview of the principles and results of multimodal pain management regimens as a practical guide for the management of joint arthroplasty patients.

Multimodal denotes administering two or more than two types of medications that work with different mechanisms. The following are the techniques used:

  1. Pre-operative Femoral Nerve Block: Prior to the surgery, a catheter is placed beside the femoral nerve for blocking it. This nerve is located in the upper thigh. Medication is delivered through the catheter for the nerve to be numbed for 24 hours. Thus, pain signals to the brain are blocked. This method reduces the use of narcotics and the consequent side effects.

  2. Patient Controlled Analgesia (PCA): This method is also known as ‘Pain Pump’. An intravenous pump is used to administer pain relief medications, such as oxymorphone or morphine, after the surgery. The control button of the machine could be pressed, by the patient for 6 to 10 times per hour. The machine is used for two post-operative days.

  3. Oral Medications: The oral medications include Non-Steroidal Anti-Inflammatory drugs or NSAID; such as Celebrex which is similar to aspirin, structurally. Alternatively, acetaminophen, such as Tylenol or its equivalent composition, can also be used.

  4. Acetaminophen: It acts on the Central Prostaglandin Synthesis and relieves the patient of pain through multiple mechanisms.

  5. Epidural Analgesia: It produces lower pain scores and involves less time for achieving physical therapy goals. However, this is subject to side effects such as dizziness, urinary retention and itchiness.

  6. Gabapentinoids: These medications include membrane stabilizers such as Gabapentin and Pregabalin.

The objective of multimodal treatments is to provide quick relief to the patient and immediately so. Earlier the rehabilitation, more successful will be the knee replacement surgery.

Sleep disorders are often associated with substance abuse. The problems in sleep arise as a result of direct effects of a particular substance. Substance induced sleep disorders can be caused by many drugs, regardless of whether or not the substance is medically prescribed, legal or socially acceptable. It can also be caused due to alcohol abuse.

If you experience problems in sleep on not having resorted to any substance or even if you are trying to quit, it is called ‘withdrawal’. The severity of the sleep disorder depends on the type of substance used, and not much on whether or not the drug is illegal. A number of prescription as well as non-prescription drugs are capable of causing sleep disorders. However, the severity of the sleep disorders tends to vary among individuals.

Problems in sleep that can be caused due to prescription drugs such as:

  1. Hormonal drugs e.g. oral contraceptives
  2. Drugs for high blood pressure e.g. beta blockers
  3. Inhalable respiratory drugs
  4. Steroids, such as prednisone
  5. Medications for attention deficit hyperactivity disorder
  6. Diet pills
  7. Seizure medications
  8. Some antidepressants

Non-prescription drugs that can cause problems in sleep:

  1. Medications along with caffeine: These may include many cold and cough medications.
  2. Pseudoephedrine
  3. Illegal drugs such as cocaine, amphetamines as well as methamphetamines.
  4. Nicotine is capable of disrupting sleep and reducing the total sleep time.

Alcohol is often regarded to be a calming drug or a sedative. While alcohol is capable of inducing sleep, the quality of sleep tends to be fragmented during the second half of the sleeping period. Consumption of alcohol usually tends to increase the number of times you wake during the latter part of the sleep. This is because the relaxing effect of alcohol wears off eventually. Alcohol tends to keep you in the lighter sleep stages more and thus, is at fault of not inducing the much required deep sleep.

If you consume alcohol just before you go to bed, the sleep inducing capacity might decrease as the effects that are disruptive continue to increase. The alcohol induced sleep disorders may lead to sleepiness and fatigue during the daytime. With age, these problems tend to grow in severity. Consumption of alcohol before bedtime, among the elderly, might lead to unsteadiness while walking with a higher risk of injuries and falls.

There are a few things in our lives which we don’t value until we lose it and our hair is one of those blessings that we are endowed with. For many such people, the hair transplant treatment can bring back the hair that they have lost owing to both external and internal factors. There are a variety of causes behind hair fall such as regular exposure to pollution and harmful UV rays. Also, our daily dose of stress and tension has a role to play in this respect.

What is a hair transplant treatment?
If an individual is bothered about the thinning of hair at certain places, then hair transplant is a treatment worth consideration. It will not only restore back the lost tresses but boost up the self-confidence as well.
Hair transplant is a kind of surgery which helps in moving the hair that you already possess to an area which has less or no hair at all. The treatment has been in practise since the 1950s in the United States of America although the advancement of medical science has contributed to making hair transplant much effective only in the recent years.

How is hair transplant done?
The hair transplant treatment is done at the doctor’s office where he removes a thin strip of skin measuring between 6 and 12 inches from the back of the head. This strip of skin is kept aside, and the scalp is closed and is instantly hidden by the hair present in the surrounding.

The next task of the surgeon is to divide the removed piece of scalp into 500 to 2000 little grafts where each of them comprises of a single or a few hairs. The type and the amount of graphs that an individual requires is based on the type of hair, quality, hair colour, hair condition and the size of the area where the hair is required to be transplanted. The actual process of hair transplant is all the same after this point. Once the graft is prepared, the surgeon would clean the area and numb the zone where the hair would be transplanted. Then slits or holes are created with the aid of needle or scalpel and finally the each graft is delicately placed in one of the holes. The whole process of hair transplantation may take 4 to 8 hours depending on the extent of treatment. An individual may require another session of the treatment if he or she decides to get thicker hair or if the problem of hair loss persists.

What you may expect after the treatment?
After the hair transplantation, the scalp may become very tender, and the doctor may prescribe am antibiotic and a painkiller to alleviate the condition. It is normal for the transplanted hair to fall out but the new growth of hair should start within a few months after the treatment. Most people who are treated with hair transplant notice 60 percent of renewed hair growth within 6 to 9 months after the treatment.

Hi, hair growth has it's own cycle. This most often cannot be changed but factors like inappropriate nutrition can affect it. Please take lots of sprouts (half baked better), walnuts, almonds and boiled egg (if you like). Minoxidil can only lengthen the growth phase but does not increase the speed of growth. Stress also delays growth. Sleep atleast 7 hours at a stretch and exercise for an hour daily.

Please apply adapalene containing creams on alternate night on nose. But please protect your skin with sunscreen during daytime. Avoid oily food, sweets, chocolates and junk. Have more of green leafy veggies, carrot, tomato. Use a mild moisturizing soap to wash face.

Hello. Can ecg and 2d echo test detect arteries blockages? Due to high blood pressure my doctor suggested me to do ecg, 2d echo, chest xray, lipid profile, thyroid,diabetes, tests 3 months ago and all were perfectly normal. These tests tell the condition of the heart at the moment. It is not possible to rule out if blockages are there and whether those blockages are significant with Echo and ECG unless a herat attack has already happened or is happening. A TMT or a Stress Echo has a higher probability of suggestive if there are blockages which may be harmful. So you can get either of tests done if you are over-concerned.

Diet is very important if you want to reduce weight because weight loss is 80% diet and 20% sweat. Working out alone is not going to help you reduce your weight. I do not deny the fact that exercise helps to reduce weight, but diet plays a much more important role in weight loss.

If you want to be healthy then make sure that, you eat a well nourished and well-proportioned diet as too much of something is not good. If you eat too much then you will become obese which will lead to all kinds of health problems like blood pressure, diabetes, cholesterol, arthritis, back ache, etc.

The above quote is indeed true and gives a very good message, so watch what you eat.

Avoid processed and canned food because, they are filled with sugar and salt, which act like preservatives and are very bad for health. The fresh packaged fruit juices, which we drink contains high amounts of sugar and the frozen food and other ketchup’s and sauces, which we consume, are filled with fat, MSG, sugar and salt, which are quite dangerous to our health.

Make sure that you include lots of fruits and vegetables in your diet and avoid oil as much as possible. Fruits and vegetables help to detoxify your system and oil is necessary for our body to function but too much of oil is very bad. Make sure that you fast at least once a month to give rest to your digestive system.
