Sciatica is probably one of those things which is vaguely familiar but a person is not fully aware of it. The sciatic nerve is one which is part of the spine and then spreads out over the legs. When this nerve gets pinched, it leads to sciatica, or pain of the sciatic nerve, which is tremendous! But, then, what exactly is a person to do in order to reduce the pain caused on account of sciatica?

The best answer for this is not medication, though it does have its uses, but rather, exercises which help the situation improve and reduce the risk of the person going mad due to the pain! One of the simplest exercises that a person can start with is known quite commonly as the knee to shoulder exercise. All that one needs to do is to lie down on the back and then, raise one knee and pull it across the body, with hands wrapped around it, so that it touches the opposite shoulder. This position of the knee touching the shoulder is to be held for about half a minute. About 3 repetitions need to be done and the same with the other side.

While it is important in the effort to relieve pain due to sciatica, it is equally important not to push oneself beyond natural limits, especially initially as this can cause injury. After all, more pain is the last thing which a person with sciatica is likely to want!

Since it is important to keep in mind the fact that consistency is one of the key secrets to reducing the sciatic pain, it is worth keeping in mind the standing hamstring stretch. What this exercise can boast of is the fact that it is one which can be done nearly anywhere. Also, it is pretty simple, too!

To do the standing hamstring stretch, one leg needs to be kept on an elevated surface which is not as high as the level of the hip. It is important to keep in mind that the leg is to be kept straight. The torso needs to then be bent towards the elevated leg. This process should be done slowly and again, maintained for about half a minute.

The sitting pose is useful to remember, as well. Sitting on the floor, one leg’s ankle should rest on the other’s knee. Like the standing hamstring stretch, the upper torso is to be bent towards the extended leg. With all these exercises, relief is not far away!

Cataracts are a common eye problem experienced by the elderly. In some cases, babies may also be born with cataract as a congenital defect or it may develop as a result of conditions such as diabetes. Cataracts can affect one or both eyes and is characterized by the clouding of the lens in the eye. This can make it difficult to see clearly and if not treated in time can lead to blindness.

Common symptoms of cataract include:

  1. Blurred vision
  2. Nearsightedness
  3. Changes in the perception of colour
  4. Night blindness
  5. Increased sensitivity to glares
  6. Double vision

An eye examination is the first step to diagnosing cataracts. To confirm a diagnosis the doctor will also dilate the pupil to check the lens. Most cataracts can be easily treated with surgery. This surgery involves the separation of the cataract from the lens and its removal after which an intraocular lens implant is placed in the eye. In cases where this cannot be used, corrective contact lenses or glasses will need to be used. One of the most popular methods of cataract surgery is known as phaco surgery or phacoemulsification.

This surgery involves making two, small incisions on the eye where the cornea meets the sclera. This is then used to create a circular opening in the lens through which a probe is inserted into the eye. Sound waves or laser is then used to break the cataract into small pieces. This is then removed from the eyes and an intraocular lens implant is put in its place. Stitches are rarely required for this surgery and it is usually performed as an outpatient procedure while the patient is under local anaesthesia. When treating cataract, both eyes are usually not addressed simultaneously.

This surgery is usually pain free. The operated eye may be bandaged overnight and the patient will be given a protective eye shield to be worn for a week. The doctor will ask you to get a check up 2-3 days after the surgery where they can evaluate the clarity of the lens, overall health of the eye, eye pressure and visual acuity.

Cataract surgery is generally considered to be a safe surgery but like any other surgery there are risks involved. Some of the possible complications of this type of surgery include:

  1. Pain and redness of the eye

  3. Swelling of the eye
  4. Discharge from the eye
  5. Flashes or floaters in the patient’s vision

Get her antenatal check ups done regularly specially BP and urine examination for proteins, blood test for sugar, hemoglobin and other tests to rule out infections which can affect the growing baby. Reduce salt intake total bed rest more potassium through citrus fruits like oranges and lemon juice your doctor may have to give her a diuretic medicine to drain the swelling by more urine take care.

What is Acne?

Acne is a chronic inflammatory disease of the pilo sebaceous follicles (i.e. Hair with oil producing glands attached to hair) characterized by comedones (blackheads and whiteheads), papules, pustules, nodules and ultimately leading to scars.

Who gets Acne?
Close to 100% of people between the ages of twelve and seventeen have at least an occasional whitehead, blackhead or pimple regardless of race or ethnicity. Acne starts between the ages of ten and fifteen years of age and usually lasts till twenty years of age; however, acne can persist into the late twenties or thirties or even beyond. Some people get acne for the first time as adults and this is known as adult acne.

What are the causes of Acne?

  • Bacteria: Bacteria that normally lives on the skin plays a role in acne development. The bacteria are known as propionibacterium acnes (p. acnes). The bacteria content can increase in people whose faces are not clean. 
  • Hormones: During the teen years increased levels of sex hormones causes the oil glands of the skin to produce increased amount of oil. These excess skin oils tend to bind with cellular debris causing a blockage and producing a fertile environment for the bacteria to grow. 
     

The bacteria causes inflammation, pus, swelling and redness. In adult acne, excessive hormones produced due to polycystic ovaries and other hormonal imbalances causing excess oil production and inflammation, which leads to acne.

What are the factors that precipitate or aggravate acne?

  • Stress- Stress is frequently implicated in aggravation of acne while acne itself induces stress. 
  • Sweat- Sweating in a hot and humid environment causes deterioration in 15% of acne patients. 
  • Menstruation- Pre menstrual flare of acne occurs in 70% of female acne patients. 
  • Friction and pressure- Friction and pressure from helmets, backpacks and tight collars can induce blackheads and papules. Pressure from face massages or facials can aggravate acne.
  • Pregnancy- Pregnancy has an unpredictable effect on acne. Pre-existing acne may aggravate or remit during pregnancy.
  • Diet- High glycemic load foods (sugar loaded and starchy foods), oily foods may aggravate acne.
  • Drugs- Oral contraceptives, anti tuberculosis medication, oral steroids, anti depressants may aggravate acne.

It is advisable to visit a board certified dermatologist for treatment of acne instead of over the counter treatment as the dermatologist treats the root cause of acne.

Mr. lybrate-user, Thanks for the query. I get a feeling that I had answered this or a very similar query, because the PP blood glucose level mentioned was 321 mg. If you are the same person, my answer is again same. There is no special breakfast for lowering blood glucose. Basically when we take any food (which contains carbohydrates) plus also proteins & fat it is bound to increase glucose level. All the items consumed in breakfast like paratha, dosa, idli, upma, poha, bread (including brown bread), cereals etc. All contain starch, hence blood glucose is bound to go up. Even taking only fruits also will increase some amount of glucose. Therefore, what is most important is to control blood glucose with appropriate medication, then take a restricted diet that provides exact amount of calories based on ideal body weight (in your case 75 kgs), extent of daily physical exercise and considering need for weight reduction. Plus increase intake of green leafy vegetables, salads and permitted fruits (about 100 g a day). Ideally in good controlled diabetes fasting blood glucose should b e<100 mg, PP 150 to 160 mg & HbA1c%<6.8. This will help in either preventing or postponing long term complications related to diabetes. Otherwise, in an uncontrolled diabetes all the complications can be seen to occur early and with an unexpected severity. If you are seriously considering achieving a very good blood glucose control (as mentioned earlier), then please come in a private conversation with current details of fasting, PP glucose levels, HbA1c%, dietary pattern and extent of daily exercise, then I will be able to guide, as in this communication we are supposed talk only about general guidelines without giving any prescriptions. Noncancerous growths of the muscle tissue surrounding the uterus are known as uterine fibroids. This is a common disease which about 70 to 80% of women contract by the time they are 50 years of age. The uterine fibroids can sometimes be very big and cause heavy periods as well as severe abdominal pain while at other times, uterine fibroids give no signs or symptoms whatsoever and go away on their own. This is why it is crucial to know what type of uterine fibroids you have and how to diagnose them. Here are the types of uterine fibroids and how to diagnose them;

Types
There are three main types of uterine fibroids. They are;

1. Intramural fibroids
The most common type of uterine fibroids are intramural fibroids. They typically appear in the endometrium and may grow larger which results in your womb getting stretched.

2. Subserosal fibroids
Subserosal fibroids are called so because they form on the serosa. The serosa is the outside of your uterus. Sometimes, Subserosal fibroids may grow so large that your uterus appears bigger on one side.

3. Pedunculated fibroids
Pedunculated fibroids tumors are basically Subserosal fibroids with a stem. A base which supports the tumor is called the stem.

Diagnosis
There are a number of tests done to diagnose uterine fibroids. They are;

1. Pelvic exam
A pelvic exam is a thorough inspection of a woman&#x2019;s pelvic area. The organs which are in the pelvic area include the cervix, ovaries, uterus and vagina. Normally, this and the next test in this article are enough to diagnose uterine fibroids.

2. Medical history
The history of your periods as well as the other symptoms you have will often be enough to diagnose the uterine fibroids. If your medical history is not enough, then you might need to undergo a pelvic exam.

3. Pelvic ultrasound
An ultrasound is when high-intensity sound waves are used to produce images of the pelvic area. This is only done when a pelvic exam and your medical history are not enough to diagnose uterine fibroids.

A number of problems when contracted can affect your life and the life of your partner, especially problems which are related to your physical relationship. Premature ejaculation is one such problem and it refers to the inability to retain the erection until the end of the intercourse and usually happens when the male ends up ejaculating sooner than planned or immediately upon penetration. Premature ejaculation is a condition that many men suffer due to the advancement of age as well as low pelvic strength and other ailments. Also, it can be caused due to stress and panic regarding one's performance. Whatever be the cause, it is an embarrassing situation that calls for help if it persists for too long. Here are a few tips to ensure that you gain better control over premature ejaculation.

  1. Deep breathing Exercise: Sometimes, too much of excitement can lead to a variety of problems including premature ejaculation. If this becomes a normal occurrence, you can try practicing deep breathing during the act of sex to calm yourself down and slow down the frenzy so that you are more attuned to your surroundings and partner rather than yourself and the panic you may be feeling. Taking deep and slow gulps of air can ensure that you focus on the act rather than getting caught up in the excitement.
  2. Medications: there are broadly two kinds of medical treatments - local application anaesthetic gels that numb the area around the tip of penis and tablets that prolong ejaculation time. One needs to consult a sexologist, psychiatrist or a urologist to know more about them.
  3. Stop and Start: This is a method that many sexologists recommend. The stop and start method is one that will have to be practiced individually so that you are able to retain more control over your ejaculation when you are with your partner. Basically, in this method, you are required to start the process of masturbation and stop just short of ejaculation. After doing this several times and on a regular basis, you will find that your brain reacts in the same way when you are having intercourse, which will delay the ejaculation to a normal period.
  4. Kegel: If you thought that kegel exercises are only for women, then you need to think again. Premature ejaculation also signifies low pelvic strength for many cases and you would do well to invest time in a few exercises like the kegel, which will build up your pelvic strength so that you are able to control premature ejaculation in a better way.

Persistent problems that are not fixed with these methods should be addressed with a visit to a doctor.
