Hello, Thanks for the query. Please answer following questions before I can suggest any treatment for him. 1) Since when is he having diabetes? 2) Has it been always been not under control or only recently has the control gone down? 3) What is his HbA1c%? 4) The evening rise of body temperature has been seen since how long? 5) Did have Tuberculosis infection any time earlier? 6) Similarly getting breathless has been noticed since when? Get following investigations: 1) Full diabetic profile (Fasting, PP, HbA1c%, creatinine, urea, uric acid,), lipid profile, sputum test for AFB, chest x-ray Come back with all these details, or consult your local physician. His diabetes needs to be controlled first effectively. Ideally FBG <100 mg, PP 160 to 170 mg & HbA1c% <7, these are the standards to be reached. I await your response. Thanks.

Keto Diet and problem with high level of Uric acid & creatine. Is keto good for Indian people who usually eat rice and chapati Is keto diet good for fatty liver, diabetes patients and to burn tummy fat. I was on keto diet before I stopped to fix my uric level. I ate chicken for lunch and salads in the evening and I felt energetic and lose weight of more than 3 kg in a week. My question is should I go back and re-start my keto Diet where I only eat vegetables and less fat and carbohydrates to fix my fatty liver? What are the main things should I look when I am on ketogenic diet. Thank you.

Hi lybrate-user, I would not recommebd you to go on a ketogenic diet. If you want to maintain a long term health plan, try to keep your choices as simple and easy to maintain. If you have a fatty liver, switch to eating more of fruits and vegetables. If you want to burn tummy fat, there's no magic potion available. You have to engage in some form of physical exercise to maintain it lifelong!

Mr. lybrate-user, Thanks for the query. To control blood glucose or your diabetes in addition to appropriate anti-diabetic drugs, diet control and exercise are also needed. Since there is no mention of current blood glucose levels & HbA1c%, plus the drugs being taken with dosage, there is no way to give any opinion. As regards your weight, with 1.83 meters, height your weight is excellent. Your present BMI is only 23.29 kgs/M2, which is what is expected. Reducing the weight to 70 kgs is not required at all. For controlling diabetes, please come in a private conversation with above mentioned details, plus your diet pattern and extent of daily exercise. Thanks.

I am 21 years old Male. I have thyroid problem. Today I went for test. In blood test, the result is 24.3 of TSH test. What does it mean? M I alright? Or not?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, Thanks for the query. TSH 24.3 is very high, suggesting uncontrolled hypothyroidism. It means your current dosage of thyroxine too less, needs to be increased. If you allow the level to remain high, then all the symptoms of hypothyroidism will be back in full. Thanks.

Both white sugar and jaggery are derived from the same source – sugar cane. Sugar crystals have no natural nutrients while jaggery has essential nutrients like magnesium, calcium, phosphorus, potassium, iron, copper and zinc. Jaggery contains the same amount of sugar. Both white sugar and jaggery have a high glycemic index. The absorption of sugar in the body takes place at a faster pace and thus raises blood sugar levels faster whereas Jaggery has sucrose which is a complex sugar and takes time to break down when consumed, but sugar levels go up within a few hours. So its not advised to take jaggery for diabetics. Sugar in all forms should be restricted.

Mr. lybrate-user, Thanks for the query. Sugar and jaggery are the products made from sugar cane juice, hence both are containing sucrose. So there is no difference in their glucose content. Both are bad for a person with diabetes. Both will increase blood glucose rapidly. Thanks.

Lybrate-user, Thanks for the query. HbA1c% 7, means diabetes is confirmed. 6.5% is the value at which diabetes is diagnosed. So if you are not yet taking treatment, it is necessary for you to initiate the same. Plus you will have to take a restricted diet and exercise daily for over 1 hour. Thanks.

lybrate-user, Thanks for the query. Jaggery is made from sugar cane juice like sugar is made. So there is no difference in sugar and jaggery. Both are containing sucrose, hence eating either of these substances is bound to increase blood glucose level. Thanks.

Mr. lybrate-user, Thanks for the query. You will have to take proper anti-diabetes treatment from your local doctor. Plus take a restricted diet and exercise regularly for over 1 hour every day. If you are interested in getting treatment online then come back with following details: Fasting, Pp glucose levels, HbA1c%, lipid profile, present medications if any with dosage, dietary patter, extent of daily exercise. Based on those details I will be able to guide in controlling the disease. Thanks.

Can jaggery (gur) be consumed by person on medication for blood sugar in the range of 120+10 on fasting?
Mr. lybrate-user, Thanks for the query. Sugar and jaggery both are made from sugar cane juice only. Both are containing sucrose. There is no difference between them in terms of glucose contents. Only difference is in the process of making, jaggery has some anti-oxidants and may be some vitamins. But both are bad for a diabetic person. Thanks.

Im chalapathi naidu! I need to know the details regarding surgery for diabetes which I have seen in few newschanels! ...
Mr. lybrate-user, Thanks for the query. The surgery being talked about is actually for treating excess obesity. Thsese procedures are called as "Bariatric surgery" Usually performed in patients who are excessively obese (BMI above 40 kgs/M2).In some of these patients following the operation there is a major weight loss, which could be 30 to 50 kgs or even more depending upon earlier weight. This results in the cure of diabetes if the disease about 5 years old or less at the time of surgery. In others there is a better control achieved on blood glucose, because of loss of aft there is reduced insulin resistance. Plus the procedure reduces total food intake to a fraction of what was earlier. This itself helps blood glucose reduction. Thanks.

Thanks for the query. In hypothyroidism one has to avoid soy beans and foods containing soy ingredients. All other things are fine. Also avoid bakery items, pastries and foods made out of maida. Always use whole grain flour for chapati and roti. Consume more of green leafy vegetables and fruits. Thanks.

Mr. lybrate-user, Thanks for the FBG 119 mg, is in the pre-diabetes state. Whereas HbA1c% 8.95 shows presence of diabetes for quite some time and it is not in control. TGL value is slightly high, no need to worry about it. Plus you have mentioned eltroxin as the medication being taken, which means there is hypothyroidism also. As regards eating paratha and curds yes you can have both, but your over all diet has to be controlled. Total caloric intake has to be regulated based on your weight, extent of daily exercise and blood glucose levels. Since diabetes is there, you will have to avoid sweets, deep fried foods, junk foods, pastries, bakery items and foods made of maida. Consume more of green leafy vegetables, salads and certain fruits. Avoid colas, fructose corn syrup containing products and alcohol. Daily 1 hour of exercise is a must. Since you have not mentioned anything about treatment for diabetes, it appears youa are not yet taking treatment for that. You will have to initiate the treatment for that too. If you have any doubts please come back. Thanks.

Dear Lybrateuser, - Your postprandial blood sugar is on the borderline so increase the fibre content & proteins in your diet, reduce carbs, avoid fried, processed & junk foods, take a walk for 10-15 min after meals - you require to reduce weight & if you do so then as your sugar levels come down you may not require medication - the treatment depends on your tight blood sugar control by diet & exercise - monitor blood sugar as advised by your doctor & consult him regularly with your progress.
