Know What Alcohol Can Do To Your Liver!

MBBS, MD - Medicine, Member of the Royal College of Physicians, UK (MRCP UK)
Gastroenterologist, Delhi
Know What Alcohol Can Do To Your Liver!

If you are in the habit of consuming alcohol excessively and notice redness on the palms of the hands or pale stools, then it might point to liver disease. These are one of the many symptoms of alcoholic liver diseases which is the collective terms for liver diseases occurring due to alcohol overconsumption which includes fatty liver, hepatitis, and cirrhosis. Read on to know about this condition in brief.

Causes and The Risk Factors
The main cause of the disease is overconsumption of alcohol though not all of the heavy drinkers develop this disease and the cause is not clearly understood. But it is found that a toxic chemical called acetaldehyde produced by alcohol damages the liver leading to its scarring and ultimately resulting in cirrhosis or an end state disease of the liver.

It is also noted that the people who consume liquor and beer or spirits are at greater risk of developing the alcoholic liver disease as compared to the ones who consume different alcoholic beverages like wine. Further, studies have revealed that women metabolize alcohol slower as compared to men and hence they are at a greater risk of developing the disease. And women who consume alcohol more and at the same time are on the heavier side have a high chance of developing a chronic liver disease which might even lead to death. Genetic factors increase the chances of developing a disease of the liver too.

Diagnosis
If the alcoholic liver disease is suspected, tests such as complete blood count, liver biopsy, liver function tests and coagulation studies might be conducted to confirm the diagnosis and proceed with the treatment.

Prevention and Treatment
For the prevention of alcoholic liver disease, the most critical step is to stop the intake alcohol completely. If you stop consuming alcohol, the risk for the rapid acceleration of the liver disease stops and then you can proceed with the further treatment procedures. If you cannot give up alcohol altogether, it is recommended to limit the intake of alcohol to only one or two drinks per day but is best to stop completely.

The causes and the risk factors of the alcoholic liver disease vary from one person to another, and similarly, the procedures of treatment also vary depending on the condition of the liver of the affected person.

Medications are used to treat the liver disease which depends on the condition of the liver. The healthcare professional decides the course of medication as per the requirements. In some severe cases, the doctors are left with the only option of liver transplantation which is the only definitive therapy in modern medical science, but that involves a lot of risks and other complications. So when it comes to alcoholic liver disease prevention is better than cure.

Spondyloarthritis

MD(Am)
General Physician, Secunderabad
Spondyloarthritis

Spondyloarthritis (also known as spondyloarthropathy) is the term which describes the connective tissue diseases. It is the group of inflammatory diseases, including arthritis of the peripheral joints and spine (sacroiliitis or spondylitis); including the area where ligaments and tendons attach to bones (enthesitis or enthesopathy). These diseases cause pain or stiffness in the leg or arm joints, spine, ligaments and tendons become inflamed. Skin rashes, eye, and intestinal problems may also be able to occur.

Epidemiology

  • Spondyloarthritis is a pathology that specifically strikes young people. The symptoms most frequently start before the age of 45. It affects more males than females.
  • Predisposition to spondyloarthritis, especially spa, is determined largely by genetic factors.
  • The incidence rate is higher in populations with a higher prevalence of hla-b27.
  • Psoriatic skin lesions and colitis due to inflammatory bowel disease (ibd) have been considered as both basic, subtype-defining entities with their own genetic background (distinct from hla-b27 genotype), and as manifestations of spondyloarthritis.
  • There is a strong need to diagnose patients with spa in an earlier stage; currently, there is a delay of 5–10 years between onset of the first symptoms and diagnosis.

Types

There are five types of spondyloarthritis

Ankylosing spondylitis or bechterew disease: it is the type of arthritis that affects the spine. Symptoms include pain and stiffness in the lower back and in the neck. Spine bones fuse together and it will become rigid spine and it leads to stoope over posture.

Psoriatic arthritis: psoriatic arthritis is the form of arthritis that affects the people who have psoriasis. Most people diagnosed with psoriasis and later diagnosed with psoriatic arthritis. It may affect any part of the body such as fingertips and spine. The main symptoms are joint pain, stiffness, and swelling.

Psoriatic arthritis

Reactive arthritis: it occurs due to the infection by certain bacteria. Often the bacteria’s from genitals (chlamydia trachomatis) or bowel (campylobacter, salmonella, shigella, and yersinia). It usually targets your knees and joints of ankles or feet.

Reactive arthritis

Undifferentiated arthritis: undifferentiated inflammatory arthritis does not conform to any of the recognized inflammatory arthritis types. It may include monoarthritis of a single joint; oligoarthritis, which is usually when four or fewer joints are involved; or polyarthritis, which usually involves many small joints.

Risk factors

Risk factors may be higher due to following reasons:

  • Family member having the gene which causes spondyloarthritis
  • Positive results for the hla-b27 gene
  • Recurring bacterial infection in your gut
  • Have other inflammatory conditions such as psoriasis or inflammatory bowel disease

Causes

  • The main cause is not known, but researchers pointed out that the hereditary genetics playing an important role in this disease. Since this disease occurs often in family members of patients who have spondyloarthritis.
  • Many genes cause it. Up to 30 genes have been found. But the major gene that causes spondyloarthritis is hla-b27.
  • Bacterial infections can also cause reactive spondyloarthritis known to be triggered by a bacteria chlamydia or food borne infections.

Symptoms and complications of spondyloarthritis

The first and major symptoms of all types of spondyloarthritis are severe low back pain, swelling of arms, stiffness, and fatigue. The different types of spondyloarthritis can be manifested by the following signs and symptoms.

  • Alternative or unilateral buttock pain
  • Arthritis in small joints
  • Arthritis in large joints such as knee joint, ankle joint and elbow joint (peripheral arthritis)
  • Numbness and tingling sensation in arms and legs
  • Spinal fusion may occur in ankylosing spondylitis
  • Inflammation at the interfaces between bone and tendon or ligament of limbs (enthesitis)
  • Inflammation of intestine, urinary tract and aortic heart valves. These can lead to spinal joints inflammation.
  • Inflammation of joints in pelvis and spine (sacroiliitis)
  • Enteropathic arthritis shows symptoms such as diarrhea and abdominal pain
  • Digestive problems
  • Skin disease like psoriasis
  • Osteoporosis
  • Redness of eye (iritis)
  • Swollen fingers and
  • Pain in the heels cause difficult to walk, stand even for small duration

Diagnosis and testing

First a doctor may ask for patient’s medical history, physical exam and may suspects for symptoms of spondyloarthritis. To confirm doctor may perform the following diagnosis and tests.

  • X-rays: x-ray images shows the change of the sacroiliac joints in the pelvis and fusion of spine. If these changes not observed, but symptoms shows spondyloarthritis doctor will suggest mri scan. Mri shows these changes more promptly than the x-ray.
  • Mri scans: it can show a very clear image of enthesitis and also the changes of sacroiliac joints.
  • Ct scans: ct also used for the imaging of bone joints.
  • Hla-b27: most patients with spondyloarthritis have hla-b27 gene, but some people don’t have this gene. So it is necessary to go for a hla-b27 test if the symptoms suspects spondyloarthritis.
  • Complete blood count (cbc): cbc can be useful to find the inflammation.
  • Musculoskeletal ultrasound scanning: ultrasound imaging uses sound waves to produce the images of inflammatory conditions of muscles, ligaments, and tendons.
  • Dual-energy x-ray absorptiometry (dexa) scans: used for measuring the bone mineral density usually the bones of the lower spine and hips. Bone loss is measured through this technique.

Treatment of spondyloarthritis.

  • There is no way to cure this disease condition, but there are treatments to counterparts the symptoms.
  • Nonsteroidal anti-inflammatory drugs: certain nsaid’s like naproxen, ibuprofen, meloxicam, or indomethacin are prescribed by doctors to relief from the pain due to symptoms.
  • Corticosteroid medication: inflammation around the tendons and in the joints can be overcome by taking some corticosteroid drugs. This helps to reduce the swelling in the joints.
  • Antibiotics: antibiotics can be used in the case of reactive spondyloarthritis in which bacteria cause the major problems.
  • Disease-modifying antirheumatic drugs (dmard’s): sulfasalazine (azulfidine) and methotrexate are the best dmard’s to be used for the joint damages in limbs affected by arthritis.
  • Tumor necrosis alpha (tnf-alpha) blockers: fda approved tnab are adalimumab (humira), adalimumab-atto (amjevita), a biosimilar to humira, certolizumab pegol (cimzia), etanercept (enbrel), etanercept-szzs (ereizi), a biosimilar to enbrel, golimumab (simponi), infliximab (remicade), infliximab-dyyb (inflectra), a biosimilar to remicade, and secukinimab (cosentyx). These are biosimilars used to treat arthritis in spine joints.
  • Surgery: patients with over inflammation of pelvis joints needs hip replacement. In the case of heavy damage in the cartilage of joints needs surgery to repair. In some rare cases, spinal surgery also needed.

Prevention and control measures

  • Prevention from spondyloarthritis needed some lifestyle changes as follows.
  • Quit smoking: smoking speeds up the spinal fusion in addition to other bad health effects.
  • Follow good posture: practicing a proper sitting and posture makes your spine to escape from growing into slumped-over position and strain.
  • Daily physical exercise: frequent home exercise and physiotherapy is essential to reduce the stiffening of spine and other joint problems. It also helps to have your heart healthy.
  • Good diet: follow the diets that are rich in calcium. Dairy products are preferred foe calcium rich foods.

Vaginal Discharge - Ayurvedic Treatment

Vaginal Discharge - Ayurvedic Treatment

Vaginal discharge is a normal physiological phenomenon, in which the consistency of vaginal discharge varies throughout the cycle. Healthy vaginal discharge is clear or white in colour, with normal and non-offensive odour. Sometimes right before menstruation, your vaginal discharge may hold a brownish tinge.

The consistency of the discharge varies mostly on the hormonal changes in the body. When the levels of oestrogen are low, the cervical mucus that is liberated as discharge may be perceived as sticky and thick. When the oestrogen levels rise up, the discharge becomes wetter, more stretchy and clearer. Also, after ovulation, the consistency becomes thicker again.

When should you worry about the vaginal discharge?

Greenish, greyish or absurdly brownish in colour. When the discharge appears thicker than normal. When the discharge is more than normal. When it smells absurd like fishy or rotten meat like smell, pus like discharge, white with cottage cheese like discharge. When the discharge is associated with bloody discharge and burning sensation, rash or soreness.

Diseases caused by vaginal discharge

  • Yeast infection: the vaginal discharge is cottage cheese like and may be associated with itching of the vulva. This is also called as thrush. It can be treated easily with ayurvedic medicines.
  • Vaginitis: yellowish offensive discharge, which may be profuse, associated with itching and abdominal pain.
  • Trichomoniasis: this disease is asymptomatic in most cases and you may notice purulent discharge.

Ayurvedic medicines and panchkarma procedures can manage it effectely. For taking consultations you can talk with our experts.

CHILD SEXUAL ABUSE

  • Child sexual abuse has been reported up to 80,000 times a year, but the number of unreported instances is far greater, because the children are afraid to tell anyone what has happened, and the legal procedure for validating an episode is difficult. The problem should be identified, the abuse stopped, and the child should receive professional help. The long-term emotional and psychological damage of sexual abuse can be devastating to the child.
  • Child sexual abuse can take place within the family, by a parent, step-parent, sibling or other relative; or outside the home, for example, by a friend, neighbor, child care person, teacher, or stranger. When sexual abuse has occurred, a child can develop a variety of distressing feelings, thoughts and behaviors.
  • No child is psychologically prepared to cope with repeated sexual stimulation. Even a two or three year old, who cannot know the sexual activity is wrong, will develop problems resulting from the inability to cope with the overstimulation.
  • The child of five or older who knows and cares for the abuser becomes trapped between affection or loyalty for the person, and the sense that the sexual activities are terribly wrong. If the child tries to break away from the sexual relationship, the abuser may threaten the child with violence or loss of love. When sexual abuse occurs within the family, the child may fear the anger, jealousy or shame of other family members, or be afraid the family will break up if the secret is told.
  • A child who is the victim of prolonged sexual abuse usually develops low self-esteem, a feeling of worthlessness and an abnormal or distorted view of sex. The child may become withdrawn and mistrustful of adults, and can become suicidal.
  • Some children who have been sexually abused have difficulty relating to others except on sexual terms. Some sexually abused children become child abusers or prostitutes, or have other serious problems when they reach adulthood.
  • Often there are no obvious external signs of child sexual abuse. Some signs can only be detected on physical exam by a physician.

Sexually abused children may also develop the following:

  • unusual interest in or avoidance of all things of a sexual nature
  • sleep problems or nightmares
  • depression or withdrawal from friends or family
  • seductiveness

Statements that their bodies are dirty or damaged, or fear that there is something wrong with them in the genital area refusal to go to schoolChild Sexual Abuse, “Facts for Families,” No. 9 (5/08)

  • delinquency/conduct problems
  • secretiveness
  • aspects of sexual molestation in drawings, games, fantasies
  • unusual aggressiveness, or
  • suicidal behavior.

Child sexual abusers can make the child extremely fearful of telling, and only when a special effort has helped the child to feel safe, can the child talk freely. If a child says that he or she has been molested, parents should try to remain calm and reassure the child that what happened was not their fault. Parents should seek a medical examination and psychiatric consultation.
Parents can prevent or lessen the chance of sexual abuse by:

  • Telling children that if someone tries to touch your body and do things that make you feel
  • funny, say NO to that person and tell me right away
  • Teaching children that respect does not mean blind obedience to adults and to authority,

For example, don't tell children to, “Always do everything the teacher or baby-sitter tells you to do.”

  • Encouraging professional prevention programs in the local school system
  • Sexually abused children and their families need immediate professional evaluation and treatment.
  • Child and adolescent psychiatrists can help abused children regain a sense of self-esteem, cope with feelings of guilt about the abuse, and begin the process of overcoming the trauma. Such treatment can help reduce the risk that the child will develop serious problems as an adult.

Detox Your Body

Detox Your Body

Detox Your Body Now.

Now as we celebrated Diwali yesterday, many of us had sweets and gracious sweets.

Honestly, it would be unfair to resist this indulgence, especially during this festive season.

Lemons – The acidic content in lemons will help you get rid of a heavy stomach feeling and bloating. Lemon also helps in smooth movement of food in our gastrointestinal tract. You can have lemon with lukewarm water on an empty stomach every morning.

Honey – Along with lemon and water, addition of honey helps in relieving constipation. This water will help in digestion and get rid of toxins in the body.

Spinach – Your body needs fiber. Festival binging often brings with it foods rich in fats and low in fiber. Because of this we often end up constipated. Spinach will help build up bulk in the body and clean the gastrointestinal tract.

Tomatoes – Yes, tomatoes are an underrated super food. Since during festive time we indulge in too many fried snacks, we end up consuming trans fats which are cancerous in nature. Eating tomatoes will help cut this cancerous effect in our body.

Cucumber – Cucumbers help produce more urine in the body which consequently means it secretes toxins out of the body.

Also, cucumber is a diuretic food which means it helps in excess losing water from our body.

Beetroot – Beetroot is known to cleanse our liver. Therefore, after a heavy alcohol night, blanch beetroots and have them.

Beetroots contain betalains which help neutralize toxins and flush them from our body through urine

Onions – Surprising as it may be, onions help cut toxins from other foods. This due to the antioxidants present in onions.

If you are a diabetic and indulged in heavily fried foods during Diwali, onions will help regulate blood sugar level.

Hi, No, you need not wait upto 10 days to get positive results. If your periods are regular and if it gets postponed for few days after marriage, urine test can be done on 31st or 32nd day itself. As your urine test is now negative, the other symptoms may be due to acidity. Kindly consult your doctor and get examined.

Can Inverted Nipples be a Sign of Breast Cancer?

Doctor of Medicine, DGO
Gynaecologist,
Can Inverted Nipples be a Sign of Breast Cancer?

Breast cancer is a type of a cancerous infection that develops in the skin cells of the breast. This condition is more commonly diagnosed after you have skin cancer and is more common in women than men. There are certain signs to identify this disease, which are as follows.

1. Breast lumps - Lumps are patches of skin that occur due to the thickening and swelling of the skin. Breast lumps is a very common phenomenon for women but that does not always lead to a breast cancer; however, frequent and reoccurring instances of the same increases the risk of breast cancer. A breast lump is easily identifiable through the naked eye as it looks different from the surrounding tissues.

2. Blood discharges - The nipple is one of the most sensitive areas in the human body. In case of women, it is meant to emit milk during pregnancy, in order to feed the infant externally. A probable symptom of breast cancer can also be a frequent discharge of blood from the nipple without any visible damage or injury.

3. Breast changes - If you are experiencing any visible changes in the size, appearance or shape of the breast, then it is an indicative sign that you may have breast cancer. Thus, it is of paramount importance that you observe, touch and physically examine your breast regularly for changes. If there is any major change detected then it is advisable to consult a doctor.

4. Inverted nipple - The tip of the nipple is generally bulging and pointing outwards both in cases of men and women. But sometimes the tip of the nipple is pushed inside due to excessive suction caused by the skin tissues present inside the breast around the nipple area. This suction forces the nipple to grow inwards which is abnormal and might harm the breast and skin in various ways.

However, apart from these main symptoms, some other signs are also there that increase your chances of being diagnosed with breast cancer. They are redness or pitting of the skin over your breast like an orange, peeling or scaling of the pigmented area around the nipple, dimpling of the skin on the breasts, and such others.

MD-Ayurveda, Bachelor of Ayurveda, Medicine and Surgery (BAMS)

Hello- The vagina pain can be due to weak body status. Your gf is underweight and she might also be lacking natural vaginal lubrication fluid needed during sex to avoid penis friction. Its better to improve her diet schedule, increase natural fats in her meals, avoid junk foods and spicy diet. Apart from it a good and healthy sleep is also must for her so, advice her not to miss the 10-2 sleep at night. You can also start a female aphrodisiac like Shatavari Kalpa 1 tsp with milk during breakfast. It will solve your issue.
